MR imaging of nasopalatine duct cysts

In this report, we show a case of the extensive nasopalatine duct cyst showing remarkable nasolabial protrusion. CT and MR examinations showed remarkable expansion of the extraosseous part of the lesion toward anterior and lateral sides, and this finding is considered to show the rather aggressive nature of this common cystic lesion after penetration of the maxillary cortex.

Expansile Lesions Arising from Structures and Spaces Adjacent to the Paranasal Sinuses

Nasopalatine duct cysts (NPDCs) are the most common nonodontogenic cyst of the jaw, with a reported prevalence of between 1% and 11.6% of all jaw cysts.1 It is believed to arise from epithelial remnants of the nasopalatine duct, the communication between the nasal cavity and anterior maxilla in the developing fetus. For huge NPDCs, total excision is difficult, and there is an increase in the possibility of postoperative complications including submucosal hematoma, wound dehiscence, wound infection, injury to tooth roots, injury to nasopalatine neurovascular bundles, paresthesia of the anterior palate, facial swelling, and oronasal fistula formation. This article discusses a case with a large NPDC, which was managed surgically without any complication. Radiological findings emphasizing the importance of cone-beam computed tomography in diagnosis and optimized treatment planning of NPDCs are discussed.

Role of cone-beam computed tomography in diagnosis and management of nasopalatine duct cyst.

Nasopalatine duct cysts are a common etiology for anterior midline palatal masses, which may be seen by craniofacial surgeons. Plain radiographs, computed tomography, and magnetic resonance imaging (MRI) scans may be useful in their diagnoses. Clinically, they may be symptomatic in more than 70% of patients. Treatment is surgical excision, with a recurrence rate of approximately 10%. We present a case of a midline, palatal mass in a 13-year-old girl in whom an MRI scan was utilized and evaluated as a radiological tool in the diagnosis and treatment plan of a nasopalatine duct cyst. An MRI scan is useful diagnostic tool; it provides exceptional definition of the anatomy in this region. A critical review of the literature, as well as pathogenesis and treatment recommendations, are provided.
