Methodology: 30 participants were allocated by a simple random sampling using lottery method into MRP (Experimental-Group A) and PNF (Control/Conventional Group B).Both groups received treatment for 30 minutes per day,3 times per week for 3 weeks. Outcome measures were recorded at pretest, post test and post 1 month follow up using Timed Up and Go Test (TUG) and Sit to stand (STS) item of Motor Assessment Scale (MAS). Study was done in respective participants home. ANOVA was used for data analysis.

Motor relearning program versus proprioceptive neuromuscular facilitation technique for improving basic mobility in chronic stroke patients-a comparative study

Background: Stroke subjects face reduced tolerance to activity and sedentary lifestyle due to various impairments, such as muscle weakness, pain, spasticity, and poor balance. Thus, loss of independent ambulation especially outdoors is generally observed in them. Methods: Chronic stroke patients (> 6 months) with Functional Ambulation Category score > 2 and able to walk at least 10 meters of distance with and without assistance from a tertiary healthcare centre were selected and treated. Subjects were randomly divided into 2 groups control group (n=14) and experimental group (n=13). Each group received Motor Relearning Programme for 60 minutes, 6 times a week for 4 weeks. The experimental group received an additional shoe-raise of 1 cm on the unaffected side along with while ambulating during therapy as well as at home. Pre and post treatment the patients were assessed for spatio-temporal parameters using foot print analysis method and Rivermead Visual Gait Assessment (RVGA) Score using RVGA scale. Results: There was significant improvement seen in almost all the spatio-temporal gait parameters and RVGA score in within group analysis. Whereas on between group the results from between group comparison suggests that subjects in MRP with shoe-raise group showed better results in spatio-temporal parameters of gait than subjects receiving MRP alone. But there was no additional benefit of shoe-raise seen on RGVA score and angle of toe-out parameter. Conclusion: Additional use of shoe-raise helps to improve spatio-temporal gait parameters. However, there was no additional change seen in RVGA score.

Effect of Shoe Raise along with Motor Relearning Programme (MRP) on Ambulation in Chronic Stroke

Responsiveness depends on properties of statistical analysis, commonly referred to being as, distribution-based responsiveness. Time Up and Go Test (TUG) is a common assessment to evaluate the mobilization in patients with complications pertaining to stroke. Objective: The major emphasis of this study was to analyse the improvement in functional mobility of the patients with symptoms related to stroke. Methods: This study is carried out in Fauji Foundation Hospital and National Institute of Rehabilitation Medicine (NIRM), Islamabad Pakistan. Sample size consisted of 116 patients including both genders, with age ranging between 45-65 years. Clinically, the diagnosis of stoke can be concluded in compliance with reported criteria and standards of World Health Organization (WHO). Out of total 116 sample size 25 were excluded and 91 patients were assessed to detect mobility response by using TUG test, pertaining to four different intervals. The overall procedure implemented for the conclusion of TUG test included the following points; a) patients were advised to stand on toes, b) walk 3 meters, c) turn around and walk back to the chair from which they initially started, and d) asked to sit down quickly. Results: Between the first 7 days and 12 weeks the median of TUG test, time (mobility) was reduced from 17s to 12s. The improvement in mobility is most seen in 1st week to 3rd month. Conclusion: Findings indicate that the TUG test is capable to evaluate the change in functional mobility in patients with stroke. Hence, the outcomes justify the use of TUG in stroke rehabilitation

The Time up & Go test to evaluate the change in functional mobility in post-stroke patients

To determine the effectiveness of the motor relearning program along with routine physical therapy versus routine physical therapy alone on balance and upright mobility in sub-acute stroke patients. Methods: Patients who satisfied inclusion criteria were chosen and randomly allotted into experimental and control group, 34 patients in each. After baseline assessment first follow-up was recorded at fourth week. Patients were guided for home plan for further one month. Second follow-up data was taken at eight week. Experimental group were given motor relearning training along with routine physiotherapy in sit to stand component by putting effected foot behind the sound one, forward movement of knees on chair and trunk in flexion. Gait training, balance training, stretching exercises and strengthening exercises were given to control group. The berg balance scale and time up and go test were used to measure the outcomes. Results: Statistically significant post-treatment p-value<0.05 was shown by both groups in all items of the BBS and time up and go test while doing the intragroup analysis after 8 weeks of intervention. Statistically significant results (P-value=0.000) was shown at 2nd follow up by the BBS score and TUG measure in intergroup analysis. Conclusion: Motor relearning program is demonstrated to have more measurably noteworthy and clinically compelling intervention while comparing to routine physiotherapy alone.
