Aim: to establish the percentage hospital admission related to adverse drug reactions (ADRs) from the data available in the literature. Method: literature search in the Medline database, metaanalysis. Results: from the literature it is revealed that a considerable part of all hospital admissions are related to adverse drug reactions. However, these data are not homogenous, i.e. larger studies display a lower percentage of ADR related hospital admission, while smaller studies display a higher percentage. Subgroup analysis showed that for elderly people the odds of being hospitalised by ADR related problems is 4 times higher than for younger ones (16.6% vs. 4.1%). A considerable part of these hospitalisations can be prevented. Subgroup analysis revealed that in the elderly up to 88% of the ADR related hospitalisations are preventable; for the non-elderly this is only 24%. Comparatively more elderly people are hospitalised than younger ones. Combining these findings, twice as much elderly people are hospitalised by ADR related problems than non-elderly, while preventability of ADR related hospitalisation might yield 7 times more people in the elderly than in the non-elderly. The estimation of the costs of ADR related hospitalisations in the Health Care system in The Netherlands is discussed. Conclusion: many elderly people are hospitalised by ADR related problems; an important part of these hospitalisations can be avoided.

I am a researcher and graduate student of American Indian descent (Stockbridge-Munsee/Mohican). I have always been very aware of the absence of other American Indian students in postsecondary education programs; have noticed the shortage of American Indians employed as educators, professors, or administrators; and am acutely aware of the number of American Indian populations excluded from qualitative or quantitative data sets as part of mainstream research agendas that are published on the state or national level. This is the beginning of my dissertation research journey, and the data that I have studied thus far demonstrates a ten-year failure of the traditional, Western, and public educational system in Wisconsin to nurture, graduate, and utilize Native Americans from birth through adulthood. This historical failure in the Wisconsin educational systems and organizations has left the First Children and the First People of this country far behind as compared to their other minority and white counterparts (Bowman ). The extent of how far and why they are behind is what I am determined to discover. Anecdotally speaking, I knew in my heart that the few Indians who did survive school accomplished this despite many barriers or whatever politically correct legislation was the popular rhetoric at the time. But in my head I needed to ascertain, scientifically document, and understand the factors that were responsible for this shameful record regarding the education of Native American students and their underrepresentation as professionals within the educational community. SimUniversity competition is an innovative Society in Europe for Simulation Applied to Medicine (SESAM) initiative which has existed since 2014, with the aim of creating opportunities for undergraduate healthcare students to take part in a formative educational experience on an international platform. The main educational focus is on promoting non-technical skills such as leadership, situation awareness, decision making, communication, and assertiveness, but also clinical reasoning within a team. In preparation for the 2021 virtual conference, the team designed a new methodology to meet the same mission, and yet be offered remotely.In this article, we describe the way in which we transformed the SimUniversity competition activity from face to face to a remote simulation. We relied on Zoom as the main communication technology to enable the distance component and followed the key elements of pre-briefing, simulation, and debriefing with the students being onsite together in one location and the faculty and simulator technologists in distant locations. Thirty-eight medical and nursing students formed 8 teams from 7 different countries. Two participating teams were based in Germany and one in Italy, Belgium, the Netherlands, Romania, Portugal, and Syria. Each team consisted of between 4 and 5 members and was self-selected to consist of either medical students alone or medical and nursing students together. The SimUniversity faculty team was composed of 5 physician educators, one nurse educator, one paramedic simulation technologist, and one industry simulation technologist. The faculty members facilitated each simulation synchronously in Zoom, while being based in different geographical locations within Europe (Germany, Switzerland, and the Netherlands) and the Middle East (Qatar and Lebanon).We conclude that assuming there is access to adequate internet connectivity and minimal technical setup, conducting a remote simulation with virtual debriefing is achievable in supporting team-based learning, particularly when learners and/or faculty members are in distant locations. While the authors do not recommend this method to be superior to a face-to-face experience, we propose this model to be an alternative method to consider when educators are faced with imposed restrictions such as what we faced during the COVID-19 pandemic. We discuss lessons learned and highlight other potential benefits that this method may provide, to consider even when the restrictions are lifted.

SimUniversity at a distance: a descriptive account of a team-based remote simulation competition for health professions students

BACKGROUND In December 2002, 74 medical students in the second preclinical years at the American University of Beirut (AUB) attended a workshop on basic communication skills (CS). The students watched video clips which demonstrated different communication skills and stages of the consultation, and role-played to try out ways they could improve the consultation. Their familiarity with the skills taught was assessed at the beginning of the workshop, immediately afterwards, and again six months later. There was a significant improvement noted immediately after the intervention but this apparent gain was lost at six months. METHOD Sixteen students, who attended the workshop, also attended two focus group discussions which were aimed to assess student recall of the communications skills workshop, and explore ways in which the students thought the knowledge gained could be maintained. RESULTS In general students thought that lack of exposure to patients at the time when the CS course was held, and a lack of consistency in CS coupled with poor faculty CS role modeling hindered their ability to maintain and practice good CS. CONCLUSIONS Teaching CS may be more effective if medical students are taught these skills when they are exposed to patients on a regular basis, and the training is maintained throughout the clinical years. However, more emphasis should be on reminding faculty members of their role as teachers and good communicating role models to students through workshops and seminars.

Focus groups findings reveal barriers to teaching communication skills to medical students

e12544 Background: Body Mass Index (BMI) is a significant prognostic factor in the survival of patients with hormone receptor-positive breast cancer (BC).[1-2] However, the relationship between BMI and survival is not well-established in hormone receptor-negative (HR-) breast tumors. [3] Emerging data is mixed: some studies show that BMI does not impact outcomes in HR- tumors [4], while others reveal worse survival in patients with elevated BMI.[5] In this study, our objective was to corroborate recent evidence about the relationship between BMI and mortality in hormone-negative breast cancer. Methods: We used the PLCO cancer screening trial, a publicly available database containing information about 155,000 participants enrolled in the United States between 1993 and 2001. Data were collected on cancer mortality through 2018 (median follow-up of 19.2 years post enrollment). The methodology of the PLCO trial has been described previously.[6] All female patients with a diagnosis of HR- BC diagnoses were selected for data retrieval, including HR-/HER2+ and HR-/HER2- tumors. Patients with equivocal receptor status were excluded. We used multivariate Cox regression to analyze mortality by BMI category, after adjusting for all significant variables at the bivariate level ( p <0.25). Results: 348 women with confirmed HR- BC at the time of study participation were included. Mean age was 69 years old, and 62 patients (17.8%) passed away due to breast cancer during study enrollment or follow-up. Patients who were of normal weight (BMI 25-<30) at age of trial enrollment were not significantly more likely to survive compared to overweight or obese patients (Hazard Ratio 1.274, p= 0.623). Conclusions: Our results show no significant relationship between BMI and mortality in patients with HR- BC tumors. This finding adds to the growing literature about weight status in hormone receptor-negative tumors. Our data suggests that the mechanism of tumor progression in HR- breast tumors is unlikely to involve signaling mechanisms in adipocytes.
