Health professionals for a new century: transforming education to strengthen health systems in an interdependent world

The World Health Organization has just released the Global status report on road safety —the first broad assessment that describes the road safety situation in 178 countries, using data drawn from a standardised survey. The results provide a benchmark that countries can use to assess their road safety position relative to other countries, while at the international level these findings can be considered as a “baseline”, against which regional and global level progress can be measured.

The questionnaire used for this survey was developed in consultation with an expert committee of road safety researchers and practitioners. Data collection was carried out using a self-administered questionnaire, the content of which was based on the recommendations of the World report on road traffic injury prevention , developed by WHO, the World Bank and many other partners in 2004. The methodology used involved the identification of a National Data Coordinator in each country who identified up to seven other national road safety experts from multiple sectors who could complete the questionnaire. This paper assesses the impact of mechanisms for financing intensive home health care services in the United States on their utilization. As lengths of stay have decreased in response to prospective payment methods for hospitals, demand has increased for intensive and complex services provided to patients in the home. Third-party payers, however, are willing to satisfy only some of this potential demand that their reimbursement policies have generated. It is the policies of payers rather than the safety and effectiveness of devices and procedures that are the major constraints on the expansion of intensive home health care. We describe the effects of these policies on who receives intensive home health care services, who provides them, what services are provided, how their quality is monitored, and what they cost.

Several recent articles, including the one by Schroeder and colleagues in this issue, document violations of the Americans with Disabilities Act (ADA) by state medical boards that ask applicants for initial licensure and relicensure whether they have any history of mental illness. The authors of these articles argue that the boards' violation of the ADA is a disincentive for applicants to seek treatment, especially for depression. This commentary discusses the behavior of the boards, and efforts to address it, in the larger context of what organizations that educate, examine, license, certify, credential, and represent physicians are doing to improve the accountability of members of the profession and, as a result, the quality of their services.
