To establish the extent to which the paraspinal muscles are affected in idiopathic scoliosis, samples from patients must be compared with controls of a similar gender and age. To date, insufficient control data has been available for these purposes. The aim of this study was to redress this tissue, in order to identify whether one side of the apex of the scoliotic curve showed greater muscular abnormalities than the other. Bilateral samples of the paraspinal muscles were obtained during surgery from 14 female scoliosis patients, at the apex of the scoliotic curve at T9–T11. Percutaneous muscle biopsy samples were obtained from nine female volunteers, on the left side of the spine at T10. Samples were prepared for routine histochemistry for the identification of muscle fibre types. Fibre size was measured using computerised image analysis. Compared with control muscle, there was a significantly lower proportion of type I (slow-twitch oxidative) fibres in the muscle on the concave side of the scoliotic curve, but no difference on the convex side. The proportion of type IIB (fast-twitch, glycolytic) fibres was higher on both sides of the curve compared with controls, with the effect being significantly more marked on the concave side. The percentage of type IIA (slow-twitch, oxidative-glycolytic) fibres did not differ between the groups, and neither did fibre size (although there was a tendency for the controls to have larger type IIA fibres than the patients). Collectively, the differences in fibre type size and distribution meant that on the concave side the relative area of the muscle occupied by type I fibres was smaller, and on both sides of the curve the relative area occupied by type IIB fibres was greater and by type IIA fibres smaller, in comparison with controls. In scoliosis, the spinal musculature is most affected on the concave side of the curve’s apex. The muscle adopts a ‘faster’, or more ‘glycolytic’ profile, which would be consistent with a reduced low-level tonic activity of the muscle, perhaps consequent to a local change in activity on this side of the spine following progression of the curve. Less marked changes, in the same direction, are also evident on the convex side; these may be the result of general disuse of the paraspinal muscles associated with the spinal deformity.

The use of exercises for the treatment of Adolescents with Idiopathic Scoliosis is controversial. Whilst exercises are routinely used in a number of central and southern European countries, most centres in the rest of the world (mainly in Anglo-Saxon countries), do not advocate its use. One of the reasons for this is that many health care professionals are usually not conversant with the differences between generalised physiotherapy exercises and physiotherapeutic scoliosis-specific exercises (PSSE): while the former are generic exercises usually consisting of low-impact stretching and strengthening activities like yoga, Pilates and the Alexander technique, PSSE consist of a program of curve-specific exercise protocols which are individually adapted to a patients' curve site, magnitude and clinical characteristics. PSSEs are performed with the therapeutic aim of reducing the deformity and preventing its progression. It also aims to stabilise the improvements achieved with the ultimate goal of limiting the need for corrective braces or the necessity of surgery. This paper introduces the different 'Schools' and approaches of PSSE currently practiced (Scientific Exercise Approach to Scoliosis - SEAS, Schroth, Barcelona Scoliosis Physical Therapy School - BSPTS, Dobomed, Side Shift, Functional Individual Therapy of Scoliosis - FITS and Lyon) and discusses their commonalities and differences.

The aim of this report is the appraisal of a possible correlation of trunk asymmetry assessed with a scoliometer and lateralization of the brain as expressed by handedness in a school aged population. Many (8245) students (4173 girls and 4072 boys), 6–18 years of age were examined. A checklist was completed for each student including handedness and trunk asymmetry. The standing forward bending test was performed using the Pruijs scoliometer and the examined children were divided into three groups for each of the three examined regions (mid-thoracic, thoracolumbar and lumbar) according to the recorded asymmetry (no asymmetry, 2–7° and ≥7°). Ninety-one per cent of children were right-handed, while 9% were left-handed. A significant statistical correlation of trunk asymmetry and handedness was found both in boys and girls in the group of asymmetry 2–7° at mid-thoracic (p < 0.038) but not at thoracolumbar and at lumbar region. The effectiveness of core stabilization exercise in adolescent idiopathic scoliosis: A randomized controlled trial:

In order to monitor changes in postural performance capacity in patients with idiopathic scoliosis after an intensive in-patient Schroth rehabilitation programme lasting several weeks, we undertook electromyographic investigations in 316 patients with a mean age of 20 years (range 8-76 years) and a mean Cobb curvature angle of 38.2 degrees (range 10 degrees-147 degrees). Electromyographic activity was recorded by means of surface electrodes in the thoracic and lumbar region at the level of the apical vertebra, paravertebrally on both sides of the erector spinae muscle during trunk lifting from the prone position. Two hundred and fifty-nine recordings without artefacts were evaluated. Significant reductions in muscle activity of 6.85% in the thoracic convex region (P < 0.05) and of 14.2% (P < 0.001) on the lumbar convex side were found. The activity quotient (convesx/concave) was reduced by 11.99% (P<0.001) in the thoracic region and by 7.91% (P<0.01) in the lumbar region. These findings confirm the improvement of postural performance capacity after an intensive in-patient Schroth rehabilitation programme. As the imbalance of electromyographic activity may be influenced by scoliosis-specific exercises leading to a highly significant reduction of the Cobb angle, it is assumed to be secondary to the development of the scoliotic curve and may not be a primary factor in the aetiology of idiopathic scoliosis.
