Abstract: Patellar tendinitis can be considered as an overload lesion in athletes. Anatomical pathology reveals focal degeneration and microtearing at predisposed areas near the insertion of the quadriceps or patellar tendon. Thie prognosis and treatment are dependent on the stage of the affection. A program of conservative treatment is effective in the early stages of the disease but it fails in a high proportion (16/38) of cases in the later stages. Surgery directed towards the tendon, rather than a bony procedure, yielded favourable results in 27 out of 29 patients.

Abstract: The authors examined 34 tendons under the electron microscope, excised within 48 h after rupture of the tendons. The tendons did not exhibit any signs of inflammation or lipomatosis, but fine-structurally marked hypoxic alterations in the tenocytes could be seen. The degenerative (hypoxic tendinopathy appears in three phases, and the hypoxic lesions of the tendon occur at multiple foci or diffusely, and the various phases can exist simultaneously.

Abstract: Address for further communications: Dr. A. Chechick, Dept. of Orthopaedic Surgery A, Chaim Sheba Medical Centre, Tel Hashomer, Israel game, spontaneous rupture of the Achilles tendon occurred. The patient was operated upon the same day. An irregular tear of the Achilles tendon was found five centimetres proximal to its insertion. It was repaired by end to end suture with gastrocnemius fascia rotation flap reinforcement, as described in 1954 by Bosworth. Following six weeks of plaster immobilisation and an additional six weeks of physiotherapy, full sports activity was gradually resumed.
