I first read Davidson 30 years ago: at that time it was already in its 5th edition. A brief comparison shows that it then contained about 440,000 words in 1,100 pages: the current edition has compressed 625,000 words into 800 pages. I think I bought it as a student because it was cheap: but also because it seemed to be comprehensive and straightforward, and I have used it as a basis for my medical knowledge ever since. So I miss some of the old pictures of the acute skin rashes such as scarlet fever: in fact infectious disease has been transferred to the back and genetic factors take the first chapters. But careful linguistic comparison will still uncover the old phrases which some of us know by heart-there are minor changes such as 'alarming reactions to intravenous iron are uncommon, but have occasionally been noted', which becomes 'alarming systemic anaphyllactic reactions can occur'. Sir Stanley Davidson made 'no attempt to describe every rare disease or syndrome, but devoted most of the space available to those disorders most commonly encountered in practice'. I have grown up with successive editions, and have gradually come to appreciate the problems of the authors in the compression of knowledge. Having got to know many of them personally as real people rather than as names I can still recommend the book. It is the essential starting point for the study of internal medicine and for many doctors will remain their base reference work. The present editors have kept up-to-date and been prepared to prune the dead wood. There are many competitors in the market, and the publishers must take care with layout and illustration, although Davidson is still the best value for money. I will continue to recommend it to my clinical students: they will need to read it and know it to pass final MB. Postgraduates will need to remember the facts, but also to be able to place them in a broader perspective. The older consultant will still happily read it, and to get to know the authors themselves is really to complete your medical education. Dr John Macleod and his team have successfully kept alive the primary objective 'to provide a rational and easily comprehensible basis for the practice of medicine'. The book is economical in price and compact in size, but still contains the essential truths for the practice of good medicine.

Recent studies of the volatile compounds in tea

Typhoid fever is a major cause of death worldwide with a major part of the disease burden in developing regions such as the Indian sub-continent. Bangladesh is part of this highly endemic region, yet little is known about the spatial and temporal distribution of the disease at a regional scale. This research used a Geographic Information System to explore, spatially and temporally, the prevalence of typhoid in Dhaka Metropolitan Area (DMA) of Bangladesh over the period 2005-9. This paper provides the first study of the spatio-temporal epidemiology of typhoid for this region. The aims of the study were: (i) to analyse the epidemiology of cases from 2005 to 2009; (ii) to identify spatial patterns of infection based on two spatial hypotheses; and (iii) to determine the hydro-climatological factors associated with typhoid prevalence. Case occurrences data were collected from 11 major hospitals in DMA, geocoded to census tract level, and used in a spatio-temporal analysis with a range of demographic, environmental and meteorological variables. Analyses revealed distinct seasonality as well as age and gender differences, with males and very young children being disproportionately infected. The male-female ratio of typhoid cases was found to be 1.36, and the median age of the cases was 14 years. Typhoid incidence was higher in male population than female (χ(2) = 5.88, p 0.05). A statistically significant inverse association was found between typhoid incidence and distance to major waterbodies. Spatial pattern analysis showed that there was a significant clustering of typhoid distribution in the study area. Moran's I was highest (0.879; p<0.01) in 2008 and lowest (0.075; p<0.05) in 2009. Incidence rates were found to form three large, multi-centred, spatial clusters with no significant difference between urban and rural rates. Temporally, typhoid incidence was seen to increase with temperature, rainfall and river level at time lags ranging from three to five weeks. For example, for a 0.1 metre rise in river levels, the number of typhoid cases increased by 4.6% (95% CI: 2.4-2.8) above the threshold of 4.0 metres (95% CI: 2.4-4.3). On the other hand, with a 1 °C rise in temperature, the number of typhoid cases could increase by 14.2% (95% CI: 4.4-25.0).

Typhoid Fever and Its Association with Environmental Factors in the Dhaka Metropolitan Area of Bangladesh: A Spatial and Time-Series Approach

