Given the growing awareness of and concern for potential carcinogenic effects of exposure of children to ionizing radiation at CT, optimizing acquisition parameters is crucial to achieve diagnostically acceptable image quality at the lowest possible radiation dose. Among currently available dose reduction techniques, recent technical innovations have allowed the implementation of low tube voltage scans and iterative reconstruction (IR) techniques into daily clinical practice for pediatric CT. The benefits of lowering tube voltage include a considerable reduction in radiation dose and improved contrast on images, especially when an iodinated contrast medium is used. The increase in noise, which is attributed to decreased photon penetration, is a major drawback but is not as severe as that at adult CT because of the small body size of children. In addition, use of IR algorithms can suppress increased noise, yielding wider applicability for low tube voltage scans. However, a careful implementation strategy and methodologic approach are necessary to maximize the potential for dose reduction while preserving diagnostic image quality under each clinical condition. The potential pitfalls of and topics related to these techniques include (a) the effect of tube voltage on the surface radiation dose, (b) the effect of window settings, (c) accentuation of metallic artifacts, (d) deterioration of low contrast detectability at low-dose settings, (e) interscanner variation of x-ray spectra, and (f) a comparison with the use of a spectral shaping technique. Appropriate use of low tube voltage and IR techniques is helpful for radiation dose reduction in most applications of pediatric CT. Clinical Implementation of Dual-Energy CT for Gastrointestinal Imaging

Background Electronic cigarette or vaping product use-associated lung injury (EVALI) is a serious public health concern with substantial morbidity and mortality, particularly in young individuals. Purpose To evaluate chest radiographic and chest CT findings of EVALI in the pediatric population. Materials and Methods This was a retrospective study of children who presented to a tertiary pediatric hospital from December 2018 to December 2019. Patients fulfilled the Centers for Disease Control and Prevention criteria for EVALI and had chest radiographs and CT images available at initial presentation. Two pediatric radiologists independently reviewed imaging for pattern, distribution, and extent of pulmonary abnormalities, as well as for extrapulmonary abnormalities. Clinical information, management, and outcomes were reviewed. Interobserver agreement was measured with Cohen κ coefficient. Results Seven male patients (50%) and seven female patients (50%) (mean age, 16 years; range, 13-18 years) were evaluated. All patients underwent chest radiography and CT within 4 days of presentation (range, 0-4 days). Chest radiographic findings included ground-glass opacity in 14 of 14 (100%) and consolidation in eight of 14 (57%). CT findings included ground-glass opacity in 14 of 14 (100%), consolidation in nine of 14 (64%), and interlobular septal thickening in two of 14 (14%). At CT, subpleural sparing was seen in 11 of 14 (79%) and a reversed halo sign was seen in five of 14 (36%). Chest radiographic and CT abnormalities were predominately bilateral in 14 of 14 (100%) and symmetric in 13 of 14 (93%), with lower lobe predominance in seven of 14 (50%). Extent of abnormality was predominately diffuse at both chest radiography and CT. There was almost perfect interobserver agreement between two reviewers for detecting abnormalities on chest radiographs (κ = 0.99; 95% confidence interval: 0.97, 1.00) and CT (κ = 0.99; 95% confidence interval: 0.98, 1.00). Conclusion In pediatric patients, electronic cigarette or vaping product use-associated lung injury is characterized by bilateral symmetric ground-glass opacities, consolidation, and a lower lobe predominance at CT. © RSNA, 2020.

Pediatric Chest Radiographic and CT Findings of Electronic Cigarette or Vaping Product Use-associated Lung Injury (EVALI).

To provide an overview on dose reduction and image quality after the installation of a third-generation dual-source CT (dsCT) in a Pediatric Radiology Department. We included pediatric patients (  10 years old) including patients scanned in the same period of two consecutive years, in 2017 with a 16-row LightSpeed CT (GE Healthcare) or in 2018 with a Somatom Force dsCT (Siemens Healthineers). Technical parameters such as kVp, mAs, slice thickness, exposure times and dose indicators were retrieved and compared. Image quality was evaluated in consensus by two radiologists on a five-point semiquantitative scale. Nonparametric tests were used. In oncological patients, significantly lower kVp and tube current with better image quality were achieved with the dsCT. Radiation dose (total DLP) was 5–6 times lower with dsCT, thanks also to virtual non-contrast images. In low-dose chest CT, the frequent use of tin filter required higher tube current; a total DLP 3 times lower was achieved with dsCT in patients ≤ 10 years old. The image quality was better with the dsCT in low-dose chest CT protocols. The third-generation dsCT provides high-quality images with reduced motion artifacts at lower dose.

Trends in radiation dose and image quality for pediatric patients with a multidetector CT and a third-generation dual-source dual-energy CT.

Optimizing the CT acquisition parameters to obtain diagnostic image quality at the lowest possible radiation dose is crucial in the radiosensitive pediatric population. The image quality of low-dose CT can be severely degraded by increased image noise with filtered back projection (FBP) reconstruction. Iterative reconstruction (IR) techniques partially resolve the trade-off relationship between noise and radiation dose but still suffer from degraded noise texture and low-contrast detectability at considerably low-dose settings. Furthermore, sophisticated model-based IR usually requires a long reconstruction time, which restricts its clinical usability. With recent advances in artificial intelligence technology, deep learning-based reconstruction (DLR) has been introduced to overcome the limitations of the FBP and IR approaches and is currently available clinically. DLR incorporates convolutional neural networks-which comprise multiple layers of mathematical equations-into the image reconstruction process to reduce image noise, improve spatial resolution, and preserve preferable noise texture in the CT images. For DLR development, numerous network parameters are iteratively optimized through an extensive learning process to discriminate true attenuation from noise by using low-dose training and high-dose teaching image data. After rigorous validations of network generalizability, the DLR engine can be used to generate high-quality images from low-dose projection data in a short reconstruction time in a clinical environment. Application of the DLR technique allows substantial dose reduction in pediatric CT performed for various clinical indications while preserving the diagnostic image quality.
