Abstract: Increasing hypertriglyceridemia is a recognised complication of CAPD. To investigate the etiology lipid clearance studies using the intravenous fat tolerance test were performed in control subjects and in uremic patients before and after six months CAPD treatment. Oral carbohydrate intake was restricted and the use of hypertonic dialysate kept to a minimum. Although serum triglycerides were elevated in the uremic patients before CAPD (3.1 +/- 0.3 mMol/L) no further increase occurred during therapy. Oral carbohydrate intake was restricted to 240-250 Gms/24 hrs and hypertonic dialysate to 5.7 +/- 4.1% of the total. There were no significant changes in fractional removal rate of lipid. Therefore further increases in serum triglycerides in CAPD may be prevented by simple dietary manoeuvres.

Abstract: One of the disadvantages of indices derived from ultrasonic Doppler signals is that they are not normally available instantaneously. In an attempt to overcome this problem, a real-time spectrum analyser has been interfaced to an Intel 8085 microprocessor system and programmed to calculate pulsatility index in real time. The only additional patient signal required is a simple three-lead e.c.g. which is used for timing purposes. Although the device has only been in clinical use for a short time it has proved to be very reliable and most useful.

Abstract: Catheterization of the elderly incontinent patient is frequently unnecessary. It is also often unsuccessful, as most patients have unstable bladders and therefore leakage continues around the tube. Simple measures, like toileting regimens and restricting fluids in the evening, are often effective alone. Incontinence garments restore a patient's self-respect, reduce laundering and should be used more frequently whilst other methods, such as drugs, are being tried.

Abstract: This paper reviews the proper technique which should be adopted by the nurse in order to obtain accurate blood pressure measurements. Although this is difficult using conventional instruments, statistics uniformly confirm the harmful significance of an elevated blood pressure. Any nurse delivery high quality care must be able to carry out nursing procedures skillfully and accurately. The methodology of blood pressure recording is discussed, analysed and evaluated. It is well-documented that there are many potential sources of error in recording blood pressure, including poor technique and observer bias. The paper deals with these and highlights some of the dilemmas that this technique presents to the nurse. A claim is made that many nurses are often inadequately trained in blood pressure measurement, and with increasing reliance on the nurse for records, much more attention need to be focused on this area.

Abstract: The function of the renal tubule is reabsorption of glomerular filtrate, which is accomplished with considerable efficiency; 98 to 99 per cent of all filtered water, sodium and bicarbonate are reabsorbed. Similarly filtered protein, glucose and amino acid are reabsorbed in order to preserve body stores. Reabsorption mainly occurs in the proximal tubule, but the final regulation occurs in the distal tubule.