Tea is a perennial plantation crop grown under monoculture providing favorable conditions for a variety of pests. The concept of pest control has undergone a considerable change over the past few decades. In recent years there has been a greater dependence on the use of pesticides (7.35-16.75 kgha(-1)) with little importance laid on other safe control methods for the management of tea pests. Due to this practice, the tea pests showed a higher tolerance/ resistance status due to formation of greater amount of esterases, glutathione S-transferase and acetylcholinesterase. Thus, over reliance on pesticides end up with pesticide residue in made tea (DDT - 10.4-47.1%; endosulfan - 41.1-98.0%; dicofol- 0.0-82.4%; ethion - 0.0-36.2%; cypermethrin - 6.0- 45.1%). The growing concern about the pesticide residue in made tea, its toxicity hazards to consumers, the spiraling cost of pesticides and their application have necessitated a suitable planning which will ensure a safe, economic as well as effective pest management in tea. At present it is a global concern to minimize chemical residue in tea and European union and German law imposed stringent measures for the application of chemicals in tea and fixed MRL values at < or = 0. 1 mgkg(-1) for the most commonly used pesticides which will not be met out in the real practice and has been a major constraint to tea exporting countries like India. In order to regulate the situation of the Indian market at global level, central insecticide board and prevention of food adulteration regulation committee have reviewed the MRL position for tea and has recommended 10 insecticides, 5 acaricides, 9 herbicides and 5 fungicides for use in tea and issued the tea distribution and export control order 2005 which will help the country to limit the presence of undesirable substances in tea. This review attempts to provide the readers with a comprehensive account of pesticide use in North East in tea, surveillance report of the European community regarding the residue level in Assam and Darjeeling tea, recent amendments by international and national regulatory bodies, revised MRL values of pesticides in tea, an update about the current strategies for the management of tea pests with more focus on the use of biological control agents and a possible beneficial role or judicious use of chemical pesticides in complement with other alternative measures to achieve optimum effects in terms of limiting agricultural input, lowering production costs, reducing environmental contamination and the effect on non-target organisms, delaying the development of resistant pest biotypes and above all minimizing the pesticide residues in tea to increase the exports.

Pesticide usage pattern in tea ecosystem, their retrospects and alternative measures.

Volatile compounds are important components of tea aroma, a key attribute of sensory quality. The present review examines the formation of aromatic volatiles of various kinds of teas and factors influencing the formation of tea volatiles, including tea cultivar, growing environment and agronomic practices, processing method and storage of tea. The determination of tea volatiles and the relationship of active-aroma volatiles with the sensory qualities of tea are also discussed in the present paper.

Recent Advances in Volatiles of Teas

Darjeeling teas are the highest grown teas in the world and preferred for its flavour, aroma and quality. Apart from the genetic makeup of the plant, earlier reports suggest that insect infestation, particularly jassids and thrips triggers the aroma and flavour formation in Darjeeling tea. The present work encompasses the identification of the genes/transcriptomes responsible for the typical flavour of Darjeeling tea, besides understanding the role of jassids and thrips in particular, in producing the best cup character and quality. The quantitative real time PCR analysis was based on a suppression subtractive hybridisation forward library of B157 (tea clone infested with thrips), providing us transcripts related to aroma and flavour formation. We observed the expression of genes like leucine zipper, ntd, nced, geraniolsynthase, raffinose synthase, trehalose synthase, amylase, farnesyl transferase, catalase, methyl transferase, linalool synthase, peroxidases, elicitor responsive proteins, linamarase, nerolidol linalool synthase 2, 12-oxophytodienoate reductase, glucosidase, MYB transcription factor, and alcohol dehydrogenase, highly regulated due to insect infestation, manufacturing stresses and mechanical injury. The first report on gene expression dynamics in thrips infested Darjeeling tea leaves can be extrapolated with increase in volatiles which is responsible for enhancing the quality of Darjeeling tea, specially the flavour and aroma of the infusion. We hope to model these responses in order to understand the molecular changes that occur during Darjeeling tea flavour formation.

Understanding Darjeeling tea flavour on a molecular basis

Context: In Bangladesh, typhoid fever is a round the year problem which sometimes take epidemic proportions. The reason behind such occurrences are unsafe water supply, defective sewage system and unhygienic food handling practice. This study was designed to find out the prevalence of typhoid fever and related factors like sanitation facilities and food practice among the children of low income group people living in Kamrangir char and its adjacent areas in the semi urban Dhaka. Methodology: Data were collected from June 2009 to May 2010, from a sample of 96 patients with fever of more than seven days duration, who presented in the paediatric outpatient department of Dhaka Medical College Hospital, Dhaka. Detailed history, comprehensive physical examination and other relevant informations of the patients were recorded by following standard procedures. A raised anti O level with titre of >1:160 as well as other blood parameters were considered significant of having typhoid fever. Results: This study shows that 84(87.50%) patients had typhoid fever based on clinical examination and serological test results (widal test). It was observed that prevalence of enteric fever was high among the patients of school going age group (66.67%), habituated with unsafe drinking water (58.33%) and junk foods (72.92%). Statistical analysis showed that SSC level education in the parents who remain outside during the major part of the day for their livelihood had significant positive relationship with prevalence of typhoid fever. It was concluded that prevalence of typhoid fever was high among the children of school going age, who consume unsafe drinking water and food from outside source.
