Here, we describe the first case of intra‐articular osteoid osteoma of the capitulum, which is presented as elbow pain, extension lack, and sensation of click in joint flexion. Surgical treatment either arthroscopic or open is more in use in this location of the tumor than cortical osteoid osteoma.

Intra‐articular osteoid osteoma of the capitulum: A diagnostic challenge

Monteggia fracture-dislocations are infrequent in children accounting for approximately 2% of childhood elbow fractures, and diagnosis, mechanism of injury, treatment, and outcome are still challenging. We present a type III Monteggia equivalent lesion with fracture of the ulna, lateral condylar fracture, and lateral dislocation of radius in an 8-year-old girl. The patient presented with pain, swelling, and inability to move her right arm. No neurovascular injuries were detected. The patient was given analgesics, and the radial head was fixed with closed reduction in the emergency department. Her elbow was immobilized with a long arm splint, and she underwent surgery the day after. The lateral condylar fracture was fixed by using three 1.5 mm Kirchner wires (K-wires) to treat the humeral fracture. One 3.5 mm reconstruction plate was used to fix the ulnar fracture; however, it was not reduced properly, and the patient was referred to our hospital. She underwent surgery two days after the first surgery, the reconstruction plate was removed, and the ulnar fracture was fixed with a 3.5 mm 4-hole dynamic compression plate with cortical screws and three K-wires, and a long arm cast was applied. The patient was visited every two weeks with no complaints or complications. Six weeks after the surgery, the cast was split, and the three humeral and two ulnar K-wires were removed. Two weeks later, the other K-wire, which managed the medial facet involvement, was removed too. She had no complaints, and no complication was noted in the physical examination or radiographs. Her range of motion and rotation had returned completely two weeks later, and radiographs showed no complications. 

A rare type III Monteggia equivalent lesion in an eight-year-old girl: A case report and literature review

Background: Femoral neck fractures is uncommon but serious injuries in young adult with high complication rate. This study was designed to evaluate the outcome of surgical treatment of such a fracture fixed by screw in. Methods: In a descriptive prospective study form 2006 to 2009, 42 patients with the mean age of 37 years (15–60 yrs) were treated by closed or open reduction and screw fixation for femoral neak fractures in two university hospitals in AhvazIran. They all received surgery within the first 24 hours after injury. They were followed for 15 months on average. Results: In accordance with Garden classification, 2 patients were type I, 3 type II, 18 type III and 19 type IV Gorden fractures. Thirty two cases were fixed by open reduction and fixation and 10 by closed reduction. Union was achieved in 30 pateints. The 12 (28.6%) cases of nonunion were seen in the ones who received open reduction and fixation. Avascular necrosis was observed in 7 patients (16.7%), 4 of whom had obtained union. Conclusion: Femoral neck fracture is associated with high complication rate, in particular, if open reduction becomes necessary.

Outcome assessment of surgical treatment of femoral neck fracture in adults by screw fixation (short term results)

Background Anterior dislocation is the most common type of shoulder dislocation, and even after appropriate treatment, recurrence after a primary traumatic anterior dislocation is highly frequent. Surgical options for treating recurrent anterior dislocations mainly include Bankart arthroscopic and Latarjet open surgery. We aimed to evaluate the outcomes and complication rates of the open Latarjet procedure in a series of patients with recurrent anterior shoulder dislocation. Methods A total of 55 patients with recurrent anterior shoulder dislocation who underwent an open Latarjet procedure were included in this retrospective cohort study. Shoulder range of motion and postoperative complications, including neurapraxia, re-dislocation, hematoma, infection, dehiscence, implant failure, and pain, were evaluated. Results The mean age of the patients was 27.7 ± 6.5 years . The mean time interval after the first dislocation was 3.4 ± 2.7 years . The mean preoperative and postoperative forward flexion (P = 0.200), abduction (P = 0.200), external rotation (P = 0.066), and internal rotation (P = 0.310) were not significantly different. Postoperative complications included 1 case of postoperative musculocutaneous nerve neurapraxia, 1 case of re- dislocation, 1 case of wound dehiscence, and 2 cases of screw breakage. Postoperative pain was also recorded in 11 (20%) patients that were either in the form of occasional night pain (n = 6) or activity-dependent pain (n = 5). Conclusion Open Latarjet procedure is an efficient procedure for the treatment of recurrent anterior shoulder dislocation. However, its rate of complications remains relatively high, and surgeons must consider this drawback in their decision-making and address patients' expectations.

