Cetuximab Monotherapy and Cetuximab plus Irinotecan in Irinotecan-Refractory Metastatic Colorectal Cancer

In vitro and in vivo pharmacological screening of Betulinic acid (BA) and five dihydro-BA derivatives modified at C-3 position [4-nitrobenzyl-oximino (1), 2-4-difluoro-benzoyloxy (2), 2-4-difluoro-benzylidene-amino (3), benzoyl-hydrazono (4), and 4-fluorophenyl-hydrazono (5)], having potent in vitro anti-cancer activity was carried out using ADME, animal PK and tumor studies. We found that BA and the derivatives had poor aqueous solubility ( 70%). Although BA and 5 were metabolized by human liver microsomes, derivatives 1, 2, 3 and 4 possessed good in vitro metabolic stability. Except 3 which inhibited CYP1A2 isoform by more than 50% none of the other compounds inhibited key cytochrome P450 enzyme isoforms (CYP1A2, CYP2C9, CYP2D6 and CYP3A4) at 10 μM. Based on in vitro results one derivative 1 was tested in rodent PK and tumor studies. We found that 1 exhibited favorable pharmacokinetic characteristics of a systemically administered drug and showed better in vivo anti-tumor efficacy as compared to BA in a human colon cancer xenograft model. Our results show that BA derivatives are potential anti-cancer compounds which need to be explored in detail.

Pharmacological evaluation of C-3 modified Betulinic acid derivatives with potent anticancer activity.

Colorectal cancer (CRC) incidence has increased during the past decades in Spain, being the first malignant tumour in incidence. Observed mortality for CRC is mainly due to liver and lung metastases. The only curative treatment is surgery; new surgical techniques and neoadjuvant treatments have increased the number of surgery candidate patients. Patients should be managed with a multidisciplinary approach that includes imaging techniques, chemotherapy, surgery and pathological assessment. As an answer to this approach, a group of pathology experts interested on CRC liver metastases aimed to review the diagnosis and prognosis of liver mestastases and developed practical recommendations for its assessment. The expert group revised the current literature and prepared questions to be discussed based on available evidence and on their clinical practise. As a result, recommendations for the assessment of tumour regression of liver metastases are proposed, which could be implemented in oncology centres allowing assessment standardisation for these patients. Prospective multi-center studies to evaluate these recommendations validity will further contribute to improve the standard care of CRC liver metastases patients.

Recommendations of a group of experts for the pathological assessment of tumour regression of liver metastases of colorectal cancer and damage of non-tumour liver tissue after neoadjuvant therapy

Nowadays cancer has become the most common chronic disease in our society, as well as one of the main causes of death. Colorectal cancer is the third most frequent tumor among men. In the Canary Islands it is the most common malignancy if we exclude skin cancers. Its screening is included in the portfolio of services of the Spanish National Health System, to achieve early detection. The purpose of this work is to collect those scientific articles that relate the quality of life with patients suffering from colorectal cancer, in order to identify the needs of these people and have them covered to achieve a good quality of life. “Cáncer de colon y calidad de vida: una revisión bibliográfica” Daniela Raya Ferrera 2 To perform this review, we used the search resources PuntoQ, which found 234 articles of which only 9 meet the established criteria, as well as Google Scholar, which counted 325 results and only 12 met these criteria. As a conclusion of this bibliographical search, it has been deduced that there is a deficit of scientific literature that relate both concepts and try to identify the needs of these patients. Therefore, nursing must share their knowledge and experiences with the scientific community, to share all this information and provide quality care for these patients, which will significantly increase their quality of life.
