What can we learn about patient safety from information sources within an acute hospital: a step on the ladder of integrated risk management?

TL;DR: Analysis of seven key data sources indicated that case notes have the potential to identify the largest number of incidents and provide the richest source of information on such incidents, and triangulating information from more than one source can identify a broader range of incidents.
