A surgical stapling instrument (1) comprises a body portion (2, 3), a handle (4) and a staple fastening assembly (8). The staple fastening assembly (8) includes a curved cartridge (10), which comprises at least one curved open row of staples, and a curved anvil (22), which is adapted to cooperate with the cartridge (10) for forming the ends of the staples exiting from the cartridge (10). The staple fastening assembly (8) is adapted to allow unobstructed access towards the concave inner faces of the cartridge (10) and the anvil (22). The cartridge (10) can be moved towards the anvil (22) from a spaced position for positioning tissue therebetween to a closed position for clamping the tissue. Preferably, a knife is contained within the cartridge (10) and is positioned such that there is at least one row of staples on at least one side of the knife.

A surgical severing and stapling instrument, suitable for laparoscopic and endoscopic clinical procedures, clamps tissue within an end effector of an elongate channel pivotally opposed by an anvil. An E-beam firing bar moves distally through the clamped end effector to sever tissue and to drive staples on each side of the cut. The E-beam firing bar affirmatively spaces the anvil from the elongate channel to assure properly formed closed staples, especially when an amount of tissue is clamped that is inadequate to space the end effector. In particular, an upper pin of the firing bar longitudinally moves through an anvil slot and a channel slot is captured between a lower cap and a middle pin of the firing bar to assure a minimum spacing. Forming the E-beam from a thickened distal portion and a thinned proximal strip enhances manufacturability and facilitates use in such articulating surgical instruments.

The balloon is dilatably/deflatably provided on the tube body. The first-communication-path extends from a distal-end-portion toward a proximal-end-portion of the tube body in a state in which the first-communication-path communicates with the balloon. The second-communication-path extends from a hollow-portion toward the proximal-end-portion of the tube body. The first-mouthpiece projects from the tube body and includes a path communicating with the first-communication-path. The second-mouthpiece projects from the tube body and includes a path communicating with the second-communication-path. The first-connection-section is connected to a first-feed-portion of a fluid-feed-mechanism and is prevented from being connected to a second-feed-portion of the fluid-feed-mechanism. The second-connection-section is connected to the second-feed-portion and is prevented from being connected to the first-feed-portion.

An endoscopic insertion aid (14) includes a hollow member (62) having a communication path which provides communication between openings at the distal and proximal ends thereof. The hollow member includes a hollow member, an exposure portion and a guide portion. The hollow member insertion portion is inserted into a body cavity, having distal and proximal ends. The exposure portion is provided at the proximal end of the hollow member insertion portion and is exposed outside the body. The guide portion is provided in at least the exposure portion of the hollow member insertion portion and the exposure portion. The guide portion allows a distal end (32) of an insertion section (22) of an endoscope (12) to pass through the communication path laterally from the hollow member (62) and project from the opening at the distal end of the hollow member.

An endoscopic insertion aid (14) includes a tube member (62), a pipeline (68, 70) and a plurality of balloons (66a, 66b, 66c). The tube member allows an insertion section (32) of an endoscope (12) to be inserted therethrough, and guides the longitudinal movement of the insertion section. The pipeline is provided in the tube member, and communicates with a supply/discharge unit (22) to supply a fluid to or discharge the fluid from the distal end of the tube member. The balloons are disposed on the outer periphery of the distal end of the tube member longitudinally along the tube member, communicate with the pipeline, and inflate/deflate in accordance with the supply/discharge of the fluid via the pipeline. The balloon (66a) disposed on the distal side of the tube member is inflated to have an outside diameter larger than that of the balloon (66c) disposed closer to the proximal side than the balloon disposed on the distal side when the balloons are inflated.

The balloon disposed on the distal side of the tube member is inflated to have an outside diameter larger than that of the balloon disposed closer to the proximal side than the balloon disposed on the distal side when the balloons are inflated.
