Kinematic alignment (KA) has increased in popularity in recent years, becoming a viable alternative to MA with encouraging short- and mid-term follow-up results. Recently, the concept of restricted kinematic alignment (rKA) has been developed to restore native knee kinematics better, avoiding failure of coronal alignment. This systematic review aims to examine whether rKA improves outcome scores (PROMs) compared with MA and to evaluate the radiographic analysis of the lower limb alignment and the causes of complications and reoperations with the rKA approach.A systematic literature search was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines on the Pubmed/Medline, Scopus, Cochrane Library, and Embase databases. The following key terms were used: "restricted kinematic alignment, rKA, kinematic alignment, primary total knee arthroplasty, primary total knee replacement, TKA rKA, and TKR rKA." The initial screening identified 328 studies. Each eligible article was evaluated according to the inclusion criteria: studies with levels of evidence (LoE) 1 to 4, written in English, published through May 2022, and involving human subjects. Criteria from the Methodological Index for Non-Randomized Studies (MINORS) were used to assess the methodological quality of the articles.Six clinical studies were included in this systematic review. The study was registered in the International Prospective Registry of Systematic Reviews (PROSPERO). A total of 574 knees were included. After excluding patients due to loss of follow-up or missing data, 475 knees were analyzed. The following rKA-related data were evaluated: patient-reported outcome scores (PROMs), radiographic analysis of the lower limb alignment, and causes of complications and reoperations.The rKA is an improved concept for restoring native knee kinematics, avoiding excessive coronal varus/valgus alignment. It provides equivalent or slightly better PROMs than MA without increasing the risk of short-middle-term implant failure. Clinical studies with extended follow-up are needed to confirm this trend. 

Restricted kinematic alignment in primary total knee arthroplasty: A systematic review of radiographic and clinical data.

The use of robotics in total knee arthroplasty (TKA) is growing at an exponential rate. Despite the improved accuracy and reproducibility of robotic-assisted TKA, consistent clinical benefits have yet to be determined, with most studies showing comparable functional outcomes and survivorship between robotic and conventional techniques. Given the success and durability of conventional TKA, measurable improvements in these outcomes with robotic assistance may be difficult to prove. Efforts to optimize component alignment within two degrees of neutral may be an attainable but misguided goal. Applying the "Wald Principles" of rationalization, it is possible that robotic technology may still prove beneficial, even when equivalent clinical outcomes as conventional methods, if we look beyond the obvious surrogate measures of success. Robotic systems may help to reduce inventory, streamline surgical trays, enhance workflows and surgical efficiency, optimize soft tissue balancing, improve surgeon ergonomics, and integrate artificial intelligence and machine learning algorithms into a broader digital ecosystem. This article explores these less obvious alternative benefits of robotic surgery in the field of TKA.

Moving beyond radiographic alignment: applying the Wald Principles in the adoption of robotic total knee arthroplasty

The mid-term results of kinematic alignment (KA) for total knee arthroplasty (TKA) using image derived instrumentation (IDI) have not been reported in detail, and questions remain regarding ligamentous stability and revisions. This paper aims to address the following: 1) what is the distribution of alignment of KA TKAs using IDI; 2) is a TKA alignment category associated with increased risk of failure or poor patient outcomes; 3) does extending limb alignment lead to changes in soft-tissue laxity; and 4) what is the five-year survivorship and outcomes of KA TKA using IDI?A prospective, multicentre, trial enrolled 100 patients undergoing KA TKA using IDI, with follow-up to five years. Alignment measures were conducted pre- and postoperatively to assess constitutional alignment and final implant position. Patient-reported outcome measures (PROMs) of pain and function were also included. The Australian Orthopaedic Association National Joint Arthroplasty Registry was used to assess survivorship.The postoperative HKA distribution varied from 9° varus to 11° valgus. All PROMs showed statistical improvements at one year (p < 0.001), with further improvements at five years for Knee Osteoarthritis Outcome Score symptoms (p = 0.041) and Forgotten Joint Score (p = 0.011). Correlation analysis showed no difference (p = 0.610) between the hip-knee-ankle and joint line congruence angle at one and five years. Sub-group analysis showed no difference in PROMs for patients placed within 3° of neutral compared to those placed > 3°. There were no revisions for tibial loosening; however, there were reports of a higher incidence of poor patella tracking and patellofemoral stiffness.PROMs were not impacted by postoperative alignment category. Ligamentous stability was maintained at five years with joint line obliquity. There were no revisions for tibial loosening despite a significant portion of tibiae placed in varus; however, KA executed with IDI resulted in a higher than anticipated rate of patella complications.Cite this article: Bone Jt Open 2022;3(8):656-665.

