The prevalence of obesity is rapidly increasing globally. Epidemiological studies have associated obesity with a range of cancer types, although the mechanisms by which obesity induces or promotes tumorigenesis vary by cancer site. These include insulin resistance and resultant chronic hyperinsulinaemia, increased bioavailability of steroid hormones and localized inflammation. Gaining a better understanding of the relationship between obesity and cancer can provide new insight into mechanisms of cancer pathogenesis.

Acute hepatitis B virus (HBV) infection in immunocompetent adults is usually a self-limited disease. Spontaneous recovery with hepatitis B surface antigen (HBsAg) seroclearance and development of anti-HBs occurs in about 95–99% of cases; however, about 1% of patients progress to fulminant hepatitis with a significantly low survival rate if not treated with orthotopic liver transplantation (OLT). Treatment indications are restricted to life-threatening cases; indeed, the most recent guidelines from the European Association for the Study of the Liver and theAmerican Association for the Study of Liver (AASLD) suggest the use of nucleoside/nucleotide analogs (NUC) for patients with acute liver failure or protracted severe acute hepatitis (prolonged international normalized ratio [INR] and jaundice for >4 weeks). Nevertheless, the safety and efficacy of NUC in this setting has been reported in only a few case series and two randomized controlled trials (RCTs) of lamivudine for patients with severe acute hepatitis B, but strong evidence of benefit is still lacking. Moreover, experiences with new generation NUC are still limited to small case series treated with entecavir. Similarly, few data on the use of telbivudine in acute hepatitis B have been published. Here we report the safety and efficacy of telbivudine treatment in a young patient who was admitted to our unit for acute HBV hepatitis with fulminant hepatic failure. CASE REPORT
