The article contains a brief review of anal carcinoma. Biopsy and histopathological examinations are extremely important to ensure correct diagnosis, as there is a great therapeutic difference between squamous cell carcinoma and adenocarcinoma in this region. Primary treatment for squamous cell carcinoma is chemoradiotherapy, while surgery is second line treatment. This regimen gives excellent locoregional tumour control with 70% long-time survival. The most serious problem now is the the development of distant metastases. The main diagnostic procedure, both for primary tumours and local relapses, is well performed digital anal palpitation. A Norwegian standard protocol for anal carcinoma has been prepared to ensure good quality diagnostic procedures, therapy and follow-up in these patients.

Anal cancer. A review

La ortotanasia esta regulada en Brasil a traves de la Resolucion 1805/2006. Esta investigacion tiene como objetivo reflexionar y debatir sobre las medidas de limitacion o retirada del soporte vital en situaciones terminales. Participaron diez medicos oncologos de servicios publicos de salud del Distrito Federal, Brasil. Un guia de entrevista semi-estructurada oriento la investigacion. Los datos se analizaron mediante el analisis del contenido. Casi todos los participantes tenian una concepcion de la ortotanasia y la eutanasia en consonancia con la literatura. Siete medicos conocian la resolucion del CFM y tres mencionaron la debilidad legal de una reglamentacion proveniente de una entidad de clase. Ocho medicos dijeron que esta reglamentacion no afectara en gran medida la practica diaria. Principios de la bioetica pueden contribuir al esclarecimiento de los dilemas sobre la retirada o la limitacion del soporte vital.

Ortotanasia: una decisión frente a la terminación

Advance directives (ADs) are understood as the act of deciding what care the patient wants to receive in the period before death. Preserving the patient’s autonomy by choosing his care guarantees human dignity during the process of dying. In Brazil, life expectancy and supportive technologies have increased, leading to growth of the number of terminally ill patients. However, there is still no legislation regulating ADs causing legal uncertainty in health professionals. Nursing professionals have the support of the Federal Nursing Council to respect the ADs, but, because it is an issue little explored, nursing professionals do not feel safe in the use of ADs, and changes in the curricula of the undergraduate courses in nursing are extremely needed, ensuring that patients have their wishes met during the dying process. Thus, this chapter deals with bioethical and legal issues involving ADs and nursing in the Brazilian context, proposing to deepen reflection and criticism on the issue and subsidies for decision-making.

Terminality Advance Directives and Nursing Practice in Brazil: Bioethical Issues

Introducao: O avanco da medicina e a sobrevivencia de pacientes com doencas graves consideradas anteriormente irrecuperaveis resultou no prolongamento do processo de morrer do paciente e, consequentemente no sofrimento adicional para o paciente e seus familiares. Nesse contexto emergiu o Testamento Vital (TV). O TV e um documento redigido por uma pessoa no pleno gozo de suas faculdades mentais, com o objetivo de dispor acerca dos cuidados, tratamento e procedimentos que deseja ou nao ser submetida quando estiver com uma doenca ameacadora de vida, fora de possibilidades terapeuticas e impossibilitado de manifestar livremente sua vontade. Objetivo : Verificar o conhecimento dos discentes de um Curso de Graduacao em Enfermagem acerca do TV. Material e Metodo: Tratou-se de um estudo transversal, descritivo, de natureza quantitativa. Os sujeitos da pesquisa foram discentes do dos 7o e 8o semestres do Curso de Graduacao de Enfermagem. Foi aplicado um formulario com questoes sobre o perfil sociodemografico e relativas ao TV. Resultados: 8 (25%) discentes responderam adequadamente a definicao do TV, 14 (43,75%) responderam de forma parcialmente adequada, e 8 (25%) responderam nao ter conhecimento; com relacao a vontade expressa no TV 31 (97%) respeitariam. 18 (56%) discentes afirmaram nao ter discutido sobre o tema TV durante a graduacao e 9 (28%) discentes afirmaram ter conhecimento sobre a Resolucao CFM No 1.995/2012; 8 (89%) que adquiriram durante a graduacao na disciplina Etica e Exercicio de Enfermagem II. Discussao : Os resultados se aproximam de um estudo realizado com discentes de medicina, onde apenas 23% entendiam plenamente a definicao do TV; 77,6% dos discentes nao tiveram a oportunidade de discutir sobre o tema durante a graduacao; e 91% desconheciam a Resolucao do CFM No 1.995/2012. Conclusao: Nesse estudo concluimos que o conhecimento dos discentes de graduacao em enfermagem acerca do TV nao e totalmente adequado, apesar de ser abordado em uma disciplina do curso. Dessa forma, acreditamos que faz-se necessario que esse conteudo seja abordado em outras disciplinas da matriz curricular do curso. In this context came the Living Will (LW). The LW is a document written by a person in the full enjoyment of their mental faculties, with the purpose of disposing about the care, treatment and procedures that they want or not to undergo when they are with a life-threatening disease, beyond therapeutic possibilities and unable to freely manifest their will. Objective: Verify the knowledge of students of a Nursing Undergraduate Course about the LW. Material and methods: A cross-sectional, descriptive, quantitative study. The research subjects were students from the 7th and 8th semesters the Nursing Undergraduate Course. A form with questions about sociodemographic profile and about the LW was applied. Results: 8 (25%) students adequately answered the definition of LW, 14 (43.75%) answered partially adequate, and 8 (25%) answered have no knowledge. 31 (97%) students answered would respect the will expressed on LW; 18 (56%) students said they did not discuss the topic of LW during the graduation. 9 (28%) said they had knowledge about CFM Resolution No 1.995/2012, 8 (89%) said they had acquired knowledge about CFM Resolution No 1.995/2012 during the graduate in the subject Nursing Ethics and Exercise II. Discussion: The results are similar to a study with medicine students, where only 23% of students fully understood the definition of LW; 77.6% of students did not have the opportunity to discuss the topic during the graduation and 91% were unaware of CFM Resolution No 1.995/ 2012. Conclusion : Concluded that the knowledge of undergraduate nursing students about LW is not entirely adequate, despite being addressed in a course discipline. Conhecimento dos discentes de um curso de enfermagem acerca do testamento vital / Nursing student's knowledge about the living will