The kinematic alignment (KA) approach to total knee arthroplasty (TKA) has recently increased in popularity. Accordingly, a number of derivatives have arisen and have caused confusion. Clarification is therefore needed for a better understanding of KA-TKA. Calipered (or true, pure) KA is performed by cutting the bone parallel to the articular surface, compensating for cartilage wear. In soft-tissue respecting KA , the tibial cutting surface is decided parallel to the femoral cutting surface (or trial component) with in-line traction. These approaches are categorized as unrestricted KA because there is no consideration of leg alignment or component orientation. Restricted KA is an approach where the periarthritic joint surface is replicated within a safe range, due to concerns about extreme alignments that have been considered ‘alignment outliers’ in the neutral mechanical alignment approach. More recently, functional alignment and inverse kinematic alignment have been advocated, where bone cuts are made following intraoperative planning, using intraoperative measurements acquired with computer assistance to fulfill good coordination of soft-tissue balance and alignment. The KA-TKA approach aims to restore the patients’ own harmony of three knee elements (morphology, soft-tissue balance, and alignment) and eventually the patients’ own kinematics. The respective approaches start from different points corresponding to one of the elements, yet each aim for the same goal, although the existing implants and techniques have not yet perfectly fulfilled that goal.

Patient satisfaction following total knee arthroplasty using restricted kinematic alignment.

<h2>Abstract</h2><h3>Background</h3> Non-optimal patellofemoral relationships may influence treatment decisions during revision total knee arthroplasty (TKA). We performed this study to determine whether patellar implant malposition or patellar tilt is associated with inferior patient-reported outcome scores or patient satisfaction after primary TKA. <h3>Methods</h3> We identified 396 TKA patients (439 knees) from an institutional registry who had undergone patellar resurfacing, with preoperative and 6-week postoperative radiographs available, and patient-reported outcome measures (PROMs) completed at least 1 year after surgery (mean 505 days). Preoperative patient demographic characteristics, patient-reported expectations, National Institutes of Health - Patient Reported Outcomes Measurements Instrument Systems global health, Knee Injury and Osteoarthritis Outcome Score for Joint Replacement, and University of California Los Angeles activity scores were compared between 60 TKAs performed with non-optimal patellofemoral relationships (36 patellar implant malposition, 24 patellar tilt) and 379 TKAs performed with optimal patellar implant placement. <h3>Results</h3> There were no differences between the 2 cohorts regarding demographic features, preoperative radiographic disease severity, expectations, and PROMs; or postoperative tibiofemoral component alignment, PROMs, and patient-reported satisfaction (<i>P</i> = .48). Knee Injury and Osteoarthritis Outcome Score for Joint Replacement improved similarly (<i>P</i> = .62) for patients with optimal resurfacing (48.5-77.6 points) and non-optimal resurfacing (47.7-76.6 points). A similar proportion of optimal and suboptimal resurfaced patients reported being satisfied with their TKA (92.7% vs 88.1%, <i>P</i> = .29). <h3>Conclusion</h3> Although suboptimal patellofemoral relationships may prompt treatment considerations during revision TKA, the data obtained from this study do not suggest that patellar implant malposition or patellar tilt independently contribute to postoperative pain, functional limitation, or dissatisfaction. <h3>Level of Evidence</h3> This is a level III, retrospective cohort study.
