Fundamentals of Queueing Theory

In most of the queueing models, service is considered to be complete without any interruption. But in reality, queueing systems are subject to interruptions due to failure of server or any other cause. In the present article, we present an overview and literature survey on the performance modeling and analysis of single server, general service queueing system with service interruption using supplementary variable technique. The factors causing service interruption such as unreliable server and server vacation are elaborated. The brief of supplementary variable technique to establish the queue size distribution is explained for single server non-Markovian queueing models by incorporating the features of service interruption. The basic concepts and review of literature on the queues with server breakdown and/or vacationing server are described. The research works done during last 10 years (2010–2019) on queues with service interruption involving many other key concepts namely Bernoulli vacation, multiple vacation, bulk arrival, discouragement, etc. and queueing scenarios of service interruption are reported. Some specific applications are also highlighted.

Despite advances in the treatment of heart failure (HF), the physical symptoms and stress of the disease continue to negatively impact patients’ health outcomes. Technology now offers promising ways to integrate personalized support from health care professionals via a variety of platforms. Digital health technology solutions using mobile devices or those that allow remote patient monitoring are potentially more cost effective and may replace in-person interaction. Notably, digital health methods may not only improve clinical outcomes but may also improve the psycho-social status of HF patients. Using digital health to address biopsychosocial variables, including elements of the person and their context is valuable when considering chronic illness and HF in particular, given the multiple, cross-level factors affecting chronic illness clinical management needed for HF self-care.

Lo scopo principale di questa ricerca è di fornire una revisione sistematica della letteratura scientifica in merito all'uso della telemedicina rivolta a pazienti caratterizzati da situazioni di morbilità (in alcuni casi di vera e propria cronicità), cercando di mettere in evidenza come questa sia accolta dai pazienti, la sua efficacia nelle cure e in che modo essa incrementi l'accesso ai servizi sanitari e con quali tecnologie.La review di letteratura è stata condotta utilizzando l'approccio Tranfield (2003), metodo efficiente e di alta qualità per selezionare e valutare in maniera oggettiva e quantitativa gli studi presi in esame. La metodologia è stata seguita passo dopo passo, analizzando parole chiave, argomenti, qualità delle riviste e giungendo, alla fine, alla selezione di alcuni paper che sono stati analizzati in dettaglio. Tali lavori, inoltre, vengono confrontati tra loro e classificati secondo metriche significative, valutando anche le tecnologie e le metodologie utilizzate.A valle della review sistematica è stato possibile riscontrare che la maggior parte dei pazienti coinvolti nei programmi di telemedicina è propensa all'utilizzo di questo nuovo modello di erogazione di servizi sanitari e i risultati clinici sembrano essere incoraggianti.Gli stessi risultati suggeriscono che i servizi di telemedicina sono apprezzati dai pazienti, aumentano l'accesso alle cure e potrebbero rappresentare un modo efficace ed efficiente per affrontare al meglio emergenze e pandemie, abbassando i costi complessivi e promuovendo, altresì, l'inclusione sociale.

In this paper, we have considered an MX / (a,b) / 1 queueing system with server breakdown without interruption, multiple vacations, setup times and N-policy. After a batch of service, if the size of the queue is ξ (< a), then the server immediately takes a vacation. Upon returns from a vacation, if the queue is less than N, then the server takes another vacation. This process continues until the server finds atleast N customers in the queue. After a vacation, if the server finds at least N customers waiting for service, then the server needs a setup time to start the service. After a batch of service, if the amount of waiting customers in the queue is ξ (≥ a) then the server serves a batch of min(ξ,b) customers, where b ≥ a. We derived the probability generating function of queue length at arbitrary time epoch. Further, we obtained some important performance measures.
