Pediatric Critical Care Medicine www.pccmjournal.org 899 Although Zhang et al (11) deserve credit for attempting to control potential biases and following strict methodology, the study holds some limitations, notably the relative small sample size of 32 cases and 32 controls and also the relative long collection time for the specimens may influence the results. It is likely that that hypothesized positive effect of the orally administrated colostrum on the neonatal immune system, including the potentially protective effect on sepsis and NEC, may be demonstrated in larger studies containing more patients, even though it cannot be demonstrated in this study (11). Yet, the article by Zhang et al (11) contributes new knowledge as to the feasibility of colostrum administration to neonates. Whereas, the long-term benefits and/or adverse effects of this administration on the immune system only can be elucidated through future large, controlled studies including long-term follow-up studies, the efforts by Zhang et al (11) could serve as a stepping stone for future scientific work.
