An implant for deployment in select locations or select tissue for regeneration of tissue is disclosed. The implant comprising collagen and or other bio-resorbable materials, where the implant may also be used for therapy delivery. Additionally, the implant may be “matched” to provide the implant with similar physical and/or chemical properties as the host tissue.

Devices and methods for treating defects in the tissue of a living being

Many materials are used for sinus augmentation procedures. Anorganic bovine bone (Bio-Oss) has been reported to be osteoconductive, and no inflammatory responses have been observed with the use of this biomaterial. One of the main questions pertaining to Bio-Oss concerns its biodegradation and substitution by host bone. Some investigators have observed rapid replacement by host bone, while other researchers observed slow resorptive activity or no resorption at all. The aim of the present study was to conduct a long-term histologic analysis of retrieved specimens in humans where Bio-Oss was used in sinus augmentation procedures. Specimens were retrieved from 20 patients after varying periods from 6 months to 4 years and were processed to obtain thin ground sections. Bio-Oss particles were surrounded for the most part by mature, compact bone. In some Haversian canals it was possible to observe small capillaries, mesenchymal cells, and osteoblasts in conjunction with new bone. No gaps were present at the interface between the Bio-Oss particles and newly formed bone. In specimens retrieved after 18 months and 4 years, it was also possible to observe the presence of osteoclasts in the process of resorbing the Bio-Oss particles and neighboring newly formed bone. Bio-Oss appears to be highly biocompatible and osteoconductive, is slowly resorbed in humans, and can be used with success as a bone substitute in maxillary sinus augmentation procedures.

Bone reactions to anorganic bovine bone (Bio-Oss) used in sinus augmentation procedures: a histologic long-term report of 20 cases in humans.

Background: Extraction socket wound healing is characterized by resorption of the alveolar bone at the extraction site. This produces a decrease in ridge volume, deformations of ridge contours, and, thus, difficulties in delayed placement of root-form implants in an ideal position. Cancellous porous bovine bone mineral (PBBM) applied to fresh extraction sockets has recently been proposed to minimize the reduction in ridge volume. The aim of this study was to investigate the influence of PBBM grafted particles on the histopathologic pattern of the intrasocket regenerated bone and to evaluate histomorphometrically the healed PBBM grafted extraction socket site at 9 months' post-extraction. Porous bovine bone mineral in healing of human extraction sockets. Part 1: histomorphometric evaluations at 9 months.

Forces during the phase of occlusal contact during chewing and swallowing are surprisingly high (36.2% and 41%), about 40% of the subject's maximum biting force. Previous studies using transducers in fixed partial dentures measured only a portion of the total force and have given the impression that chewing forces are much less than the data reported in this study. The importance of occlusal stability in the intercuspal position is of utmost clinical significance. Steep anterior guidance does not appear to expose the teeth to extreme lateral forces. The gliding contacts of the teeth while entering and leaving the intercuspal position have been shown to be of short duration and low magnitude when compared with the forces generated in the intercuspal position. During chewing, the peak occlusal force occurred well after the peak EMG activity. EMG activity by itself does not directly correlate with the force generated during chewing. The sound transmission method for measuring interjaw force during chewing, which was developed as part of this project, proved to be practical for research purposes. No intraoral devices are required, and the time relationship to force is accurate to within 15 ms.

Based on the association of bacterial plaque with the initiation of chronic gingivitis and progression of chronic periodontitis, chemical antiplaque agents have been employed both in prevention of periodontal disease and its treatment. In supragingival plaque control regimens, chlorhexidine has not been superceded as a chemical anti-plaque agent, although other compounds have been shown to be useful. The local side-effects of chlorhexidine and other cationic antiseptics, however, limit their long-term use for prevention. Extrinsic tooth staining in particular remains the greatest problem. Short-term anti-plaque uses for chlorhexidine include as an adjunct to mechanical cleaning in the initial oral hygiene phase of treatment, in situations where mechanical oral hygiene is difficult, including postsurgery, intermaxillary fixation, fixed orthodontic therapy, physically and mentally handicapped individuals, systemic diseases with oral manifestations such as leukaemia. More recent interest in chlorhexidine has resulted from the delivery of compounds subgingivally in the treatment of chronic periodontitis. Such methods have extended the use of chlorhexidine into areas inaccessible to the action of antimicrobial drugs delivered locally by conventional means, such as tooth brushing or mouth rinsing. Available evidence suggests that chlorhexidine may not be as effective as some antimicrobial drugs whose activity is more specific for those organisms considered particularly pathogenic to the periodontal tissues.

Photoelastic Analysis of Internal Stresses in the Periodontium Created by Occlusal Forces

The purpose of the present study was to explore the regenerative potential of anorganic bone plus collagen (AB-C) in experimental intrabony defects. Eight healthy female beagle dogs, 3 to 4 years old and weighing 15 to 16 kilos, were used. After extraction of the mandibular third premolars (P3), surgical defects were created and inflammation induced by placement of cotton and steel braids. Eight weeks later, the braids were removed. The experimental lesions thus obtained were either treated by plain flap curettage (group 1: control) or were, in addition, implanted with AB-C (group 2: experimental). Blocks of AB-C alone were observed by scanning electron microscopy (SEM). The results show that the surface of the particles have the characteristics of a bone tissue. These particles are gathered together with a fibrillar network. Six, 18, and 36 weeks postoperative (PO), non-decalcified specimens from both groups were examined histologically by contact microradiography. In group 1, no significant bone regeneration was observed at 6, 18, or 36 weeks PO. In group 2, trabeculae undergoing mineralization and circumscribing dense particles above the reference notch were seen at 6 weeks PO; 18 and 36 week specimens showed significant bone regeneration with more or less dense remaining particles. The periodontal ligament space was always clear and the only signs of ankylosis noticed were deep in the notch on one 18 week group 2 specimen and on one 36 week group 1 specimen.
