Early surgery versus initial conservative treatment in patients with spontaneous supratentorial lobar intracerebral haematomas (STICH II): a randomised trial

Raised intracranial pressure (ICP) and low cerebral blood flow (CBF) are associated with ischaemia and poor outcome after brain injury. Therefore, many management protocols target these parameters. This overview summarizes the technical aspects of ICP and CBF monitoring, and their role in the clinical management of brain-injured patients. Furthermore, some applications of these methods in current research are highlighted. ICP is typically measured using probes that are inserted into one of the lateral ventricles or the brain parenchyma. Therapeutic measures used to control ICP have relevant side-effects and continuous monitoring is essential to guide such therapies. ICP is also required to calculate cerebral perfusion pressure which is one of the most important therapeutic targets in brain-injured patients. Several bedside CBF monitoring devices are available. However, most do not measure CBF but rather a parameter that is thought to be proportional to CBF. Frequently used methods include transcranial Doppler which measures blood flow velocity and may be helpful for the diagnosis and monitoring of cerebral vasospasm after subarachnoid haemorrhage or jugular bulb oximetry which gives information on adequacy of CBF in relation to the metabolic demand of the brain. However, there is no clear evidence that incorporating data from CBF monitors into our management strategies improves outcome in brain-injured patients.

Monitoring the injured brain: ICP and CBF

Monitoring of intracranial pressure (ICP) has been used for decades in the fields of neurosurgery and neurology. There are multiple techniques: invasive as well as noninvasive. This paper aims to provide an overview of the advantages and disadvantages of the most common and well-known methods as well as assess whether noninvasive techniques (transcranial Doppler, tympanic membrane displacement, optic nerve sheath diameter, CT scan/MRI and fundoscopy) can be used as reliable alternatives to the invasive techniques (ventriculostomy and microtransducers). Ventriculostomy is considered the gold standard in terms of accurate measurement of pressure, although microtransducers generally are just as accurate. Both invasive techniques are associated with a minor risk of complications such as hemorrhage and infection. Furthermore, zero drift is a problem with selected microtransducers. The non-invasive techniques are without the invasive methods' risk of complication, but fail to measure ICP accurately enough to be used as routine alternatives to invasive measurement. We conclude that invasive measurement is currently the only option for accurate measurement of ICP.

Intracranial Pressure Monitoring: Invasive versus Non-Invasive Methods-A Review

Background and Purpose— Treatment of intracerebral hematoma (ICH) is controversial. An advantage of neurosurgical intervention over conservative treatment of ICH has not been established. Recent reports suggest a favorable effect of stereotactic blood clot removal after liquefaction by means of a plasminogen activator. The SICHPA trial was aimed at investigating the efficacy of this treatment. Methods— A stereotactically placed catheter was used to instill urokinase to liquefy and drain the ICH in 6-hour intervals over 48 hours. From 1996 to 1999, 13 centers entered 71 patients into the study. Patients were randomized into a surgical group (n=36) and a nonsurgical group (n=35). Admission criteria were the following: age >45 years, spontaneous supratentorial ICH, Glasgow Eye Motor score ranging from 2 to 10, ICH volume >10 cm 3 , and treatment within 72 hours. The primary end point was death at 6 months. As secondary end points, ICH volume reduction and overall outcome measured by the modified Rankin scale were chosen. The trial was prematurely stopped as a result of slow patient accrual. Results— Seventy patients were analyzed. Overall mortality at day 180 after stroke was 57%; this included 20 of 36 patients (56%) in the surgical group and 20 of 34 patients (59%) in the nonsurgical group. A significant ICH volume reduction was achieved by the intervention (10% to 20%, P P =0.08). The odds ratio of mortality combined with modified Rankin scale score 5 at 180 days was also not statistically significant (odds ratio, 0.52; 95% confidence interval, 1.2 to 2.3; P =0.38). Conclusions— Stereotactic aspiration can be performed safely and in a relatively uniform manner; it leads to a modest reduction of 18 mL of hematoma reduction over 7 days when compared with control, which has a 7-mL reduction, and therefore may improve prognosis.

