Objective: To better define the effectiveness and risks of modern cataract surgery Design: Meta-analysis (formal systematic identification, selection, review, and synthesis) of published literature Patients: Patients described in 90 studies published between 1979 and 1991 that addressed visual acuity (n=17 390 eyes) or complications (n=68 316 eyes) following standard extracapsular cataract extraction with posterior chamber intraocular lens implantation, phacoemulsification with posterior chamber intraocular lens implantation, or intracapsular cataract extraction with flexible anterior chamber intraocular lens implantation Main Outcome Measures: The proportion of eyes with postoperative Snellen visual acuity of 20/40 or better and the proportion of eyes with each of 18 complications Results: The pooled percentage of eyes (weighted by sample size) with postoperative visual acuity of 20/40 or better was 955% (95% confidence interval [CI], 951% to 959%) among eyes without preexisting ocular comorbidity and 897% (95% CI, 893% to 902%) for all eyes The pooled percentage of eyes experiencing complications (weighted by sample size and, when pertinent, by quality score of the individual studies but not adjusted for variation in duration of follow-up) ranged from 013% for endophthalmitis to 197% for posterior capsule opacification Pooled proportions of eyes with other complications were as follows: bullous keratopathy, 03%; intraocular lens malposition/dislocation, 11%; clinically apparent cystoid macular edema, 15%; and retinal detachment, 07% Pooled results for postoperative Snellen visual acuity and most complications were similar for surgery performed via phacoemulsification vs standard extracapsular cataract extraction, although comparisons of the outcomes between these procedures should be interpreted with caution Conclusions: The published literature indicates that modern cataract surgery yields excellent visual acuity and, although not free of complications, is a very safe procedure regardless of the extraction technique used

Synthesis of the Literature on Visual Acuity and Complications Following Cataract Extraction With Intraocular Lens Implantation

Objective To report normal macular thickness measurements in healthy eyes using the latest commercially available optical coherence tomography (OCT) mapping software, version 3.0, from the Stratus OCT (OCT3). Methods Thirty-seven eyes from 37 healthy subjects underwent a complete ophthalmologic examination, including OCT. Six radial scans, 6 mm in length and centered on the fovea, were obtained using the OCT3. Retinal thickness was automatically calculated by OCT mapping software. Measurements were displayed as the mean and standard deviation for each of the 9 regions defined in the Early Treatment Diabetic Retinopathy Study. Results Foveal thickness (mean thickness in the central 1000-μm diameter area) and central foveal thickness (mean thickness at the point of intersection of 6 radial scans) on the OCT3 were 212 ± 20 and 182 ± 23 μm, respectively. Macular thickness measurements were thinnest at the center of the fovea, thickest within 3-mm diameter of the center, and diminished toward the periphery of the macula. The temporal quadrant was thinner than the nasal quadrant. Central foveal thickness was also manually determined as 170 ± 18 μm, approximately 12 μm less than the value automatically obtained from the OCT3 software. There was no correlation between age and foveal thickness ( P  = .80). Conclusions Mean foveal thickness measurements were 38 to 62 μm thicker than previously reported values, while mean central foveal thickness measurements were 20 to 49 μm thicker than previously published values. This discrepancy should be considered when interpreting OCT scans.

Intraocular lenses: complications and visual results.

• The efficacy of a new nonsteroidal anti-inflammatory agent, suprofen, for reducing pupillary constriction during cataract surgery was ascertained in a double-masked, multicenter, clinical study. Prior to surgery 1.0% suprofen or a placebo was instilled; the surgeon's normal regimen of mydriatics and cycloplegics was used. Suprofen (209 patients) was far more effective than the placebo (203 patients) in maintaining a dilated pupil prior to intraocular lens (IOL) implantation (or instillation of a miotic). The mean pupillary area prior to IOL implantation was 6.3 sq mm larger (20% larger) in patients treated with suprofen than in patients receiving the placebo. The investigators' subjective evaluations of the adequacy of pupil size for IOL implantation and of the difficulty of IOL implantation favored patients treated with suprofen over those receiving the placebo.

Intraocular lens (IOL) implantation is now accepted as the most effective means of aphakic correction in selected patients undergoing cataract surgery. In terms of final rehabilitation, however, complications such as corneal oedema, retinal detachment, and cystoid macular oedema remain problematic. Results of prospective concurrent trials of IOL implantation performed at the Wilmer Institute are presented, with particular reference to the posterior chamber intraocular lens and with specific emphasis on postoperative complications.
