Introduction. Bias and precision statistics have succeeded regression analysis when measurement techniques are compared. However, when applied to cardiac output measurements, inconsistencies occur in reporting the results of this form of analysis. Methods. A MEDLINE search was performed, dating from 1986. Studies comparing techniques of cardiac output measurement using bias and precision statistics were surveyed. An error-gram was constructed from the percentage errors in the test and reference methods and was used to determine acceptable limits of agreement between methods. Results. Twenty-five articles were found. Presentation of statistical data varied greatly. Four different statistical parameters were used to describe the agreement between measurements. The overall limits of agreement in studies evaluating bioimpedance (n = 23) was ±37% (15–82%) and in those evaluating Doppler ultrasound (n = 11) ±65% (25–225%). Objective criteria used to assess outcome were given in only 44% of the articles. These were (i) limits of agreement approaching ±15–20%, (ii) limits of agreement of less than 1 L/min, and (iii) more than 75% of bias measurements within ±20% of the mean. Graphically, we showed that limits of agreement of up to ±30% were acceptable. Conclusions. When using bias and precision statistics, cardiac output, bias, limits of agreement, and percentage error should be presented. Using current reference methods, acceptance of a new technique should rely on limits of agreement of up to ±30%.

A meta-analysis of studies using bias and precision statistics to compare cardiac output measurement techniques.

This third edition of what has now become a well-established textbook in cardiovascular medicine is again edited by Dr Eugene Braunwald with the assistance of 65 other authors who read like a Who's Who of American Cardiology. Since the second edition, 12 new chapters have been added or substituted and others have been significantly revised. When assessing the accuracy and precision of a new technique for cardiac output measurement, the commonly quoted criterion for acceptability of agreement with a reference standard is that the percentage error (95% limits of agreement/mean cardiac output) should be 30% or less. We reviewed published data on four different minimally invasive methods adapted for use during surgery and critical care: pulse contour techniques, esophageal Doppler, partial carbon dioxide rebreathing, and transthoracic bioimpedance, to assess their bias, precision, and percentage error in agreement with thermodilution. An English language literature search identified published papers since 2000 which examined the agreement in adult patients between bolus thermodilution and each method. For each method a meta-analysis was done using studies in which the first measurement point for each patient could be identified, to obtain a pooled mean bias, precision, and percentage error weighted according to the number of measurements in each study. Forty-seven studies were identified as suitable for inclusion: N studies, n measurements: mean weighted bias [precision, percentage error] were: pulse contour N = 24, n = 714: -0.00 l/min [1.22 l/min, 41.3%]; esophageal Doppler N = 2, n = 57: -0.77 l/min [1.07 l/min, 42.1%]; partial carbon dioxide rebreathing N = 8, n = 167: -0.05 l/min [1.12 l/min, 44.5%]; transthoracic bioimpedance N = 13, n = 435: -0.10 l/min [1.14 l/min, 42.9%]. None of the four methods has achieved agreement with bolus thermodilution which meets the expected 30% limits. The relevance in clinical practice of these arbitrary limits should be reassessed.

Minimally invasive measurement of cardiac output during surgery and critical care: a meta-analysis of accuracy and precision.

Impedance cardiography: the next vital sign technology?

Simultaneous estimates of cardiac output were made during graded upright maximal exercise in 10 male subjects by means of Doppler velocity spectrum of ascending aortic flow, apical two-dimensional echocardiograms, thermodilution, and Fick oximetry. In 15 subjects, aortic annular and root diameters were measured during similar exercise from parasternal two-dimensional echocardiograms. The linear correlation between Doppler, two-dimensional echocardiography, and the invasive estimates ranged from r = .78 to r = .92. Both echocardiographic techniques were able to predict changes in invasive flow estimates with reasonable accuracy. Two-dimensional echocardiographic flow estimates underestimated invasive values by about 60%. The accuracy of Doppler flow estimates varied with the method of estimating aortic cross-sectional area. Greatest accuracy was obtained with areas calculated from diameters measured at the aortic value anulus with the leading edge-to-leading edge method of measurement. Correlation coefficients comparing Doppler and thermodilution flow estimates were generally higher (r = .75 to .96, mean .86) for individuals than for the group, but accuracy of the Doppler estimates in single subjects was quite variable. Aortic diameters did not increase from rest to moderate levels of upright exercise. A 3% to 5% increase in resting aortic diameter was noted in the upright posture as compared with the supine. Doppler flow estimates were obtained in all subjects to maximal exertion but in only a minority of subjects with two-dimensional echocardiography or thermodilution. Thus two-dimensional and Doppler echocardiography offer a noninvasive means of estimating cardiac output during vigorous exercise. The Doppler technique is technically more suitable to the study of exercise than two-dimensional echocardiography.(ABSTRACT TRUNCATED AT 250 WORDS)

Determination of stroke volume and cardiac output during exercise: comparison of two-dimensional and Doppler echocardiography, Fick oximetry, and thermodilution.

Measurements of blood flow by three different makes of thermodilution cardiac output computer in an artificial circulation were analysed by linear regression against absolute flow measured by timed blood volume collection. For each computer the horizontal distance between the 95% confidence limits for a single prediction was calculated at a standard flow rate of 5 litres per minute. This measurement represents the range of flow rates that could give rise to an identical measurement and provides a summary of the reproducibility of the computer's results and its ability to detect a change of flow rate. This measurement was used to evaluate the effect on each computer's performance of pulsatile or continuous flow, injectate volume, and injectate temperature. With continuous flow the optimum results were 1.8, 0.85, and 0.85 litres per minute and with pulsatile flow they were 1.3, 1.05, and 1.65 litres per minute. There was generally a deterioration in performance when pulsatile flow was evaluated. Under the conditions of the experiment optimum performance in both flow modes was obtained with 5 ml of ice cold injectate, but these findings cannot necessarily be extrapolated to the clinical situation. With pulsatile flow the overall range of blood flows that could give rise to identical measurements were for each computer 2.0, 1.5, and 3.1 litres per minute, corresponding to 40, 30, and 62% changes of the standard flow rate of 5 litres per minute.
