The Health Service Journal (27 January 2005) reports that the DoH's technical guidance on practice-based commissioning will tell PCTs that they must refuse to allow practices to move to practice-based commissioning if they do not adopt the NPfIT's Choose And Book software for the GPs’ desktops.

Abstract: Conspiracy or cock-up? Let us first assume that the objective of the NHS is to deliver health care efficiently to patients in relation to their relative need, with need being defined as their capacity to benefit from care per unit of cost. The achievement of this objective requires knowledge of the cost-effectiveness of competing treatments and wheezes being offered by medical practitioners and policy makers who shape the delivery and funding of health care. Success in the pursuit of these goals results in the £70 billion-plus being spent on the NHS producing the greatest possible improvement in the health of the UK population.
