Food, Nutrition, Physical Activity, and the Prevention of Cancer : a Global Perspective : 食物、栄養、身体活動とがんの予防 : 世界的展望(後篇)

Background: The prominence of sedentary behavior research in health science has grown rapidly. With this growth there is increasing urgency for clear, common and accepted terminology and definitions. Such standardization is difficult to achieve, especially across multi-disciplinary researchers, practitioners, and industries. The Sedentary Behavior Research Network (SBRN) undertook a Terminology Consensus Project to address this need. Method: First, a literature review was completed to identify key terms in sedentary behavior research. These key terms were then reviewed and modified by a Steering Committee formed by SBRN. Next, SBRN members were invited to contribute to this project and interested participants reviewed and provided feedback on the proposed list of terms and draft definitions through an online survey. Finally, a conceptual model and consensus definitions (including caveats and examples for all age groups and functional abilities) were finalized based on the feedback received from the 87 SBRN member participants who responded to the original invitation and survey. Results: Consensus definitions for the terms physical inactivity, stationary behavior, sedentary behavior, standing, screen time, non-screen-based sedentary time, sitting, reclining, lying, sedentary behavior pattern, as well as how the terms bouts, breaks, and interruptions should be used in this context are provided. Conclusion: It is hoped that the definitions resulting from this comprehensive, transparent, and broad-based participatory process will result in standardized terminology that is widely supported and adopted, thereby advancing future research, interventions, policies, and practices related to sedentary behaviors.

Sedentary behavior research network (SBRN) - terminology consensus project process and outcome

On behalf of Mr. Koichiro Matsuura, Director-General of UNESCO, it is my great pleasure to welcome you all to this international scientific conference. Drylands are often considered fragile ecosystems, yet they have a remarkable resilience to stress. They are home to unique and well-adapted plant and animal species that we need to conserve. Some of the world’s greatest cultures and belief systems have originated in drylands. On the other hand, desertification and land degradation in drylands often result in poverty and cause environmental refugees to abandon their homes. These problems can only be addressed in a holistic manner, based on sound scientific research and findings. Solutions to the problems of dryland degradation need to be communicated as widely as possible through education at all levels. These are many reasons why UNESCO – within its mandate of science, education, culture and communication – took the intiative to organize this conference. And we are glad that so many partners have responded to our call. UNESCO considers this conference as its main contribution to the observance of the International Year of Deserts and Desertification in 2006. We have deliberately chosen the title ‘The Future of Drylands’ as we feel it is time to redefine our priorities for science, education and governance in the drylands based on 50 years of scientific research in arid and semi-arid zones. In fact UNESCO has one of the longest traditions, within the UN system, of addressing dryland problems from an interdisciplinary, scientific point of view. In 1955, the ‘International Arid Land Meetings’ were held in Socorro, New Mexico (USA). They were organized by the American Association for the Advancement of Science (AAAS), sponsored by UNESCO and supported by the Rockefeller Foundation. One important output of the International Arid Land Meetings was a book entitled The Future of Drylands, edited by Gilbert F. White and published in

BACKGROUND\nObesity in childhood is associated with increased cardiovascular risk. It is uncertain whether this risk is attenuated in persons who are overweight or obese as children but not obese as adults.\n\n\nMETHODS\nWe analyzed data from four prospective cohort studies that measured childhood and adult body-mass index (BMI, the weight in kilograms divided by the square of the height in meters). The mean length of follow-up was 23 years. To define high adiposity status, international age-specific and sex-specific BMI cutoff points for overweight and obesity were used for children, and a BMI cutoff point of 30 was used for adults.\n\n\nRESULTS\nData were available for 6328 subjects. Subjects with consistently high adiposity status from childhood to adulthood, as compared with persons who had a normal BMI as children and were nonobese as adults, had an increased risk of type 2 diabetes (relative risk, 5.4; 95% confidence interval [CI], 3.4 to 8.5), hypertension (relative risk, 2.7; 95% CI, 2.2 to 3.3), elevated low-density lipoprotein cholesterol levels (relative risk, 1.8; 95% CI, 1.4 to 2.3), reduced high-density lipoprotein cholesterol levels (relative risk, 2.1; 95% CI, 1.8 to 2.5), elevated triglyceride levels (relative risk, 3.0; 95% CI, 2.4 to 3.8), and carotid-artery atherosclerosis (increased intima-media thickness of the carotid artery) (relative risk, 1.7; 95% CI, 1.4 to 2.2) (P ≤ 0.002 for all comparisons). Persons who were overweight or obese during childhood but were nonobese as adults had risks of the outcomes that were similar to those of persons who had a normal BMI consistently from childhood to adulthood (P>0.20 for all comparisons).\n\n\nCONCLUSIONS\nOverweight or obese children who were obese as adults had increased risks of type 2 diabetes, hypertension, dyslipidemia, and carotid-artery atherosclerosis. The risks of these outcomes among overweight or obese children who became nonobese by adulthood were similar to those among persons who were never obese. Increasing prevalence of hypertension (HTN) in children and adolescents has become a significant public health issue driving a considerable amount of research. Aspects discussed in this document include advances in the definition of HTN in 16 year or older, clinical significance of isolated systolic HTN in youth, the importance of out of office and central blood pressure measurement, new risk factors for HTN, methods to assess vascular phenotypes, clustering of cardiovascular risk factors and treatment strategies among others. The recommendations of the present document synthesize a considerable amount of scientific data and clinical experience and represent the best clinical wisdom upon which physicians, nurses and families should base their decisions. In addition, as they call attention to the burden of HTN in children and adolescents, and its contribution to the current epidemic of cardiovascular disease, these guidelines should encourage public policy makers to develop a global effort to improve identification and treatment of high blood pressure among children and adolescents.

