Systematic review of the chemical composition of contemporary dental adhesives

Hygroscopic and hydrolytic effects in dental polymer networks.

The present review outlines the history of monomers used in resin composites, motivates further development, and highlights recent and ongoing research reported in the field of dental monomer systems. The monomer systems of most present-day resin composites are based on BisGMA, developed some 40 years ago, or derivatives of BisGMA. In the remaining resin composites, urethane monomers or oligomers are used as the basis of the monomer system. The main deficiencies of current resin composites are polymerization shrinkage and insufficient wear resistance under high masticatory forces. Both factors are highly influenced by the monomer system, and considerable efforts are being made around the world to reduce or eliminate these undesirable properties. The use of fluoride-releasing monomer systems, some of which are under investigation, has been suggested to mitigate the negative effects of marginal gaps formed in consequence of polymerization shrinkage. The very crux of the problem has also been approached with the synthesis of potentially low-shrinking/non-shrinking resin composites involving ring opening or cyclopolymerizable monomers. By the use of additives with a supposed chain transfer agent function, monomer systems have been formulated that improve the degree of conversion of methacrylate double bonds and mechanical properties. Many promising monomer systems have been devised, the implementation of which may be expected to improve the longevity of resin composite fillings and expand the indications for resin composites.

Resin composites in dentistry: the monomer systems

Statement of problem Developments in ceramic core materials such as lithium disilicate, aluminum oxide, and zirconium oxide have allowed more widespread application of all-ceramic restorations over the past 10 years. With a plethora of ceramic materials and systems currently available for use, an overview of the scientific literature on the efficacy of this treatment therapy is indicated. Purpose This article reviews the current literature covering all-ceramic materials and systems, with respect to survival, material properties, marginal and internal fit, cementation and bonding, and color and esthetics, and provides clinical recommendations for their use. Material and methods A comprehensive review of the literature was completed seeking evidence for the treatment of teeth with all-ceramic restorations. A search of English language peer-reviewed literature was undertaken using MEDLINE and PubMed with a focus on evidence-based research articles published between 1996 and 2006. A hand search of relevant dental journals was also completed. Randomized controlled trials, nonrandomized controlled studies, longitudinal experimental clinical studies, longitudinal prospective studies, and longitudinal retrospective studies were reviewed. The last search was conducted on June 12, 2007. Data supporting the clinical application of all-ceramic materials and systems was sought. Results The literature demonstrates that multiple all-ceramic materials and systems are currently available for clinical use, and there is not a single universal material or system for all clinical situations. The successful application is dependent upon the clinician to match the materials, manufacturing techniques, and cementation or bonding procedures, with the individual clinical situation. Conclusions Within the scope of this systematic review, there is no evidence to support the universal application of a single ceramic material and system for all clinical situations. Additional longitudinal clinical studies are required to advance the development of ceramic materials and systems.

Statement of problem Endodontically-treated, dowel-restored teeth may experience fracture, but investigations of variables related to fracture are often inconclusive and occasionally contradictory. Purpose The finite element method was used to analyze the stresses in dowel-restored teeth. The variables studied were material, shape, bonding, modulus of elasticity, diameter, and length of the dowel. Material and methods The model of the dowel-restored tooth involved dentin, ligament, cortical and trabecular bone, gingiva, and gutta-percha. The dowels were made of glass fiber, titanium, or zirconia and modeled as an approximation of the brands ParaPost Fiber White, ParaPost XH, and Cerapost, respectively. The dowel was cemented with zinc-phosphate cement or with bonded or nonbonded resin luting agents, and an approximation of the material properties of these 2 materials were used in the modeling. The restoration included a composite resin core and a gold crown. Other variables included taper versus parallel-sided posts, modulus of elasticity, diameter, and length of post. The model was axisymmetrical in 3 dimensions. A load of 100 N was applied to the crown at an angle of 45 degrees, and tensile, shear, and von Mises stresses were calculated. Results The generated stresses decreased with respect to the dowel material in the following order: glass fiber, titanium, and zirconia. Stresses were in general higher with tapered than with parallel-sided dowels. Stresses were reduced by bonding and with an increasing modulus of elasticity, increasing diameter, and increasing length of the dowel. Conclusions Within the limitations of this study, it was found that all investigated dowel-related factors influenced the stress field generated in dowel-restored teeth.
