TL;DR: Echinococcal disease of the psoas has been very rarely reported, sometimes associated with paraspinal disease and often with vertebral involvement, and Cystic or complex retroperitoneal tumour, pyogenic abscess of thepsoas and even tuberculosis should be considered in the differential diagnosis.