Resumo A participação do paciente no planejamento de seus cuidados de saúde respeita seu direito à autodeterminação. Com isso, esta pesquisa teve como objetivo avaliar o posicionamento de médicos que atuam no Serviço de Emergência Hospitalar do Hospital de Clínicas de Porto Alegre em relação às diretivas antecipadas de vontade de pacientes. Trata-se de estudo transversal com 32 médicos. A maioria dos participantes (81,3%) afirmou conhecer as diretivas antecipadas de vontade, mas apenas 6,3% tinham conhecimento suficiente; 87,5% foram favoráveis à sua utilização e a vontade do paciente foi considerada determinante em três dos quatro cenários apresentados; e 84,4% consideraram necessária legislação específica além da Resolução 1.995/2012 do Conselho Federal de Medicina. Ao final do estudo foi concluído que a maioria dos médicos tinha conhecimento prévio sobre diretivas antecipadas de vontade e se posicionara a favor da utilização deste tipo de documento em emergência hospitalar.

With the intent of increase the autonomy of terminal patients the Federal Council of Medicine was created the Resolution 1.995/2012, which allows the living will. With the objective of evaluate the knowledge of medical students from the first to sixth years about concepts and medical conducts in terminally ill patients, this research was conducted in the form of a cross-sectional study, through the application of a questionnaire, which was completed by 348 medical students. From those, 251 (72,1%) correctly identified concepts about conducts in terminally ill patients, but only 23,5% claimed to know and chose correctly the meaning of the living will. Despite the low knowledge, 80, 1% stated that they intend to respect it when they encounter a terminal patient. Further research is needed, as a way of warning at the current medical curriculum and to bring to light discussions of conducts over terminally ill patients and open ways to the humanized medical practice.

Resumo Visando dar mais autonomia a pacientes terminais, foi criada pelo Conselho Federal de Medicina a Resolução 1.995/2012, que dispõe sobre o testamento vital. Para avaliar o conhecimento dos estudantes de medicina do primeiro ao sexto ano em relação a conceitos e condutas éticas diante de pacientes terminais, foi realizado este estudo transversal, mediante aplicação de questionário a 348 alunos. Destes, 251 (72,1%) identificaram corretamente questões relacionadas a condutas na terminalidade, mas apenas 23,5% demonstraram conhecer o conceito de testamento vital. Apesar do baixo índice, 80,1% afirmaram ter a intenção de respeitá-lo, no caso de pacientes terminais. Pesquisas sobre o assunto são importantes para alertar sobre lacunas nos currículos desses futuros médicos. Além disso, o caráter reflexivo desse tipo de estudo permite trabalhar questões éticas fundamentais para o atendimento humanizado.

Objetivando avaliar o conhecimento dos estudantes de medicina do primeiro ao sexto ano em relacao aos conceitos e condutas eticas diante de pacientes terminais e sobre o TV, foi realizada esta pesquisa, na forma de estudo transversal, atraves de aplicacao de questionario, o qual foi preenchido por 348 estudantes de medicina. Destes, 251 (72,1%) identificaram corretamente conceitos a respeito das condutas na terminalidade, mas apenas 23,5% demonstraram conhecer o conceito de TV. Apesar do baixo conhecimento, 80,1% afirmaram ter a intencao de respeitar o TV de um paciente terminal, quando se depararem com tal. Pesquisas sobre o assunto sao importantes para alertar sobre falhas nos curriculos destes futuros medicos, alem do carater reflexivo desse tipo de estudo, permitindo trabalhar questoes eticas fundamentais para o atendimento humanizado.
