A dual-respiration chamber system with automated calibration

The measurement of basal metabolic rate (BMR) is an important element in the estimation of energy requirements in man. Based on a survey of 2822 BMR measurements conducted in the tropics, a series of new predictive equations to estimate BMR in tropical peoples is presented. These new equations based on bodyweight show that the current FAO/WHO/UNU predictive equations overestimate BMR of peoples living in the tropics by an average of 8 per cent, and by up to 11.5 per cent for males over 30 years old. These differences were significant over between 65 and 100 per cent of the normal bodyweight range found in our data, for both males and females over 10 years old. However, for younger ages, 3-10 years, the FAO/WHO/UNU equations predicted BMR quite accurately. The inclusion of height made no significant difference to the predictive equation. Although caution must be exercised in interpreting these results, it is concluded that the lower BMR in the tropics appears to be a general and genuine phenomenon in adults. Further discussion of energy requirements in tropical peoples must consider these observed deviations in BMR from the FAO/WHO/UNU predictive equations.

New predictive equations for the estimation of basal metabolic rate in tropical peoples.

Since females have a greater prevalence of obesity compared with males, the question arises whether females have lower metabolic rate than males after adjusting for differences in body weight and composition. 24-h energy expenditure (24EE), basal metabolic rate (BMR), and sleeping metabolic rate (SMR) were measured in a respiratory chamber in 235 healthy, nondiabetic Caucasian subjects (114 males, 121 females). Body composition was determined by hydrodensitometry. 24EE was 124 +/- 38 kcal/d (P less than 0.002) higher in males than females after adjusting for differences in fat-free mass, fat mass, and age. Spontaneous physical activity was not significantly different between males and females. Since adjusted 24EE was 106 +/- 39 kcal/d (P less than 0.01) higher in females during the luteal phase of the menstrual cycle compared with females during the follicular phase, energy expenditure was analyzed in a subset (greater than 50 yr) to minimize the confounding effect of menstrual status. 24EE (160 +/- 66 kcal/d; P less than 0.03), BMR (116 +/- 45; P less than 0.02), and SMR (208 +/- 68 kcal/d; P less than 0.005) were higher in males compared with females of the older subset after adjusting for differences in body composition, age, and activity. In summary, sedentary 24EE is approximately 5-10% lower in females compared with males after adjusting for differences in body composition, age, and activity.

Lower sedentary metabolic rate in women compared with men.

Reduction in energy intake below the acceptable level of requirement for an individual results in a series of physiological and behavioural responses, which are considered as an adaptation to the low energy intake. This ability of the human body to adapt to a lowering of the energy intake is without doubt beneficial to the survival of the individual. However, what is more controversial is the view held by some that the body can metabolically adapt in a beneficial manner to a lowered intake and consequently that the requirements for energy are variable given the same body size and composition and physical activity levels. Much of this confusion is the result of considerable evidence from studies conducted in well-nourished adults who, for experimental or other reasons, have lowered their intakes and consequently demonstrated an apparently enhanced metabolic efficiency resulting from changes in metabolic rates which are disproportionate to the changes in body weight. Similar increases in metabolic efficiency are not readily seen in individuals who on long-term marginal intakes, probably from childhood, have developed into short-statured, low-body-weight adults with a different body composition. It would thus appear that the generally used indicator of metabolic efficiency in humans, that is a reduced oxygen consumption per unit fat free mass, is fraught with problems since it does not account for variations in contributions from sub-compartments of the fat free mass which include those with high metabolism at rest such as brain and viscera and those with low metabolism at rest such as muscle mass. Metabolic rate per unit fat free mass thus, does not reflect true variations in metabolic efficiency and is due largely to variations in body composition. This finding combined with the evidence that behavioural adaptation in habitual physical activity patterns which occurs on energy restriction is not necessarily beneficial to the individual raises doubts about the role of adaptation to low intakes in determining one’s requirement for energy. The evidence is overwhelming that both in children and adults, changes in body size and composition as well as in levels of habitual physical activity may be the most important consequences of a lowered energy intake and cannot be assumed to be a part of a beneficial adaptation that influences energy requirements.

Twenty-four-hour energy expenditure and basal metabolic rate measured in a whole-body indirect calorimeter in Gambian men

A transportable, whole body indirect calorimeter, designed for use in the tropics, is described. The calorimeter was built to study energy expenditure of people having chronically or acutely low levels of food intake, and it will help to determine energy adaptations made by individuals with restricted food intake. The calorimeter comprises two units: a 27 m3 ventilated chamber connected to an office housing control and monitoring equipment. The system also allows the experimenter to assess the rate of energy expenditure by means of a ventilated hood or a baby respiration chamber. The incoming air flow rate is variable and is typically set at approximately 30 l/min. Carbon dioxide production (VCO2) and oxygen consumption (VO2) are continuously monitored by means of differential gas analysers via a computerized data acquisition unit. Gas production/consumption rates are measured with a delay of 80 s, the complete response to step changes in VCO2 or VO2 consumption being calculated over 15 min using the rate of change terms in the gas exchange equations. The total electrical power required for the whole system is 12 kW. The calorimeter has been functioning for nearly 4 years in a rural village of The Gambia during which ambient temperatures have ranged from 16 to 44 degrees C and dewpoints from -8 to 24 degrees C. The performance and accuracy of the calorimeter were tested using 20 per cent CO2 in N2 infusion and butane burning. Agreement between the theoretical and the measured values was found to be 99 per cent for VO2 and 100 per cent for VCO2 with a precision for both gases of +/- 10 ml/min over a 1-h period.
