XI. STRATEGIES FOR IMPROVING DIABETES CARE D iabetes is a chronic illness that requires continuing medical care and patient self-management education to prevent acute complications and to reduce the risk of long-term complications. Diabetes care is complex and requires that many issues, beyond glycemic control, be addressed. A large body of evidence exists that supports a range of interventions to improve diabetes outcomes. These standards of care are intended to provide clinicians, patients, researchers, payors, and other interested individuals with the components of diabetes care, treatment goals, and tools to evaluate the quality of care. While individual preferences, comorbidities, and other patient factors may require modification of goals, targets that are desirable for most patients with diabetes are provided. These standards are not intended to preclude more extensive evaluation and management of the patient by other specialists as needed. For more detailed information, refer to Bode (Ed.): Medical Management of Type 1 Diabetes (1), Burant (Ed): Medical Management of Type 2 Diabetes (2), and Klingensmith (Ed): Intensive Diabetes Management (3). The recommendations included are diagnostic and therapeutic actions that are known or believed to favorably affect health outcomes of patients with diabetes. A grading system (Table 1), developed by the American Diabetes Association (ADA) and modeled after existing methods, was utilized to clarify and codify the evidence that forms the basis for the recommendations. The level of evidence that supports each recommendation is listed after each recommendation using the letters A, B, C, or E.

Research Design: Qualitative, Quantitative, and Mixed Methods Approaches

The American Diabetes Association and the European Association for the Study of Diabetes convened a panel to update the previous consensus statements on the management of hyperglycemia in type 2 diabetes in adults, published since 2006 and last updated in 2019. The target audience is the full spectrum of the professional health care team providing diabetes care in the U.S. and Europe. A systematic examination of publications since 2018 informed new recommendations. These include additional focus on social determinants of health, the health care system, and physical activity behaviors, including sleep. There is a greater emphasis on weight management as part of the holistic approach to diabetes management. The results of cardiovascular and kidney outcomes trials involving sodium-glucose cotransporter 2 inhibitors and glucagon-like peptide 1 receptor agonists, including assessment of subgroups, inform broader recommendations for cardiorenal protection in people with diabetes at high risk of cardiorenal disease. After a summary listing of consensus recommendations, practical tips for implementation are provided.

Background Patient satisfaction has become an integral component of the quality of healthcare services. It has been used for the purpose of performance assessment, reimbursement, and quality management of health service delivery. It has been suggested that patient satisfaction could be a predictor of health-related behavior. Objectives To develop and validate a tool for use within the Arabic context to assess patient satisfaction. To assess patient satisfaction with current community pharmacy services in the UAE using the validated tool. Methods A systematic process was used to develop an assessment tool that could be used within the Arabic context and establish its validity and reliability. Survey participants assessed their satisfaction with the services based on a 5-point Likert-type scale: Poor = 1, Fair = 2, Good = 3, Very good = 4, Excellent = 5. The anonymous questionnaire was distributed over a 5-month period to eligible participants in public places such as malls and shopping markets, in various emirates across the UAE. Those who were 21 years or older, taking at least one scheduled (regular) medication and having adequate Arabic or English language proficiency were included. Results The instrument comprised four dimensions: Information, Relationship, Accessibility and Availability. Participants required more information about medications and self-management (Mean = 2.49 ± 1.19). Measures of competence, i.e., care, interest, time, confidence and trust, could also be improved (Mean = 3.05 ± 1.07). Accessibility scores measuring physical, geographical and financial items were lowest (Mean = 2.80 ± 1.33). Overall scores on availability of medications indicated relative satisfaction with this dimension (Mean = 3.51 ± 0.7). Conclusions This study is the first to use a patient satisfaction tool specifically developed for the Arabic context. Patient satisfaction scores in all dimensions were significantly lower than published data, suggesting patients have unmet expectations of community pharmacy services in the UAE. Stakeholders could utilize this information to help in the design and delivery of improved services that could lead to increased demand.

Assessing patient satisfaction with community pharmacy in the UAE using a newly-validated tool.

Objective. To develop a tool to teach pharmacy students assertive communication skills to use when talking with physicians over the telephone. Design. As an assignment for their Communication Skills and Counseling course, students were asked to write a script involving a patient care issue or problem covering 3 different communication styles that could be used when contacting a prescriber by telephone: passive, aggressive, and assertive. Students worked in groups to write and act out the scripts for the class. Assessment. Eight scripts were developed by students and rated by peers and faculty members. The script that received the highest ratings was used in the development of a multimedia educational CD. Conclusion. The development of hypothetical scripts describing a drug therapy problem and illustrating the types of interactions between physicians and pharmacists while discussing the problem allowed pharmacy students to explore different communication techniques and improve their communication skills.

A Tool to Teach Communication Skills to Pharmacy Students

Objective The main objective of this study was to offer an insight on the issue of continuing education (CE) in the UAE and to determine the type and format of CE pharmacists in this country prefer to attend and consider most effective. Methods A multi-theme survey was developed to find the reasons pharmacists choose to attend different CE programs, the survey assessed continuing education needs and preferences of pharmacists. Survey items included the types of formats and topics pharmacists prefer to attend and think are most useful to enhance their knowledge and skill. Finally the survey explored some barriers pharmacists conceive as such to attending effective CE. Results One hundred thirty-two surveys were included in this study, the vast majority of the participants were bachelor’s degree holders who were 40 years and younger. The participant’s main types of employment were marketing and hospital practice. Pharmacists’ preferences as for the format and topic type for programs they would like to attend were identified and compared to other practice settings. Barriers to attending effective CE programs were also elicited. Conclusion: Interactive workshops were recognized as the most favorable format for CE in this study, computer and internet-based formats were also ranked highly by participants followed by live-in person and printed material-based programs. Topics covering innovations in pharmacy practice and disease management were at the top of priorities for pharmacists who would also like to see more certificate programs be offered to them.

Continuing education needs assessment of pharmacists in the United Arab Emirates

Background: Team-based learning (TBL) has been implicated in teaching different modules to pharmacy students, no studies have incorporated (TBL) in teaching ethics.Aims. To utilize a team-based learning approach in teaching ethics in pharmacy practice to undergraduate pharmacy students. Method. The ethics section of a pharmacy practice course consisting of 11 contact hours was modified in to a team-based module consisting of initial individual and group readiness assurance tests followed by group application exercises.Results. Faculty-student contact time was about the same for both the traditional method and the team-based method. Students rated the TBL sessions positively. Faculty time preparing for the course increased since it is the first time to introduce the course using this approach.Conclusion. The approach has shown to benefit student performance in their assessments and to help in attaining skills of higher problem solving and application of knowledge. Students possibly acquired team-working and self-learning skills needed in the future.
