According to W. Edwards Deming, American companies require nothing less than a transformation of management style and of governmental relations with industry. In Out of the Crisis, originally published in 1982, Deming offers a theory of management based on his famous 14 Points for Management. Management's failure to plan for the future, he claims, brings about loss of market, which brings about loss of jobs. Management must be judged not only by the quarterly dividend, but by innovative plans to stay in business, protect investment, ensure future dividends, and provide more jobs through improved product and service. In simple, direct language, he explains the principles of management transformation and how to apply them.

Out of the Crisis

To provide an update to “Surviving Sepsis Campaign Guidelines for Management of Sepsis and Septic Shock: 2012”. A consensus committee of 55 international experts representing 25 international organizations was convened. Nominal groups were assembled at key international meetings (for those committee members attending the conference). A formal conflict-of-interest (COI) policy was developed at the onset of the process and enforced throughout. A stand-alone meeting was held for all panel members in December 2015. Teleconferences and electronic-based discussion among subgroups and among the entire committee served as an integral part of the development. The panel consisted of five sections: hemodynamics, infection, adjunctive therapies, metabolic, and ventilation. Population, intervention, comparison, and outcomes (PICO) questions were reviewed and updated as needed, and evidence profiles were generated. Each subgroup generated a list of questions, searched for best available evidence, and then followed the principles of the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system to assess the quality of evidence from high to very low, and to formulate recommendations as strong or weak, or best practice statement when applicable. The Surviving Sepsis Guideline panel provided 93 statements on early management and resuscitation of patients with sepsis or septic shock. Overall, 32 were strong recommendations, 39 were weak recommendations, and 18 were best-practice statements. No recommendation was provided for four questions. Substantial agreement exists among a large cohort of international experts regarding many strong recommendations for the best care of patients with sepsis. Although a significant number of aspects of care have relatively weak support, evidence-based recommendations regarding the acute management of sepsis and septic shock are the foundation of improved outcomes for these critically ill patients with high mortality.

There is a growing movement in environmental politics and political theory toward what we might call “egalitarian environmentalism.” Egalitarian environmentalists are weak anthropocentrists who extend (cosmopolitan) liberal conceptions of procedural and distributive justice to environmental issues. Typically, egalitarian environmentalists are committed to (at least one of) two general principles. First, they advocate equal substantive environmental rights for all humans. More specific, they often support the idea of equal per capita shares of ecological (or environmental) space. Second, they advocate equal procedural rights for all humans in environmental decision making. In other words, they support public and stakeholder involvement in an active “environmental democracy.” There are, of course, many different conceptions of each of these principles and, therefore, many different versions of egalitarian environmentalism. Most of the contributors to Haugestad and Wulfhorst’s eclectic selection of papers are egalitarian environmentalists of one version or another. They offer quite different perspectives on a wide range of environmental issues and problems—ranging from nuclear waste to genetic modification (GM) science, from tradable fuel permits to ecological building, from solid waste management to illegal logging—but for the most part, their diagnosis of the problem or their solution to it is either more public and stakeholder participation or a more equal distribution of environmental goods and services. This book is the product of an interdisciplinary and multidisciplinary conference held in Copenhagen in February 2003, which was one of an ongoing series of annual conferences on the theme of “Environmental Justice and Global Citizenship” (p. vii). The editors’ introduction sets the tone of the book from the very beginning:

Introduction: Asthma guidelines recognise the importance of correct inhaler technique and patient preference when individualising therapy. 1 A novel breath-triggered inhaler (flutiform ® K-haler ® ) has been developed to deliver fluticasone propionate/formoterol (FP/FORM), currently available in a pMDI. A device handling study was conducted in patients with asthma and/or COPD (NCT01739387). We present preference and ease-of-use data for the FP/FORM K-haler compared to patients9 current ICS/LABA device. Methods: An open-label, randomised, 2-period, crossover study using placebo FP/FORM K-haler and pMDI inhalers. Patients (≥12 years) were trained and assessed for correct device handling then asked to rate the FP/FORM K-haler compared with their current device on ease-of-use and preference. Results: 307/311 patients were included in the per-protocol analyses. The Table shows FP/FORM K-haler ease-of-use and preference data versus 4 comparator inhalers.
 Conclusions: The majority of patients reported the FP/FORM K-haler easier to use and preferred it to their current inhaler. 1. GINA 2015. Sponsor: Mundipharma Research Ltd.
