Abstract: BACKGROUND: Mycosis fungoides (MF) are rare cutaneous T-cell lymphomas with an indolent long-term clinical course in elderly patients. The more advanced stages of the illness may require systemic chemotherapy, extracorporeal photophoresis (ECP) and/or radiotherapy (including total body irradiation), denileukin diftitox, alemtuzumab and allogeneic stem cell transplantation. ECP is unique among these treatment modalities because it specifically induces an immune reaction directed against T cells.METHODS: We report a case of a 61-year-old Caucasian woman with MF, non-responder to the first line treatment, treated with ECP with very well fastly response.RESULTS: The treatment was evaluated with several parameters and among these, the one that gave the greatest correspondence to the clinical course of the illness was IL2.CONCLUSIONS: ECP is a tolerable treatment and free of the side effects that may occur with conventional chemotherapy, therefore it should be used more frequently, especially in MF, where it is frequent that patients have to undergo multiple treatment lines for indolent trend that characterizes it.

Abstract: BACKGROUND: Valproic acid is one of the most common antiepileptic drugs administered for epilepsy in children. It has a lot of adverse effects, and nocturnal enuresis is one of them. An association between the incontinence/nocturnal enuresis and valproate is not well defined in the literature. We aimed to investigate the results of uroflowmetry in VPA administered children who had nocturnal and/or diurnal incontinence.METHODS: Twenty-one patients were included in the study who were followed up with the diagnosis of epilepsy and who had nocturnal enuresis and/or nocturnal enuresis and daytime incontinence with increased micturition after the initiation of VPA. Incontinence symptoms scores and results of uroflowmetry were recorded for each patient as well as detailed physical examination, laboratory and radiologic investigationsRESULTS: After the initiation of VPA; 16 patients had only enuresis, five patients had enuresis and day-time incontinence with increased frequency of micturition. Except for one patient with staccato pattern, uroflowmetry results were normal in all 20 patients. No significant difference was found between patients who had only nocturnal incontinence and those who had both nocturnal and daytime incontinence in terms of uroflowmetry results.CONCLUSIONS: Enuresis and daytime incontinence are a distressful problem both for children and parents, so families should be questioned about the presence of enuresis and/or day-time incontinence before the initiation of the VPA treatment.

Abstract: BACKGROUND: High-energy demand for adolescent athletes not followed by adequate dietary intake can effect metabolism function in the body. The aim of this study was to investigate the effect of intakes from cow milk as an additional energy sources for metabolism (physiologic aspect), before and after performed physical activities.METHODS: Twenty-four 18-21 years old soccer athletes, with no allergy history toward cow milk experienced soccer competition in regional level at least 3 years, with routine physical activities between 3-5 times a week. Athletes were divided randomly into three group (fresh cow milk, pasteurization milk and mineral water) that consumed for three weeks respectively. This research used Pretest-Posttest Randomized-Groups Design, with doing test twice that is pre-test (before beverages consumption) and post-test (after beverages consumption three weeks respectively). Every pre-test and post-test always doing in 2.4 km Cooper test. Before (p1) and after (p2) 2.4 km Cooper test measurements for blood lactate were performed.RESULTS: Fresh cow milk (1.49±1.23) and pasteurization milk (1.71±0.94) gave significant effect (P<0.01) toward blood lactate before 2.4 km Cooper test (p1). After 2.4 km Cooper test (p2), only pasteurization milk (10.08±6.71) gave significant effect (P<0.05) withmblood lactate lower than mineral water (14.60±4.10) and fresh cow milk consumption (12.08±3.03).CONCLUSIONS: Pasteurization milk gives significant effect toward physiological system, in case of blood lactate, with the enormous influence seen when prevent lactate accumulation before the 2.4 km Cooper test. And lower blood lactate after 2.4 km Cooper test may help athletes to do subsequent physical activity without feeling significant fatigue.
