The authors summarize the dementia caregiving literature and provide recommendations regarding practice guidelines for health professionals working with caregivers. Family caregiving of older persons with disability has become commonplace in the United States because of increases in life expectancy and the aging of the population, with resulting higher prevalence of chronic diseases and associated disabilities, increased constraints in healthcare reimbursement, and advances in medical technology. As a result, family members are increasingly being asked to perform complex tasks similar to those carried out by paid health or social service providers, often at great cost to their own well-being and great benefit to their relatives and society as a whole. The public health significance of caregiving has spawned an extensive literature in this area, much of it focused on dementia caregiving because of the unique and extreme challenges associated with caring for someone with cognitive impairment. This article summarizes the literature on dementia caregiving, identifies key issues and major findings regarding the definition and prevalence of caregiving, describes the psychiatric and physical health effects of caregiving, and reviews various intervention approaches to improving caregiver burden, depression, and quality of life. Authors review practice guidelines and recommendations for healthcare providers in light of the empirical literature on family caregiving.

Family Caregiving of Persons with Dementia: Prevalence, Health Effects, and Support Strategies

Mothering and motherhood are the subjects of a rapidly expanding body of literature. Considered in this decade review are two predominant streams in this work. One is the theorizing of mothering and motherhood and the other is the empirical study of the mothering experience. Conceptual developments have been propelled particularly by feminist scholarship, including the increasing attention to race and ethnic diversity and practices. The conceptualizations of the ideology of intensive mothering and of maternal practice are among the significant contributions. Study of mothering has focused attention on a wide array of specific topics and relationships among variables, including issues of maternal well-being, maternal satisfaction and distress, and employment.

Conceiving and Investigating Motherhood: The Decade's Scholarship

Several research literatures are reviewed that address the associations of emotional, informational, and instrumental social support to psychological adjustment to cancer. Descriptive studies suggest that emotional support is most desired by patients, and correlational studies suggest that emotional support has the strongest associations with better adjustment. However, the evidence for the effectiveness of peer discussion groups aimed at providing emotional support is less than convincing. Moreover, educational groups aimed at providing informational support appear to be as effective as, if not more effective than, peer discussions. Reasons for inconsistencies between the correlational and intervention literatures are discussed, and future directions are outlined.

Balancing the multiple roles of work and caregiving for children, adults, and elders.

Intergenerational programs (organized activities designed to bring together two generations for the purpose of attitude change) are generally based, explicitly or implicitly, on a number of assumptions. First is the assumption that older people and youth have negative attitudes toward each other. Second, programs assume that in our mobile society youth and the elderly often have relatively little contact (Sussman & Pfeifer, 1988). Third, they assume that increasing the amount of contact between the generations will build understanding and more positive attitudes. Although many programs have been developed to increase contact and to encourage formation of better relationships between adolescents and older adults, relatively little is known about the accuracy of the assumptions upon which such programs appear to be based, nor have many evaluations of such programs been conducted. This paper examines these assumptions through an evaluation of a 1985-86 intergenerational program, the Elderly-Youth Exchange (EYE) program, which brought together older people and adolescents. The program was unique in that it involved youth and elders as either helpers or recipients of help in a variety of contexts involving intergenerational contact. The Effects of Intergenerational Experiences on Adolescents and Older Adults

Three demonstration projects that developed ways to strengthen the informal helping networks of elderly clients are described. The following aspects of network intervention are discussed and illustrated by these projects: assessment of client networks, possible goals of network intervention and the ability of programs to achieve them, and evaluation of interventions. Each program found barriers to intervening in networks derived from the client, the network, and the social service agency. Major barriers encountered include the client's privacy preferences and attitudes about relying on the network for help, networks that are exhausted or unsupportive, and agency structures that limit interventions. Although each program had some measure of success in strengthening social support for its clients, it does not appear that informal resources can easily and cost effectively substitute for formal services.

Working with the Informal Helping Networks of the Elderly: The Experiences of Three Programs

SUMMARY Although exposure to the natural environment has therapeutic benefits for nursing home residents and residents with dementia (Cohen-Mansfield & Werner, 1998; Lovering et al., 2002; Mooney & Nicell, 1992), many elders living in congregate facilities have limited access to the natural world. Long-term care facilities often do not incorporate the use of plants and natural settings into their daily activities due to limited knowledge, time, and funding. A twenty-two hour training program was developed and tested to increase the knowledge of activity staff about horticulture and how to involve their residents in the outdoor environment. Staff members were able to introduce changes both to their activity programs and to the outdoor environments in their facilities as a result.
