This article reviews the literature on the use of hydrogen peroxide in three professionally administered bleaching techniques from historical, technique, and safety viewpoints. Safety over time, absolute safety, and relative safety of nonvital bleaching, in-office vital bleaching, nightguard vital bleaching, and over-the-counter bleaching kits are compared. The advantages and disadvantages of different bleaching options, as well as indications for individual or combined use of the techniques, are discussed. In addition, specific indications for the use of the nightguard vital bleaching technique are presented.

The effects of peroxide bleaching on composite to enamel bond strength

Internal bleaching procedures such as the walking bleach technique can be used for whitening of discoloured root-filled teeth. The walking bleach technique is performed by application of a paste consisting of sodium perborate-(tetrahydrate) and distilled water (3% H2O2), respectively, in the pulp chamber. Following a critical review of the scientific literature, heating of the mixture is contra-indicated as the risk of external cervical resorption and the formation of chemical radicals is increased by application of heat. An intracoronal dressing using 30% H2O2 should not be used in order to reduce the risk of inducing cervical resorption. This review provides advice based on the current literature and discusses how the walking bleach technique can lead to successful whitening of non-vital root-filled teeth without the risks of side-effects.

Bleaching with 35 percent hydrogen peroxide causes enamel surface changes, which result in lower bond strengths of composite resin. Although a previous SEM study showed that home bleaching with 10 percent carbamide peroxide does not cause such surface changes, the results of our study indicate that carbamide peroxide bleaching reduces the shear bond strength of composite to etched enamel. Removal of surface enamel, however, restores bond strengths to normal levels.

This paper describes a clinical case in which a combination bleaching technique was used to improve the appearance of the patient's discolored anterior teeth. Enamel microabrasion was used first to remove superficial fluorosis stains. A home bleaching regimen was then used to lighten the overall shade of the teeth, which had appeared more yellow after microabrasion.

Recientemente se han introducido radiomedidores sencillos para calibrar la intensidad de la luz de polimerizacion dental. Este estudio examino la relacion entre las mediciones de intensidad de luz con tres radiomedidores, con la distancia de la punta de polimerizacion a la resina compuesta, y con la polimerizacion de la resina (indicada por su micropureza). Las mediciones de intensidad de luz, que disminuyen conforme aumenta la distancia a la punta de polimerizacion, correlacionaron de forma importante con la microdureza de la resina compuesta. Las intensidades de luz menores produjeron resinas compuestas mas blandas, particularmente en el fondo de los especimenes (2 mm de profundidad). Los diferentes radiomedidores aportaron indicaciones similares de los cambios en la intensidad de la luz a distintas distancias de polimerizacion.
