<h3></h3> Мониторинг онкологических пациентов и оценка риска кардиотоксичности химиопрепаратов осуществляются с использованием стандартных диагностических методов: электрокардиографии, эхокардиографии с оценкой глобальной продольной деформации и анализа сердечных биомаркеров. Применение указанных методов позволяет выявить субклиническое поражение сердца до появления симптомов. Для достижения более высоких показателей выживаемости необходимо обеспечить доступность диагностических методик, особенно для пожилых пациентов, используя одноканальный ЭКГ-монитор.

Influence of the Hand Position and the Pulse Oximeter Sensor Position on the Pulse Blood Filling Signal

BACKGROUND AND AIMS: Diastolic dysfunction of the left ventricle is a precursor to heart failure with preserved ejection fraction (HFpEF). Detection by electrocardiography (ECG) when asymptomatic (stage B heart failure) would be valuable. We hypothesised that an explainable advanced ECG (A-ECG) score derived from standard 12-lead ECG could accurately diagnose diastolic dysfunction. METHODS: Included patients had undergone resting 12-lead ECG and echocardiography demonstrating normal systolic function, with at most mild valve disease, and either the presence (n=150) or absence (n=264) of grade II or III diastolic dysfunction. Stepwise multivariable logistic regression was used to generate an A-ECG score that was cross-validated using bootstrapping. RESULTS: A 6-measure A-ECG score was able to identify diastolic dysfunction with an area under the receiver operating characteristics curve [95% confidence interval] of 0.91 [0.88-0.94], sensitivity 83 [76-91]%, specificity 87 [77-92]%, positive predictive value 78 [69-85]%, negative predictive value 90 [86-94]%, positive likelihood ratio 6.38 [3.30-11.38] and inverse negative likelihood ratio 5.12 [3.21-10.22]. CONCLUSIONS: Standard 12-lead ECG can be used to accurately identify diastolic dysfunction by echocardiography via A-ECG. This may have clinical utility for early identification of patients who may benefit from further cardiac assessment and risk factor management to prevent progression to symptomatic heart failure.

Diagnosing diastolic dysfunction using explainable advanced electrocardiography

Aim      To evaluate the incidence of prolonged corrected QT interval (QTc) by remote single-channel electrocardiogram (ECG) monitoring in primary oncological patients with elective polychemotherapy (PCT).Material and methods  This study included 49 oncological patients with elective PCT. A single-channel portable CardioQVARK electrocardiograph was used to record single-channel, one-lead ECG between the first and second courses of PCT.Results Analysis of QTc interval detected a prolonged QTc interval >500 msec in 8.2 % of cases, prolonged QTc >480 msec in 18.3 % f cases, and prolonged QTc interval >60 msec compared to baseline in 12.2 % of cases.Conclusion      Remote recording of single-channel ECG using a portable electrocardiograph is an effective method for recording and detecting various forms of heart rhythm disorders.

[Remote Monitoring of the QT Interval During Polychemotherapy].

In the article the influence of the hand position and the pulse oximeter sensor position on the measured pulse blood filling signal was examined. The photoplethysmography method was used to register pulse waves. This method was implemented on the basis of the ARDUINO UNO board and the Pulse Sensor photoplethysmographic sensor. To study the influence of the hand position, signals were recorded from the phalanx of the finger and the radial artery of the right hand in three positions: the hand lies on the table, the hand is lowered down and the hand is raised up. The influence of the position of the sensor, in its turn, was examined on the basis of pulse waves recorded from the artery and at some distance to the left and right of it. All signals were processed in Matlab with the use of the previously developed algorithm for analyzing the contour characteristics of the wave. The processing results show that the higher the hand is positioned, the smaller the amplitude of the measured wave for both vessels. A sensor shift of 1 centimeter does not have a significant effect on the shape of the recorded signal, however, a shift to a greater distance can lead to incorrect diagnostic results.

Pulse wave parameters in assessment of left ventricular systolic function

Aims: To investigate the potential of a signal processed by smartphone-case based on single lead electrocardiogram (ECG) for left ventricular diastolic dysfunction (LVDD) determination as a screening method. Methods and Results: We included 446 subjects for sample learning and 259 patients for sample test aged 39 to 74 years for testing with 2D-echocardiography, tissue Doppler imaging and ECG using a smartphone-case based single lead ECG monitor for the assessment of LVDD. Spectral analysis of ECG signals (spECG) has been used in combination with advanced signal processing and artificial intelligence methods. Wavelengths slope, time intervals between waves, amplitudes at different points of the ECG complexes, energy of the ECG signal and asymmetry indices were analyzed. The QTc interval indicated significant diastolic dysfunction with a sensitivity of 78% and a specificity of 65%, a Tpeak parameter >590 ms with 63% and 58%, a T value off >695 ms with 63% and 74%, and QRSfi > 674 ms with 74% and 57%, respectively. A combination of the threshold values from all 4 parameters increased sensitivity to 86% and specificity to 70%, respectively (OR 11.7 [2.7-50.9], P < .001). Algorithm approbation have shown: Sensitivity—95.6%, Specificity—97.7%, Diagnostic accuracy—96.5% and Repeatability—98.8%. Conclusion: Our results indicate a great potential of a smartphone-case based on single lead ECG as novel screening tool for LVDD if spECG is used in combination with advanced signal processing and machine learning technologies.

