Understanding the Barriers and Opportunities for Effective Management of Shared Sanitation in Low-Income Settlements—The Case of Kumasi, Ghana

Open defecation remains a major environmental sanitation challenge facing all areas of Ghana. This notwithstanding, the socioeconomic drivers of this phenomenon are overlooked. This study, therefore, analysed the factors that influence the choice of toilet facilities over the practice of open defecation in the country. Ghana Living Standard Survey round 7 (GLSS7) data were analysed using multinomial logit regression. From the data, a majority of households used improved toilet facilities (WC, KVIP, and pit latrines with slab) in Ghana and over one-fourth of households engaged in open defecation. The regression result revealed that the choice of toilet facilities over the practice of open defecation was significantly influenced by the sex of the household head, age, household size, education, marital status, locating in urban areas, regional locations, ownership of dwelling, type of dwelling, expenditure on rent, expenditure quintile, and per capita consumption expenditure of the household. Specifically, male, younger, less educated, and first income-quintile household heads have higher probability of practicing open defecation in Ghana. These variables point to specific policy directions that should be corrected or targeted to minimize, if not eliminate, the practice of open defecation in the country. The Media Coalition Campaign against Open Defecation should be intensified and directed towards the males, youths, and the less educated populace. This study also justified that calls for Ghanaians to change their attitudes or behavior towards open defecation are mere rhetoric if such calls are not defined within the socioeconomic conditions of the people of the area.

Explaining the Effects of Socioeconomic and Housing Characteristics on the Choice of Toilet Facilities among Ghanaian Households.

Children in developing countries have continued to suffer morbidity and mortality arising from a few illnesses. This study was designed to examine the within and between spatial variations in childhood morbidity from cough, fever, and diarrhea among six West African countries in a manner that transcends geographical boundaries. Data from six countries including their geographical boundaries were obtained from Demographic and Health Surveys. The spatial modelling was through Bayesian models and appropriate prior distributions were assigned to the different parameters of the model. Parameter estimation was through integrated nested Laplace approximation. Results show similar significant spatial distributions for the three illnesses, and they demonstrate that children in Benin Republic and Mali are less likely to suffer from these illnesses, whereas higher likelihood were obtained in the case of Cote d'Ivoire, Burkina Faso, Togo, and some parts of Ghana. The nonlinear effects of child's age show that the risks of contracting the illnesses peak among children aged 10-14 months while, as the mothers advance in age, their children have reduced risks. Breastfeeding and a woman's working status and education are among the significant factors that either aggravate or prevent these illnesses in the West African countries. The results pinpointed regions of the West African countries with high and low risks of the illnesses, and this would enhance intervention strategies of policy makers and international donors in the subregion.

Investigating the Spatial Patterns of Common Childhood Morbidity in Six Neighboring West African Countries.

Decision-makers in developing communities often lack credible data to inform decisions related to water, sanitation, and hygiene. Quantitative microbial risk assessment (QMRA), which quantifies pathogen-related health risks across exposure routes, can be informative; however, the utility of QMRA for decision-making is often undermined by data gaps. This work integrates QMRA, uncertainty and sensitivity analyses, and household surveys in Bwaise, Kampala (Uganda) to characterize the implications of censored data management, identify sources of uncertainty, and incorporate risk perceptions to improve the suitability of QMRA for informal settlements or similar settings. In Bwaise, drinking water, hand rinse, and soil samples were collected from 45 households and supplemented with data from 844 surveys. Quantified pathogen (adenovirus, Campylobacter jejuni, and Shigella spp./EIEC) concentrations were used with QMRA to model infection risks from exposure through drinking water, hand-to-mouth contact, and soil ingestion. Health risks were most sensitive to pathogen data, hand-to-mouth contact frequency, and dose-response models (particularly C. jejuni). When managing censored data, results from upper limits of detection, half of limits of detection, and uniform distributions returned similar results, which deviated from lower limits of detection and maximum likelihood estimation imputation approaches. Finally, risk perceptions (e.g., it is unsafe to drink directly from a water source) were identified to inform risk management.

Navigating Data Uncertainty and Modeling Assumptions in Quantitative Microbial Risk Assessment in an Informal Settlement in Kampala, Uganda.

\n This paper presents an overview of Ghana's sanitation situation post-Millennium Development Goals (MDGs). It specifically examines why the MDG target on sanitation was missed, Ghana's preparedness towards achieving the Sustainable Development Goals (SDGs) sanitation target, potential barriers and opportunities for the sanitation sector. An eight step policy analysis framework guided the review. The findings indicate that the low sanitation has largely been driven by rather complex economics, institutional and sociocultural factors including inadequate financial commitment, poor implementation of policies, bad attitude and perception towards sanitation and extensive use of shared and public toilets. Nevertheless, it was found that recent reforms and programmes in the sanitation sector such as the creation of new Sanitation and Water Resource Ministry, National Sanitation Authority (NSA), introduction of sanitation surcharges included in property tax, and programmes such as the Greater Accra Municipal Assembly sanitation and water project put Ghana in a better position to rapidly increase its coverage. However, efforts would have to be accelerated by forging partnerships with the private sector to provide efficient and low-cost technologies, financial schemes, human and technical resources for improved service delivery.

Ghana's post-MDGs sanitation situation: an overview

Background: Non-communicable diseases (NCDs) are a major cause of morbidity and mortality globally. In low-income settings, some NCDs are without symptoms so regular screening for early detection is key. However, routine screening for NCDs is limited in the general public and even among the elite. We therefore set out to assess health screening practices among lecturers in a university in Ghana. Methods: This was a cross-sectional study involving 205 lecturers in Kwame Nkrumah University of Science and Technology from February to August 2022. A questionnaire was used to gather data from both male and female university lecturers based on their self-reported declaration of being male or female. Data were analyzed using descriptive and inferential statistics. Results: We found that, 41 (20.0%) lecturers (both men and women) had never checked their blood pressure (BP), 140 (68.3%) check their BP twice a month and 24 (11.7%) do so more than 3 times a month. Overall, 105 (57.18%) lecturers have high BP (>120 mmHg, >80 mmHg). Among the lecturers with hypertension, 59 (50.9%) often checked their BP each month, whereas 22 (18.97%) did not. The study found that, 164 (80%) of the lecturers have never checked their blood sugar level since they assumed lectureship position. Among the lecturers who check their blood sugar, 78 (47.55) are not happy with their blood sugar levels. Lecturer’s age (40 to 49 years) was found to be associated with BP in the bivariate analysis (p=0.036), but not in the multivariate analysis (p=0.114). In the bivariate analyses, female lecturers were found to have a higher risk (OR 1.35; 95% CI 0.29-6.21) of developing hypertension compared to male lecturers. Conclusions: The study has revealed that lecturers, just like the general population have moderate health care checks. The need to setup occupational health therapy units in all universities is overdue.
