Sixty-six teachers from local public schools (42 womenand 24 men, mean age 42 6 5 years) were asked to sample saliva for cortisol analysis on 3 consecutive days. On eachday, cortisol levels were measured at the time of awakening and 15, 30, and 60 minutes thereafter. On the nightbefore the third day, subjects took 0.5 mg dexamethasone orally for testing glucocorticoid feedback inhibition.Burnout and perceived stress were measured by three different questionnaires.

Burnout in psychiatric nursing

Introduction.  Burnout in nursing is of both individual and organizational concern with ramifications for well-being, job performance, absenteeism and turnover. Burnout is rarely assessed as part of a comprehensive model of occupational stress, a short-coming which this paper attempts to redress.

Method.  A randomly selected sample of 510 psychiatric nurses from one Scottish Trust completed a questionnaire based on a psychological model of occupational stress which included the Maslach Burnout Inventory (MBI) as the dependent variable.

Findings.  The respondents reported average, low and average levels of emotional exhaustion, depersonalization and personal accomplishment, respectively. The study sample had significantly lower scores on emotional exhaustion and depersonalization than normative data but also significantly lower levels of personal accomplishment than a normative group of physicians and nurses. Only 2·0% of the study sample could be categorized as having high burnout overall (i.e. high emotional exhaustion, high depersonalization, low personal accomplishment) and they differed significantly from the rest only in terms of males being over-represented. Hierarchical regression analysis revealed that selected explanatory variables accounted for 41·9% of emotional exhaustion, 16·4% of depersonalization and 25·6% of personal accomplishment in the study sample.

Implications.  The paper discusses the implications of the findings in terms of a comprehensive approach to intervention aimed at minimizing the risk of burnout in psychiatric nurses. Such an approach will involve interventions at the organizational and individual level.

Fundamento: El termino Burnout se refiere a una situacion que es cada vez mas frecuente entre los profesionales que prestan sus servicios a traves de una relacion directa y mantenida con las personas, entre los que se encuentran todos los profesionales sanitarios. El objetivo de nuestro estudio ha sido determinar la prevalencia del sindrome de Burnout y de sus tres componentes (cansancio emocional, despersonalizacion y falta de realizacion personal) entre los profesionales del area de enfermeria del Hospital Clinico Universitario San Carlos de Madrid y su relacion con determinados factores sociodemograficos, laborales e institucionales. Metodos: Se realizo un estudio tranversal entre los profesionales adscritos al area de enfermeria del mencionado hospital. Las variables implicadas se recogieron mediante un cuestionario de elaboracion propia. En cuanto al sindrome de Burnout se midio mediante el Maslach Burnout Inventory, en su version validada en espanol. Resultados: El personal de enfermeria esta mas despersonalizado (p=0,004) y menos realizado (p=0,036) que el grupo de auxiliares/tecnicos. Cuando se analizaron los resultados de las 4 escalas por servicios comprobamos que el agotamiento es superior en los trabajadores de oncologia y urgencias (p=0,001), la despersonalizacion en urgencias (p=0,007), y el Burnout es de nuevo mas alto en las areas de oncologia y urgencias (p=0,000). Los profesionales que respondian que su labor asistencial era poco reconocida obtenian las peores puntuaciones en el Burnout y sus tres dimensiones (p =0,000). A menor grado de satisfaccion laboral mas altas son las puntuaciones en las 4 escalas (p=0,000). Conclusiones: Podemos concluir que en nuestro estudio el perfil del sujeto afecto de Burnout se corresponde con el de un profesional con experiencia laboral que sin embargo manifiesta un escaso grado de reconocimiento a su labor asistencial y un alto nivel de insatisfaccion con la gestion de su centro de trabajo.

Caregiver stress and burnout in an oncology unit.

Background: The present study tested the hypothesis that particular personality dispositions act as resistance resources in the encounter with stressful life events. Methods: The sample consisted of 100 student nurses working in critical areas (oncology and AIDS). At the beginning of the 2nd year course subjects were administered the Kobasa Hardiness Scale and at the end of the year the Maslach Burnout Inventory. Results: Higher hardiness levels at the beginning of the course were associated with lower emotional exhaustion and higher personal achievement scores at the end. As predicted, findings show hardiness scores to be predictive of the risk of burnout. Conclusions: A screening of personality traits could be useful in preventing the risk of burnout in nurses working in critical areas.