A Screening Method for Predicting Left Ventricular Dysfunction Based on Spectral Analysis of a Single-Channel Electrocardiogram Using Machine  Learning Algorithms

<h3>ЦЕЛЬ ИССЛЕДОВАНИЯ</h3> Определение диагностической точности алгоритма выявления клинически значимой (2-й и 3-й ст.) диастолической дисфункции левого желудочка (ЛЖ) с помощью анализа одноканальной электрокардиограммы, разработанного на основе элементов машинного обучения. <h3>МАТЕРИАЛ И МЕТОДЫ</h3> В исследование проспективно включены 200 пациентов старше 18 лет. Всем было проведено трансторакальное эхокардиографическое исследование с определением диастолической функции левого желудочка с применением скоростной и тканевой доплерографии. Регистрацию электрокардиограммы проводили с помощью одноканального монитора. После спектрального анализа электрокардиограммы были рассчитаны временные, амплитудные и частотные параметры. Проведена оценка модернизированного алгоритма определения значимой диастолической дисфункции ЛЖ, учитывая результаты, полученные на обучающей выборке пациентов. <h3>РЕЗУЛЬТАТЫ</h3> Средний возраст пациентов составил 53,2±18,1 года. У 31% больных была выявлена диастолическая дисфункция ЛЖ средней и тяжелой степени. Чувствительность и специфичность алгоритма в выявлении диастолической дисфункции ЛЖ 2-й ст. составили 98,3% и 97,8%, диастолической дисфункции ЛЖ 3-й ст. — 87,5% и 99,5%, диастолической дисфункции ЛЖ 2-й и 3-й ст. — 95,6% и 97,7% соответственно. Диагностическая точность составила 97,5%, 99,0% и 96,5% соответственно. <h3>ВЫВОД</h3> Достигнутые результаты обосновывают принципиальную возможность удаленного скринингового определения диастолической дисфункции ЛЖ на основании параметров электрокардиограммы. 

A new machine-learning algorithm for assessing the left ventricular diastolic dysfunction by a single-channel ECG monitor

<h3>ЦЕЛЬ ИССЛЕДОВАНИЯ</h3> Разработка моделей машинного обучения для определения снижения систолической функции миокарда левого желудочка (ЛЖ) по данным электрокардиограммы (ЭКГ) и фотоплетизмограммы (ФПГ), зарегистрированным с помощью одноканального ЭКГ-монитора с функцией фотоплетизмографии. <h3>МАТЕРИАЛ И МЕТОДЫ</h3> В исследование были проспективно включены 400 пациентов. Каждому участнику исследования была выполнена эхокардиография, при которой определяли фракцию выброса (ФВ) ЛЖ и VTI выносящего тракта ЛЖ. Затем проводили регистрацию ЭКГ и ФПГ одноканальным ЭКГ-монитором с функцией фотоплетизмографии, который имеет вид чехла для смартфона. Затем все зарегистрированные записи передавали на единый сервер, где проводили расчет параметров ЭКГ и ФПГ. На основе полученных параметров были построены модели для оценки прогнозирования снижения систолической функции ЛЖ с применением регрессии Лассо и алгоритма «случайный лес». <h3>РЕЗУЛЬТАТЫ</h3> Были получены модели для ФВ менее 55, 40, 30% и VTI менее 16 и 13 см соответственно. Для каждой модели рассчитывали площадь под ROC-кривой (AUC), чувствительность, специфичность. Для моделей с применением регрессии Лассо результаты были следующими: для ФВ &lt;55% AUC составила 0,857 (чувствительность 0,818, специфичность 0,860); для ФВ &lt;40% — 0,971; для ФВ &lt;30% — 0,982; для VTI &lt;13 — 0,754, для VTI &lt;16 — 0,746. Для моделей, построенных на основе алгоритма «случайный лес», результаты были также достаточно высокими: для ФВ &lt;55% AUC составила 0,913; для ФВ &lt;40% — 0,955; для ФВ &lt;30% — 0,962; для VTI &lt;13 — 0,776, для VTI &lt;16 — 0,782. <h3>ВЫВОД</h3> Модели на основе машинного обучения, построенные с использованием параметров ЭКГ и ФПГ, показали достаточно высокую точность в оценке снижения систолической функции ЛЖ.
