One fundamental stage is developing nursing diagnoses, which is a complex theme and not much explored in Neonatal Nursing. The potential use of computer technology in teaching and continuous education in nursing was the motivation for developing the present study with the objective to develop and evaluate an interactive virtual learning object about diagnostic reasoning in nursing applied to preterm newborns at the neonatal intermediate care unit. This descriptive study used the pedagogical framework of problem-posing and computer-assisted instruction, Bernardo's method of software development and Risner's thinking model to elaborate the nursing diagnosis, according to the NANDA-I taxonomy. The teaching simulations and learning exercises for diagnostic reasoning in nursing included real cases of preterm newborns, which were adapted and designed based on a model for developing clinical cases with educational means, and organized according to basic human needs. The cases were validated by five nursing professors, experts in neonatal nursing and diagnostic reasoning. The software was developed using the tool by Macromedia Authorware®, included in a virtual learning environment on the Internet, the Moodle, and media processed in specific software depending on their type. The virtual interactive learning object was organized in modules, consisting of a presentation, justification, objectives, technical record, image gallery, support tests and references; theoretical content about Nursing Diagnosis; automatic presentations of three clinical cases of preterm newborns at the neonatal intermediate care unit, totaling 32 Nursing Diagnoses, and simulation exercises of four clinical cases of preterm newborns, thus adding up to 40 Nursing Diagnoses. Appearance and content evaluations of the virtual object regarding the aspects related to presentation, organization, use and overall impression were performed by experts in computer technology (12) and nursing (31). Each sub-item from the instruments contained the evaluation on a Likert scale and a space for comments/suggestions. All items received positive evaluations by over 80% of the evaluators, except for the information density criterion on the evaluation by computer technology experts. Hence, the developed product is considered adequate to be used in students and continuous education in nursing about diagnostic reasoning in the elaboration of Nursing Diagnoses for preterm newborns, in the pedagogical framework of problem-posing.

Este trabalho caracterizou os idosos frequentadores de grupos de convivencia sob os aspectos fisico, social, psicologico e economico. Realizou-se estudo descritivo transversal com 197 idosos, utilizando o Brazilian Old Age Schedule modificado, com perguntas sobre saude fisica, servicos medicos, atividades da vida diaria, recursos sociais, economicos e saude mental. A maioria dos participantes era mulher, viuva, com escolaridade primaria, media de idade de 71,66 ± 6,8 anos; 51,27% consideraram sua saude boa, 85,28% referiram pelo menos uma doenca; 63,96% nao relataram quedas no ultimo ano e 56,85% relataram incontinencia urinaria. A maioria frequentava os grupos de convivencia de idosos ha mais de cinco anos; 65,99% apresentaram renda de ate um salario minimo e 24,37% apresentaram sintomas depressivos. Houve associacao da autopercepcao de saude com quedas e incontinencia urinaria; de quedas com problemas neurologicos, musculo-esqueleticos, digestivos e sintomas depressivos; de incontinencia urinaria com sintomas depressivos e atividades da vida diaria e de satisfacao com a familia e estado civil. As alteracoes de saude encontradas reforcam o perfil epidemiologico dos idosos e podem contribuir para o direcionamento de acoes para a saude coletiva dos mesmos.

[A profile of elderly members of community groups in Belo Horizonte, Minas Gerais State, Brazil].

In this article we present an overview of the publication about the health care in the family context. We developed a bibliographical research taking as source the following databases: SciELO, MEDLINE and Lilacs using the keywords: health care and family caregivers considering the period from 1995 through 2007. We realized that there are few studies on the subject and some of them are quite superficial in their approach. Our focus was directed to the understanding of: types of methodologies applied; theoretical approach; "care" concepts; analysis of culture and social class; references to networking; gender considerations and finally consensus and gaps in the studies. The results show that most of the studies assume a qualitative methodology compatible with the high complexity level of the studied subject and theoretical approaches of comprehensive character exploring meanings and conceptions. The discussion of the role of culture, social class distinctions and gender relations in the production and sourcing of health care arise once in a while but without the proper depth. The importance of the role of woman-mother in producing and providing health care is generally disregarded of criticism. We noticed a trend to use broader health care concepts that synthesize material, affective and social aspects, although without a discussion in a concrete way.

Produção de conhecimento sobre cuidados da saúde no âmbito da família

The study aimed to identify, in the view of the caretaker, the changes that happened in the family relationships after a dependence generator event on aged person, and the factors that provoked it. It is a field study with a qualitative approach. It was carried out recorded interviews with eight family caregivers, during home visits, with guiding questions and observations being recorded in a field diary. The content analysis of Bardin was applied, being constructed categories and subcategories. The familiar union preexisting to the dependence situation predisposes positive changes, favored with formal and informal support. The family caregiver burden due to lack of support, age and activities of caregiving, leads to negative situations in the family. Performed in a geriatric outpatient unit, the study used a content analysis methodology, and data were collected with interviews and 14 group sessions with 24 subjects. The following categories emerged: denial versus acceptance of the disease before and after diagnosis; how and why the main caregiver assumes this role; caregiver's characteristics; caregiver's experiences; meanings ascribed to care-giving; history of the caregiver's relationship to the elder before the disease; caregiver/elder relationship since onset of the disease; changes in the caregiver's life; changes in the family; and causes for the appearance of the disease. In conclusion, there are three dimensions in the target issue: (1) implications of the legacies, multi-generational transmission, repetition of the patterns, and myths and beliefs in each family system; (2) seniors as active participants in the care-giving decision; and (3) the support group as a key resource for the family.
