TL;DR: The data imply that coma in acidotic children (CM + RD) and those with an isolated encephalopathy (CM) may result from quite different pathophysiological mechanisms and implicate hypovolaemia and renal impairment in the pathogenesis of metabolic acidosis in severe childhood malaria.
