Abstract: What a great start to 1999. Papers bursting with stories describing ‘trolley jams’ in the corridors of many of the country's hospitals as patients wait several hours before an appropriate in-patient bed becomes available; the relatives of patients being asked to help ease the workload on nurses by washing, dressing and feeding patients themselves, and there seems little dignity for the deceased either with refrigerated lorries being employed by hospitals with overflowing mortuaries.

The King's Fund has been working with four London pilot sites on their local evaluations, and this paper presents early messages from the first year of data collection.