These observations suggest that the timely surgical treatment of type III Monteggia equivalent fracture results in acceptable outcomes. However, the lack of a universal classification system limits the generalization of the results

Type III Monteggia Equivalent Fracture Successfully Treated by Open Reduction and Internal Fixation: A Report of Two Cases

Background: Despite the advances in the methods of treating femoral neck fractures, treating some types of these fractures is still challenging. Therefore, understanding these fractures, their treatments, and the ways to control their complications are necessary for the orthopedists.  Objectives: In this study, we evaluate patients with acute femoral neck fractures with respect to the type of fractures and complications. Methods: In a 4-year cross-sectional study, the patients with acute femoral neck fractures who were admitted to Rasoul Akram Hospital were selected. Anatomical features and the type of fracture were determined by using plain radiography and CT scan. Fractures were classified according to the Garden classification. The patients were also evaluated for any postoperative complications for at least 6 months. Results: A total of 124 patients were enrolled. The Mean±SD age of the patients was 64.9±12.7 years and 87 (74.4%) were male. Based on the Garden classification, 7 fractures (5.9%) belonged to type I, 10 fractures (8.5%) to type II, 48 fractures (41%) to type III, and 52 fractures (44.4%) to type IV. Avascular necrosis in 28 cases (23.9%), non-union in 14 cases (12%), and claudication and pain in 35 cases (29.9%) were noted. The mean age of patients was different between fracture groups (P=0.01).  Conclusion: Our results showed that most of the patients with femoral neck fractures admitted to Rasoul Akram Hospital were associated with displacement. In this study, the most common complications were pain and claudication, avascular necrosis of the femoral head, and nonunion. In sum, further displacement of the fracture will result in more complications.

Four-Year Prospective Evaluation of Femoral Neck Fractures: Types and Complications

Background: Acrometastases are rare and account for approximately 0.1% of metastases. The most common primarycancer site is the lung, followed by colorectal and genitourinary system.Methods: We searched PubMed (www. pubmed.com), Google scholar (www.scholar.google.com), Science Direct (http://www.sciencedirect.com), and Springer (http://link.springer.com) databases, using a combination of controlled vocabularyand text word terms and reviewed the last 10 years literature in order to describe demographic trends, anatomicaldistribution, the most common primary sources of malignancy, and survival rates in the reports of foot metastases.Results: In conclusion 38 cases were included in this review analysis. Lung and genitourinary system were the mostfrequent primary sites.Conclusion: Forefoot was involved in 71% of all metastases to foot either alone or in combination with other areas ofthe foot. Calcaneus was involved in about 23% of patients either alone or in combination with other foot bones.Level of evidence: IV

Foot Metastasis: Review of 38 Cases.

Background The effect of spinopelvic alignment on low back pain (LBP) incidence has been studied in many investigations. However, the interrelation between spinopelvic parameters and LBP is poorly understood. In particular, it is unknown whether particular patterns of spinopelvic parameters render nonspecific LBP. In this study, we aimed to evaluate the role of spinopelvic parameters as risk factors of nonspecific LBP. Methods In this case-control study, spinopelvic parameters, including lumbar lordosis (LL), sacral slope (SS), pelvic tilt (PT), and pelvic incidence (PI), were compared between 148 patients with nonspecific LBP and 148 healthy controls. Demographic characteristics of the patients, such as age, gender, occupation, smoking, diabetes mellitus, and body mass index (BMI), were recorded as confounders. Spinopelvic parameters were assessed using radiographic findings in 2 groups. The analysis was done once as univariate (Kolmogorov-Smirnov test) and once as multivariate (multivariate logistic regression) analysis. Results Univariate analysis showed that female gender, higher BMI, smoking, and blue-collar jobs were associated with a higher risk of nonspecific LBP. LL, SS, and PI, but not PT, were all greater in LPB patients in the univariate analysis regarding the spinopelvic parameters. Multivariate analysis showed female gender (odds ratio adjusted (ORAdj) = 4.26 [95% CI, 2.11-9.58]; P = 0.001) and LL (ORAdj = 1.58; [95% CI, 1.18-3.22]; P = 0.026) were predictable risk factors for Nonspecific LBP. Conclusion Spinopelvic parameters, particularly LL, could be considered as risk factors of nonspecific LBP so that a more significant LL might indicate a greater risk of LBP. However, the role of other parameters in this association could not be neglected.