Ultrasound non-invasive measurement of intracranial pressure in neurointensive care: A prospective observational study

A non-invasive method of assessing intracranial pressure (ICP) would be of benefit to patients with abnormal cerebral pathology that could give rise to changes in ICP. In particular, it would assist the regular monitoring of hydrocephalus patients. This study evaluated a technique using tympanic membrane displacement (TMD) measurements, which has been reported to provide a reliable, non-invasive measure of ICP. A group of 135 hydrocephalus patients was studied, as well as 13 patients with benign intracranial hypertension and a control group of 77 volunteers. TMD measurements were carried out using the Marchbanks measurement system analyser and compared between the groups. In 36 patients, invasive measurements of ICP carried out at the same time were compared with the TMD values. A highly significant relationship was found between TMD and ICP but intersubject variability was high and the predictive value of the technique low. Taking the normal range of ICP to be 10–15 mmHg, the predictive limits of the regression are an order of magnitude wider than this and therefore Vm cannot be used as a surrogate for ICP. In conclusion, TMD measurements do not provide a reliable non-invasive measure of ICP in patients with shunted hydrocephalus.

Clinical comparison of tympanic membrane displacement with invasive intracranial pressure measurements.

Lack of an effective treatment for spontaneous intracerebral haemorrhage (ICH) is partly because the mechanism of neuronal damage in ICH is not fully understood. Animal experiments have shown that there is a zone of ischaemia and oedema around the haematoma which can be reduced by early evacuation of the mass lesion. We set out to study Cerebral Blood Flow (CBF) changes in patients with ICH. We present data on 13 patients (mean age 60). SPECT scans were performed within 48 hours of ictus and 4–7 days later. Four patients had surgical evacuation of the clot; 9 were managed conservatively. The ratio of uptake of the isotope in the cerebral hemisphere containing the haematoma to the isotope uptake in the contra-lateral (un-affected) cerebral hemisphere was taken as an index of perfusion of the affected cerebral hemisphere. The perfusion index of the affected hemisphere improved between the first and the second scans in all the surgically treated patients; in the conservatively managed group, it was worse in 6 patients, the same in 1 and very slightly better in 2. There was an overall mean improvement of 3.87% in the surgical group, and an overall mean deterioration of 3.61% in the medical group. This data suggests that surgical evacuation of the clot may improve perfusion in the ipsilateral cerebral hemisphere in ICH. It underlines the importance of a prospective randomised trial to assess the value of surgery in patients with ICH. The Surgical Trial in Intracerebral Haemorrhage (STICH) is currently underway worldwide. We also describe the application of Difference Based Region Growing (DBRG) to SPECT image analysis. This method overcomes the difficulties posed by 1) the presence of a mass lesion and 2) surgical evacuation of haematoma.

Changes in Cerebral Blood Flow as Measured by HMPAO SPECT in Patients Following Spontaneous Intracerebral Haemorrhage

The aim of this study was to determine the in vivo accuracy and reliability of intracranial pressure (ICP) measurement using the Codman MicroSensor by comparison with the Camino ICP transducer and associated clinical and radiological information. Paired ICP readings were recorded every minute in 17 patients. A total of 56 790 validated paired measurements were obtained over a wide range of ICP values (-16 mm Hg to 114 mm Hg). Recording periods ranged from 3 hours to 6 days (median 41 hours). In 11 patients the MicroSensor and Camino readings were in good agreement. Paired readings were within 10 mm Hg for 99% of the recording time and both readings were compatible with clinical intracranial events at all times (in one case it was not possible to verify the clinical information). In six patients large differences occurred between transducer readings (>10 mm Hg apart for 41% of the recording period). In one case, either reading could have been compatible with intracranial clinical events. In two cases, although both readings were comparable, Camino readings were more consistent with clinical and radiological findings. In three cases, the MicroSensor readings were inconsistent with the clinical condition of the patients whereas the Camino readings were compatible. These results suggest that, during routine clinical use in our department, the MicroSensor provided misleading information in 18% of our patients and thus is not sufficiently reliable for routine use in the detection of adverse clinical events.

Intracranial pressure and clinical status: assessment of two intracranial pressure transducers

The International Surgical Trial in Intracerebral haemorrhage has been set up to determine the role of surgery in spontaneous supratentorial intracerebral haemorrhage. This is an interim report as the results will remain blinded until all patients have been recruited and followed up.
