Abstract: The authors analyzed the results of palliative irradiation in 102 cases of primary inoperable rectal and rectosigmoid cancer and recurrent cancer of the rectum. Symptomatic relief followed irradiation in 80% of the patients; overall survival was 5%, colostomy was avoided in 71%. In 13 of 16 cases of primary inoperable carcinoma, treated intensively with the hope of conversion from inoperability to operability, the tumor regressed sufficiently for exploration; 9 neoplasms could be resected. Of the 13 patients, 23% were alive at 60 months (18% of the 16).

Abstract: Between 1965 and 1967 splenectomy was performed on 21 Watuts and 5 other New Guineans to relieve persistent incapacitating symptoms of tropical splenomegaly syndrome. There were 3 operative deaths. Post-operatively abolition of the splenic red cell pool, correction of the shortened red cell survival time and reduction in plasma volume occurred, leading to a mean rise in haemoglobin concentration of 3·7 g./100 ml. Neutropenia and thrombocytopenia were also corrected. There was a rapid post-operative fall in serum IgG and IgM concentrations, and a prolonged reduction in intravascular pools of both immunoglobulin fractions. The 18 Watuts discharged from hospital and followed for from 41 to 74 months have had significant symptomatic relief, although 7 have died following short illnesses; this mortality rate (36%) is substantially less than that of a comparable group of Watuts not subjected to splenectomy.

Abstract: Although noninfiltrative ophthalmopathy can be seen with hyperthyroxinemia on any basis, infiltrative ophthalmopathy is peculiar to Graves' disease. With infiltrative ophthalmopathy involvement of extraocular motor muscles can cause shortening of the levator palpebrae superiorus and fixed upper eyelid retraction. Paresis of other muscles may cause diplopia or strabismus. Complications of proptosis include corneal damage from exposure and optic nerve injury from traction or compression; from venous engorgement: chemosis, subconjunctival hemorrhage, orbital pain, and glaucoma. Restoration of the euthyroid state improves noninfiltrative ophthalmopathy. For mild infiltrative disease, local and mechanical measures may provide symptomatic relief. Adrenal steroids are indicated for progressive infiltrative ophthalmopathy. The principal indications for surgical intervention are cosmetic, for correction of diplopia, or for decreased vision. For best results, surgery must be properly timed and planned.

Abstract: The efficacy of 4 different escalating dosage schedules of synthetic estrogen (ethinyl estradiol or EE) for control of perimenopausal symptoms is assessed in 338 patients aged 35 to 72 during 2744 treatment cycles. Doses used ranged from 0.025 to 0.15 mg daily of EE and 10 mg daily of medroxyprogesterone acetate was added to the regimen for the terminal 7 days of each schedule (randomly selected patients taking schedule C did not receive the progestogen to allow evaluation of endometropic effects of the regimen). 57 of the patients had had hysterectomy with bilateral salpingo-oophorectomy. In most patients symptomatic relief was achieved within 4 to 8 weeks of therapy and maintained during treatment period. 22% of patients taking schedule D (Profen) had mild recurrence of symptoms (sensations of warmth sweating and flushes) during therapy but episodes were self-limited disappearing within a few weeks without change in medication. Of 59 patients on Schedule A (Meno-tabs) 5 experienced slight symptomatic relief 41 had moderate relief and 13 had marked relief. Of 21 patients in Schedule B (15-AT) 4 had slight relief 13 moderate relief and 4 marked relief. Of 18 patients on Schedule C (52BA) 1 had slight relief 13 moderate relief and 4 marked relief. 87 Scheule D patients experienced marked relief--the best results observed among the 4 treatment groups. Medroxyprogesterone acetate component of the regimen induced progestational effects in most endometrial biopsies obtained from Schedule C patients and prevented hyperplastic changes in all. There were no reported serious side effects. The study suggests that the escalating dose estrogen with added MPA is an effective and safe treatment for perimenopausal symptoms.

Abstract: The administration of anisotropine methyl bromide to patients with a variety of gastrointestinal disorders, most of which were functional gastroenteropathies, brought symptomatic relief only to a minority. These disappointing results were reflected further in the outcome of a double-blind study in 100 patients in whom, when the randomized drug numbers were decoded, therapeutic responses and side effects were not significantly different from those of patients receiving a placebo.

Abstract: Differential diagnosis of dizziness is important to determine if the problem is in the ear or in the central nervous system or other organ systems. Vertigo can be caused by any disorder that unilaterally reduces vestibular function, such as tumor, trauma or infection, but the most common cause is Meniere's disease. Drug therapy usually provides symptomatic relief, but in cases of incapacitating Meniere's disease, surgery may be necessary.

Abstract: A total of 3,000 patients in general practice, with a diagnosis of moderate to severe anxiety with depressive overtones were treated with benzoctamine 10 mg tid for two weeks.During this period 75 % of the group obtained symptomatic relief, and 45 % were symptom free after two weeks.The profile of patients responding significantly well to this treatment was studied in detail.In this group of patients the drug was well tolerated, 11 % of the patients reporting no side effects. Drowsiness was the most commonly reported adverse reaction with an incidence of 9 %.
