Chronic obstructive pulmonary disease (COPD) remains a major public health problem. It is the fourth leading cause of chronic morbidity and mortality in the United States, and is projected to rank fifth in 2020 in burden of disease worldwide, according to a study published by the World Bank/World Health Organization. Yet, COPD remains relatively unknown or ignored by the public as well as public health and government officials. In 1998, in an effort to bring more attention to COPD, its management, and its prevention, a committed group of scientists encouraged the U.S. National Heart, Lung, and Blood Institute and the World Health Organization to form the Global Initiative for Chronic Obstructive Lung Disease (GOLD). Among the important objectives of GOLD are to increase awareness of COPD and to help the millions of people who suffer from this disease and die prematurely of it or its complications. The first step in the GOLD program was to prepare a consensus report, Global Strategy for the Diagnosis, Management, and Prevention of COPD, published in 2001. The present, newly revised document follows the same format as the original consensus report, but has been updated to reflect the many publications on COPD that have appeared. GOLD national leaders, a network of international experts, have initiated investigations of the causes and prevalence of COPD in their countries, and developed innovative approaches for the dissemination and implementation of COPD management guidelines. We appreciate the enormous amount of work the GOLD national leaders have done on behalf of their patients with COPD. Despite the achievements in the 5 years since the GOLD report was originally published, considerable additional work is ahead of us if we are to control this major public health problem. The GOLD initiative will continue to bring COPD to the attention of governments, public health officials, health care workers, and the general public, but a concerted effort by all involved in health care will be necessary.

BACKGROUND: Airway pressure release ventilation (APRV) is a commonly used mode of ventilation designed to increase mean airway pressure and thus oxygenation. Different strategies for clinical management have been described in the literature but are largely based on physiologic concepts, animal data, and small clinical trials. The purpose of this study was to determine how APRV is currently managed by surveying practicing respiratory therapists with experience using APRV. METHODS: A 15-item survey was developed by the authors and posted on the AARConnect online media platform in January 2016 after being declared exempt by our institution9s institutional review board. Survey questions were derived from a literature review of recommended APRV settings. Responses were limited to one per institution. RESULTS: The survey was completed by 60 respondents who used APRV. Of the 4 key initial APRV settings (P high, P low, T high, and T low), there was good agreement among survey responders and published guidelines for setting initial T high (4–6 s) and initial P low (0 cm H2O). There was some disagreement regarding initial P high, with 48% of responders matching P high to conventional ventilation plateau pressures but another 31% using conventional ventilation mean airway pressure plus 2–5 cm H2O. The most disagreement was with the T low setting, with only 47% of survey responders agreeing with published guidelines about using the expiratory flow signal to set T low. There was good agreement among survey responders and published guidelines for what changes to make when gas exchange was outside of the targeted range. A substantial number of respondents accepted P high and APRV release volumes that may exceed lung-protective limits. CONCLUSIONS: There is only limited consensus among practitioners for initial APRV settings, probably reflecting the paucity of good clinical outcome data and confusion surrounding the physiology of this mode.

Clinical Management Strategies for Airway Pressure Release Ventilation: A Survey of Clinical Practice.

Ventilator-associated pneumonia (VAP) significantly increases intensive care unit morbidity, mortality, and costs. VAP is thought to be caused by bacterial entry into injured airways, which produces tracheobronchitis that evolves into diffuse pneumonia. The use of aerosolized antibiotics is conceptually attractive, especially when the infection is early and limited to the airway epithelium. Data show that aerosolized antibiotics kill airway bacteria and improve outcomes in cystic fibrosis. The clinical evidence for aerosolized antibiotics to prevent VAP is weak but suggestive. Concerns about the high cost, possible development of antibiotic resistance, and other potential risks of aerosolized antibiotics led several evidence-based consensus groups to recommend against routine use of aerosolized antibiotics for VAP prevention until better data are available. Importantly, the clinical evidence that aerosolized antibiotics can treat established VAP is negative, and multiple consensus groups recommend against treating established VAP with aerosolized antibiotics.

An endotracheal tube (10) adapted for aerosol generation at the distal end (14) thereof and which is a modification of existing endotracheal tubes. The improved tube (10) incorporates first (20A) and second (20B) conduits along the length thereof in order to deliver a liquid to be aerosolized and a high velocity gas at the distal end (14) of the endotracheal tube (10) so as to create an aerosol within the lungs. The distal end of the gas conveying catheter (20B) is configured to direct the high velocity gas transported therethrough across the pathway of the liquid solution exciting the distal end of the first conduit (20A) to facilitate the creation of an aerosol for high efficiency delivery of the liquid solution.
