Abstract: : Hypertension (HTN) is one of the most prevalent, modifiable and treatable causes of cardiovascular diseases in the elderly. Damage to cardiovascular system due to chronic hypertension causes hypertensive heart disease including left ventricular hypertrophy (LVH). It is a physiological adaptation to chronic blood pressure (BP) indicating grave prognosis and should be diagnosed early. Present study therefore aimed to assess LVH and other Cardio-vascular (CV) changes and to ascertain correlation between the electrocardiographic (ECG) and radiographic / chest x-ray (CXR) evidences and find out their determinants among elderly hypertensive patients. 100 elderly hypertensive subjects were included in study. Cardio-vascular changes and LVH was assessed by using chest x-ray and ECG. Other bio-chemical investigations were done also.: 29% and 32% subjects had LVH detected by Chest x-ray and ECG respectively. Female, increasing age, family history and duration of HTN, smoking, alcohol consumption were found independent determinants of LVH. Subjects with LVH had higher mean blood pressures, mean pulse pressure and higher blood urea nitrogen than those without LVH. Micro-albuminuria was significantly more frequent in patients with LVH as detected by CXR. Fasting Blood sugar and dyslipidaemia was associated with greater odds of developing LVH. In low resource settings, where standard diagnostic tests are unavailable- chest x-ray and ECG are the useful tools for detection of cardio vascular changes / LVH among chronic hypertensive.

Abstract: As the field of medicine tries to tap into various facets of gamification, the potential of video games and gaming-based interventions has become a hotbed of interesting activity in the Dementia circles of medical research. This paper, exploratory in nature is a bid to find common grounds for the fields of neurosciences, psychology, and psychiatry with the sub-field of ludology in a hope to find unique insights from the benefits of interdisciplinary research and extrapolating findings from gaming addiction.From the perspective of game studies, the paper will draw from elements of concepts like flow, immersion and engagement in a bid to understand the various tools that can be of use if people ailing from neurocognitive disorders were to engage in gaming activity. This paper will lay the framework that will enable the beginning of a matching gaming exercises based on cognitive deficits and psychological profiling of clients diagnosed with neurocognitive disorders and advance the fields of neurosciences and psychology.By combining the findings from existing studies and original work this paper will propose both a means to study the possible effects of gaming-based interventions and suggest ways to streamline and optimize the use of gamification in patient care.

Abstract: Gallbladder cancer is frequently transmitted to the liver and other gastrointestinal organs through direct extension. Ovarian metastases of biliary origin are quite uncommon. We report a rare case having Krukenberg tumours in both ovaries, an uncommon metastatic location of biliary cancer. A thorough analysis of the proposed tactics was carried out after a thorough critical review. The final diagnosis was made using a combination of radiology, histology, and serum markers. In the case of gallbladder cancer, the presence of a Krukenberg tumour should be examined.

Abstract: Sleromyxedema is a rare disease characterised by extensive mucin deposition with fibrosis in dermis. It is often associated with monoclonal gammopathy. Its true prevalence and/or cause is unknown due to its rarity. Various extra-cutaneous manifestations can be associated with sleromyxedema such as dermoneuro syndrome, neuropathy, rheumatological problems or various malignancies. This case report showed that a patient with sleromyxedema and neuropathy had positive outcomes when treated with intravenous immunoglobulin (IVIg) and steroids pulses. The underlying cause of the patient’s condition was unknown.

Abstract: Cognitive disorders are common in elderly population and are becoming an increasingly important public health problem, partly because of the rapid aging of the population. This study was conducted to find out the differences between cognitive ability between normal adults and healthy elders of north region. The mild cognitive impairment (MCI) resulting in limitations and delayed treatment of dementia, should be considered an entry point for researching recent changes in the lives of healthy elderly. In this study we have applied MoCA test on the 36 normal healthy elders belonging to high socioeconomic status and normal young adults. Results have shown that there was no significant difference amongst the young normal adults; all the participants had normal MoCA scores. The MoCA scores were significantly impaired in all the healthy elders and there was a significant difference between the normal young adults and healthy elders. Age has a significant influence on MOCA score in older adults. Hence there is a need for age specific stratification in cut-off scores.

Abstract: CT scoring helps to stratify patient’s risk and predict short-term outcome of patients with COVID-19 pneumonia. we aim to assess the severity of COVID by examining the variations and degree of the pulmonary involvement on the HRCT thorax. We observed trend of increasing severity of COVID as predicted on HRCT in Eastern India population.HRCT Thorax of 2000 consecutive COVID positive patients were analysed to look for the patterns of lung involvement and to calculate a CT severity score. The CT severity score was compared with laboratory parameters like D-Dimer, CRP, ESR, Procalcitonin, WBC and Lymphocyte count by Pearsons correlation analysis.Out of 2000 COVID positive patients 1220 (61%) patients were found to be radiologically positive on HRCT chest imaging while 780 patients (39%) had normal CT findings.Out of the 62 patients who died, 54 ( 87%) were having CTSS of > 15. Among the dead patient the prevalence of one or more comorbidities was significantly higher (49/62 patients, 79%) The proportion of patients having CT severity score between 16-25 (Severe patients) had increased from 4% in April-May to 27% in August. Trend of severity among COVID patients increased as the months passed from April to August. Most of the deaths in our cohort were associated with pre-existing comorbidities suggesting that the COVID was not the sole cause of these deaths. Among the biochemical markers D-Dimer and CRP correlated well with the CT severity score.

Abstract: : Antibiotics play a monumental role in infectious diseases control and management and have been used in all drug treatment strategies. But the inappropriate use and practice of antibiotics among health professionals results in adverse effects and accelerates the growth of microbial resistance against the majority of effective antibiotics. This study aimed at assessing the knowledge and practice of antibiotic awareness and its usage among staff nurses working in hospitals and also to identify the association of demographic factors on knowledge and practice of antibiotic awareness and its usage.: A descriptive cross-sectional survey design was used. A total of 100 staff nurses (4 males and 96 females) were selected using a convenience sampling method. Data was collected using a questionnaire to assess the knowledge and a rating scale for practice about antibiotics awareness and usage.About 50 percent of the participants demonstrated good knowledge of antibiotic awareness and its usage, and 72 percent had very good practice of antibiotics usage in hospitals. A highly significant, weak positive correlation (r=0.316, at p ˂ 0.01) between knowledge and practice of antibiotic awareness and usage among staff nurses was obtained. A significant association of gender with practice (χ =59.64, p = 0.002) and educational qualification with knowledge was found (χ=53.82, p = 0.017).: The study results suggested that staff nurses in general have good knowledge of antibiotic awareness and usage and very good practice in antibiotic usage. However, there should be effective training programs to consistently keep the knowledge and practice of staff nurses regarding antibiotic awareness and its practice at a satisfactory level and to raise awareness among those who demonstrated less knowledge and practice for its judicious use and practice.

Abstract: Management of masks disposal in the COVID-19 era is a highly ignored segment in public health management and clinical care, which might be due to the unexpected emergence of pandemic and unpreparedness of the system. Meanwhile with over time newer information has evolved, now authorities should focus on adequate management of the masks wastes, especially at community levels. Some of the hurdles and suggestions are discussed here in this article.