Da sich die Wertschöpfungsmechanismen digitaler Services grundlegend von denen physischer Produkte unterscheiden, sind die Preisgestaltung und die Entwicklung nachhaltiger Geschäftsmodelle eine Herausforderung für produzierende Unternehmen. Identifizierung von Wertpotenzial für digitale Services von B2B-Fertigungsunternehmen am Beispiel eines Küchengeräteherstellers

Abstract Background Despite the crucial role of regular physical activity (PA) for preventing chronic non-communicable diseases, fewer than half of older adults in Germany engage in the recommended levels. Objective: The aim of this study was to compare acceptance and effectiveness of two interventions for PA promotion among initially inactive community-dwelling adults aged 60 years and above in a nine-month randomized trial with a cross-over design. Methods Participants were recruited offline and randomized to (a) a print-based intervention (PRINT n = 113) and (b) a web-based intervention (WEB, n = 129). Thirty percent (n = 38) of those in group (b) received a PA tracker in addition to WEB (WEB+, (c)). All intervention groups were offered ten weekly face-to-face group sessions led by trained student assistants. Afterwards, participants could choose to stay in their group or cross over to one of the other groups. Group sessions were continued monthly for another six months. Three-dimensional accelerometers to assess PA at baseline (T0), three-months (T1) and nine-months (T2) were employed. Intervention acceptance was assessed via self-administered paper-based questionnaires. Linear mixed models were used to calculate differences in moderate-to-vigorous PA (MVPA) between time points and intervention groups. Results Of the initially recruited n = 242 participants, n = 91 (37.6%) were randomized to the WEB group, n = 38 (15.7%) to WEB+ and n = 113 (46.7%) to PRINT and n = 195 participants completed T1. Only n = 1 moved from WEB to PRINT and n = 15 from PRINT to WEB (WEB-WEB: n = 103, PRINT-PRINT: n = 76), when offered to cross over at T1. One-hundred and sixty participants completed T2. MVPA in min per day increased between baseline and T1, but these within-group changes in time disappeared after adjusting for covariates. MVPA decreased by 9 min/day between baseline and T2 (βtime = -9.37, 95% CI: [-18.58; -0.16]), regardless of intervention group (WEB vs. PRINT: βgroup*time = -3.76, 95% CI: [-13.33; 5.82], WEB+ vs. PRINT: βgroup*time = 1.40, 95% CI: [-11.04; 13.83]). Intervention acceptance was generally high. Conclusions Despite high levels of acceptance of web- and print-based interventions for PA promotion and little movement between groups at T1, when given the choice, participation was not associated with increases in PA over time.

Zusammenfassung Die Lebensmittelindustrie steht aufgrund komplexer, internationaler Lieferketten, einem gesellschaftlich relevanten Versorgungsauftrag bei gleichzeitig verderblichen Produkten, und steigender Anforderungen durch Endverbraucher:innen an Nachhaltigkeit vor großen Herausforderungen. Die zunehmende Digitalisierung von Wirtschaft und Gesellschaft verschärft diese Problematik, da sie neue Abhängigkeiten schafft und Erwartungen der Endverbraucher:innen weiter erhöht. Im Gegensatz dazu bieten digitale Innovationen aber auch große Potenziale, um die Herausforderungen zu adressieren. So werden digitale Technologien von einzelnen Akteur:innen der Wertschöpfungskette für Lebensmittel einerseits bereits zur inkrementellen Verbesserung von Produktionsprozessen und andererseits für die Schaffung völlig neuer Geschäftsmodelle und Services rund um digitale Einkaufserlebnisse genutzt. Digitale Innovationen auf Ebene einzelner Akteur:innen reichen jedoch nicht aus, um die weitreichenden Herausforderungen der Lebensmittelindustrie vollständig zu bewältigen. Vielmehr gilt es, die stärkere Vernetzung und Konnektivität durch digitale Technologien zu nutzen, um digitale Innovation auf ganzheitlicher Ebene und entlang der gesamten Wertschöpfungskette voranzutreiben. Wie daraus hervorgehende Nutzenpotenziale digitaler Innovation gehoben werden können, ist aus wissenschaftlicher und praktischer Perspektive noch ungeklärt. Dementsprechend ist es das Ziel unserer Arbeit, auf Basis einer Interviewstudie zu untersuchen, welche Nutzenpotenziale digitale Innovationen für einzelne Akteur:innen und die gesamte Wertschöpfungskette für Lebensmittel bereithalten und welche praktischen Handlungsempfehlungen sich daraus für Entscheidungsträger:innen ergeben. Dabei wollen wir darauf eingehen, wie sich Ambidextrie entlang der Wertschöpfungskette entfaltet und wie diese zukünftig bei der Adressierung der Herausforderungen helfen kann. Der vorliegende Beitrag hebt sich somit von bestehenden Arbeiten ab, da wir das Potenzial digitaler Innovation nicht nur aus Perspektive einzelner Akteur:innen, sondern entlang der gesamten Wertschöpfungskette untersuchen.
