Both in therapeutic and in character-analyses we notice that two themes come into especial prominence and give the analyst an unusual amount of trouble. It soon becomes evident that a general principle is at work here. The two themes are tied to the distinction between the sexes; one is as characteristic of males as the other is of females. In spite of the dissimilarity of their content, there is an obvious correspondence between them. Something which both sexes have in common has been forced, by the difference between the sexes, into different forms of expression [32, p. 2501. IVe often have the impression that with the wish for a penis and the masculine protest we have penetrated through all the psychological strata and have reached bedrock, and thus our activities are at an end. This is probably true, since, for the psychical field, the biological field does in fact play the part of the underlying bedrock. The repudiation of femininity can be nothing else than a biological fact, a part of the great riddle of sex. I t would be hard to say whether and when we have succeeded in mastering this factor in an analytic treatment. TVe can only console ourselves with the certainty that we have given the person analysed every possible encouragement to re-examine and alter his attitude to it [32, pp. 252-2531.

Thyroid disorders are common in the Down syndrome population but many specific areas of importance remain to be resolved. A detailed review of previously published case reports and research studies highlighting the clinical association between Down syndrome and thyroid disorders was undertaken. Historical, epidemiological, immunological, diagnostic and treatment issues are addressed. Recommendations for future management and research are considered.

Down Syndrome and Thyroid Disorders: A Review

Several editions of this Journal exist and those commonly available are the later ones. The importance of this first edition lies in its timing. Within a year of his return from this voyage, Darwin, then in chronic ill health, had begun notes on The Origin of Species. By 1842 he had made a 35-page summary of his ideas; and his 230-page manuscript of 1844 contains all the essentials of his final work. Thus this first edition of the Journal is the last picture of his thoughts before they crystalized. Here we can study the development of his ideas, can sense his misery from seasickness and loneliness, and feel his rebellion at the goldfish-like privacy of his quarters-the end of the chart room in a vessel no bigger than a fishing boat. Those interested in how ideas grow as well as in the perfected theory owe a debt to the Hafner Publishing Company for making this important edition available in facsimile.
