Background: Nursing as a clinical discipline is developing in the emergency wards. Health care systems should continuously assess and prioritize indicators of clinical competency in these wards. The lack of clear standards of clinical competency indicators challenges evaluation. The purpose of this study was to determine clinical competency indicators and its priority based on nurses' views in educational and therapeutic centers in Guilan. Materials and Methods: The Q methodology was conducted in three phases, that is, phase I (determining the clinical competency indicators), phase II (classifying clinical competency indicators by an expert panel), and phase III (prioritizing clinical competency indicators). The subjects were selected by convenience sampling among nurses working in the emergency wards of teaching hospitals affiliated to Guilan in 2013. Finally, clinical competency indicators were prioritized using exploratory factor analysis. Results: In the prioritizing phase, data were collected from 710 nurses over two months. Five factors with 30 general competencies were found in three domains: communication, professional maturity, and personality characteristics. Six factors with 37 specific competencies were also found in two domains: scientific and technical capabilities and basic clinical skills that can provide a structured instrument for assessing clinical competence in emergency nurses. Conclusions: Achieved competencies can be used as a reference for nursing education and practice in emergency. Further research on health care system is needed in order to achieve a reliable and valid instrument.

Assessing emergency nurses' clinical competency: An exploratory factor analysis study

The Relationship between Nursing Care Quality and Patients Satisfaction among Hospitals affiliated to Zahedan University of Medical Sciences in 2014

Background & Aim : Nurses’ clinical competence is the most important topics of nursing professional influenced by intrinsic motivational factors in addition to external motivational factors. This study is designed to determine the relationship between clinical competence and motivational factors from McClelland’s theory of needs that include: need for affiliation, need for achievement and need for power. Methods & Materials : This descriptive–correlation survey was performed on 200 nurses in three hospitals of Babol University of Medical Sciences, by simple random sampling method. Data collection tools for this study were included: demographic information, motivational needs questionnaire, and clinical competence questionnaire. Among 200 questionnaires distributed, 175 questionnaires were collected, of which 171 questionnaires completed (response rate 87.5%), this number exceeded the number was defined by sample size calculation as based sample (165 participants). The data after collecting were analyzed, using SPSS, statistical software version 19 with descriptive statistics and Pearson tests, ANOVA test, Scheffe test, and t-test. Results : Based on the results of the survey, there was a significant relationship between clinical competence and the motivation need for affiliation (r = 0.377, P = 0.000), need for achievement (r = 0.291, P = 0.000) and need for power (r = 0.200, P = 0.009). As well as corresponding selfassessment nurses, need for achievement with the highest mean of 45.27 ± 6.39 ranked first, need for affiliation with a mean of 44.36 ± 5.83 ranked second and need for power with a mean of 39.24 ± 7.19 was ranked third. In addition, the results showed significant differences in clinical nurses competence of hospital “C” with a mean of 115 ± 14.97 compared with nurses competence of hospital “A” with a mean of 122.55 ± 11.65 and hospital “B” with a mean of 121.67 ± 9.57. Conclusion : There is a significant relationship between motivational needs and clinical competence, hence that the strongest relationship had existed between need for affiliation and clinical competence and the weakest relationship had existed between need for power and clinical competence, so more attention by nursing managers is essential to provide appropriate feedback to steernurses’ efforts for improving patients’ care.

Relationship between clinical competence and motivation needs of nurses based on the McClelland theory

Background: The medical and nonmedical care of patients is the necessary skills in nursing profession. That it needs proper knowledge and attitude. Hence, it is important to promote nurses' knowledge and attitude by education based on need assessment. This study aimed to define the efficacy of a medical and nonmedical intervention educational workshop on nurses' knowledge and attitude until 3 months after holding the workshop in psychiatric wards of educational hospitals in Isfahan. Materials and Methods: This is a quasi-experimental study. The study population comprised all nurses working in psychiatric wards of Nour and Farabi Hospitals (64) in Isfahan in 2012. An educational workshop was held through educational sessions in the form of lectures and group discussion in two above-mentioned hospitals. Nurses' level of knowledge and attitude were investigated by a researcher made questionnaire before, immediately after and 3 months after intervention. Data were analyzed by descriptive statistical tests of repeated measure ANOVA and Bonferroni. Results: A significant increase was observed in mean scores of nurses' knowledge immediately after and 3 months after education compared to before education (P < 0.001). Also, the mean of attitude score in 3 phases have significant different (P < 0.009). Nurses have the high satisfaction (86.3%) of need assessment based education workshop. Moreover, it is effective in the science information revival of nurses. Conclusion: Educational sessions notably affected the promotion of nurses' knowledge and attitude. With regard to nurses' satisfaction with the workshop which was held, designing and organizing educational workshops based on constant needs assessment is suggested for the promotion of nursing cares.
