Maternal lipid levels during pregnancy and gestational diabetes: a systematic review and meta-analysis

BACKGROUND Characterizing ovarian pathology is fundamental to optimizing management in both pre- and post-menopausal women. Inappropriate referral to oncology services can lead to unnecessary surgery or overly radical interventions compromising fertility in young women, whilst the consequences of failing to recognize cancer significantly impact on prognosis. By reflecting on recent developments of new diagnostic tests for preoperative identification of malignant disease in women with adnexal masses, we aimed to update a previous systematic review and meta-analysis. METHODS An extended search was performed in MEDLINE (PubMed) and EMBASE (OvidSp) from March 2008 to October 2013. Eligible studies provided information on diagnostic test performance of models, designed to predict ovarian cancer in a preoperative setting, that contained at least two variables. Study selection and extraction of study characteristics, types of bias, and test performance was performed independently by two reviewers. Quality was assessed using a modified version of the QUADAS assessment tool. A bivariate hierarchical random effects model was used to produce summary estimates of sensitivity and specificity with 95% confidence intervals or plot summary ROC curves for all models considered. RESULTS Our extended search identified a total of 1542 new primary articles. In total, 195 studies were eligible for qualitative data synthesis, and 96 validation studies reporting on 19 different prediction models met the predefined criteria for quantitative data synthesis. These models were tested on 26 438 adnexal masses, including 7199 (27%) malignant and 19 239 (73%) benign masses. The Risk of Malignancy Index (RMI) was the most frequently validated model. The logistic regression model LR2 with a risk cut-off of 10% and Simple Rules (SR), both developed by the International Ovarian Tumor Analysis (IOTA) study, performed better than all other included models with a pooled sensitivity and specificity, respectively, of 0.92 [95% CI 0.88-0.95] and 0.83 [95% CI 0.77-0.88] for LR2 and 0.93 [95% CI 0.89-0.95] and 0.81 [95% CI 0.76-0.85] for SR. A meta-analysis of centre-specific results stratified for menopausal status of two multicentre cohorts comparing LR2, SR and RMI-1 (using a cut-off of 200) showed a pooled sensitivity and specificity in premenopausal women for LR2 of 0.85 [95% CI 0.75-0.91] and 0.91 [95% CI 0.83-0.96] compared with 0.93 [95% CI 0.84-0.97] and 0.83 [95% CI 0.73-0.90] for SR and 0.44 [95% CI 0.28-0.62] and 0.95 [95% CI 0.90-0.97] for RMI-1. In post-menopausal women, sensitivity and specificity of LR2, SR and RMI-1 were 0.94 [95% CI 0.89-0.97] and 0.70 [95% CI 0.62-0.77], 0.93 [95% CI 0.88-0.96] and 0.76 [95% CI 0.69-0.82], and 0.79 [95% CI 0.72-0.85] and 0.90 [95% CI 0.84-0.94], respectively. CONCLUSIONS An evidence-based approach to the preoperative characterization of any adnexal mass should incorporate the use of IOTA Simple Rules or the LR2 model, particularly for women of reproductive age.

Comparison of four malignancy risk indices in the detection of malignant ovarian masses

Minimal information exists on unintended health consequences following childbirth, particularly in relation to mode of delivery. This study aimed to evaluate the impact of mode of delivery on long-term sexual satisfaction of women by using a validated questionnaire. Forty-five primiparous women who had cesarean deliveries and 90 primiparous women who had vaginal deliveries with mediolateral episiotomies enrolled in the study. Quality of sexual relations and sexual satisfaction were self-reported by using Golombock-Rust Inventory of Sexual Satisfaction. Prevalence of sexual dissatisfaction was compared between the two groups and logistic regression analysis was carried out to identify the predictors of sexual dissatisfaction. The prevalence of overall sexual dissatisfaction was 4.4% in cesarean group while it was 14.4% in vaginal delivery group (P=0.081). Vaginal delivery group demonstrated a trend toward higher prevalence of dissatisfaction in all subscales except sensuality area than cesarean group, however, differences between two groups did not reach to statistical significance. Degree of pleasure for relationship with the spouse was identified as a significant predictor for overall sexual dissatisfaction (P=0.008). Our findings suggest that sexual dissatisfaction should not be assumed simply a product of the delivery mode. Individual, socio-demographic, lifestyle and marital characteristics should also be taken into account.

Aim The aim of this study was to evaluate whether the use of lidocaine 10% spray was effective in pain control during hysterosalpingography (HSG). Methods A total of 81 patients were randomly assigned to three groups: Group 1, 10 mg lidocaine hydrochloride 10% spray; Group 2, 20 mg lidocaine hydrochloride 10% spray; and Group 3, Placebo. We used a standard 10 cm Visual Analog Scale for pain scoring. The '0 cm' end corresponded to 'no pain', and the '10 cm' end represented the 'worst pain ever'. We asked the patients to mark their pre-HSG pain expectancies initially, then to mark their pain during uterine traction after the tenaculum has been applied. Finally, they were asked to mark their pain during contrast medium injection. Results In Group1, the mean anticipated pain (AP) score was 64.51 +/- 12.62, the mean pain (MP) score during cervical traction with uterine tenaculum was 57.48 +/- 11.32 (P: 0.011), and the MP score during contrast medium injection was 46.22 +/- 13.00 (P: 0.005). In Group 2 the mean AP score was 66.44 +/- 12.02, the MP score during cervical traction with uterine tenaculum was 48.03 +/- 13.44 (P: 0.001), and the MP during contrast medium injection was 46.25 +/- 11.57 (P: 0.001). In Group 3 the mean AP score was 61.8 +/- 18.5, the MP score during cervical traction with uterine tenaculum was 59.5 (P: 0.07), and the MP during contrast medium injection was 57.5 +/- 14.5) (P: 0.304). Conclusion Topical lidocaine spray is a practical and effective analgesic for decreasing pain perception scores during the HSG procedure. A 10 mg dose is comparable to a 20 mg dose in pain reduction with less chance of side effects and better cost-effectiveness.

Lidocaine 10% spray reduces pain during hysterosalpingography: a randomized controlled trial.

Objective: The aim of this study was to evaluate plasma gamma-glutamyltransferase (GGT) in gestational diabetes mellitus (GDM) in pregnant women at oral glucose tolerance test (OGTT) and the diagnosis of GDM and to explore whether this activity is associated with metabolic parameters. Method: This prospective control study included 37 women with GDM and 42 women with normal glucose tolerance in pregnancy (control group). In the study group (GDM), blood was taken for analyzing 100 g OGTT from women who have abnormal 50 g glucose challenge test (GCT). Results: Compared with the controls, the GDM group had significantly higher mean values for serum fasting glucose, insulin, homeostasis model assessment-insulin resistance (HOMA-IR), triglyceride and GGT. Within the GDM group, GGT levels were only negatively correlated with high-density lipoprotein (r = −0.41, p = 0.01). GGT was determined to be an independent metabolic parameter for GDM.
