Abstract: Injections of 0.01% carbachol and 1% acetylcholine chloride (Miochol) into the anterior chamber in round pupil cataract extractions prevent the formation of peripheral anterior synechia but do not preserve the integrity of the vitreous face. Carbachol (0.01%) is as effective for prompt miosis as acetylcholine (1.0%), but the miosis is more prolonged with carbachol.

Abstract: Filamentary keratitis, a little-known complication of cataract surgery, caused significant symptoms in the patients it affected. In most instances, the filaments disappeared in a week or two with therapy consisting of filament removal with (0.5%) silver nitrate impregnated cotton-tipped applicators. However, in two out of eight patients, the filaments recurred up to four years after surgery, although the visual acuity usually was not significantly affected.

Abstract: Pupillary-block flat anterior chamber (AC) requiring iridectomy was found to be a unilateral condition. It was present in only 0.27% of eyes among 8,533 cataract surgeries. Pupillary-block flat AC presented within two weeks of cataract surgery in 16 of 21 eyes (76%). Patients with preexisting chronic simple glaucoma or diabetes were more susceptible to develop it. In 50% of eyes with flat AC, and prior to iridectomy, the intraocular pressure was 20 mm Hg or higher. Iridectomy reformed the AC depth in 19 eyes but additional iridectomy was required in two eyes. Vitreous loss did not occur during iridectomy. Secondary glaucoma developed in five eyes and late iris prolapse in three eyes, both complications resulting from prolonged periods with flat AC.

Abstract: The lens was removed using the ab externo approach with a limbal-based flap and corneo-scleral section, followed by a round pupil cryoextraction with or without zonulysin. The zonulysin used was o05 to i ml. Chymozym (Ikapharm) containing I63 to 332 Anson proteolytic units. Postoperatively the patients received atropine and antibiotic drops. Steroid drops were started on the third postoperative day.
