This study was performed to predict pregnancy by intrauterine insemination (IUI) using computer-aided sperm analysis (CASA) estimates and strict criteria in patients with male factor infertility IUI was performed in 682 cycles on 160 male factor infertile patients Semen examinations were carried out by CASA and strict criteria before and after sperm preparation using continuous-step density gradient centrifugation Receiver operating characteristics (ROC) curves were constructed for assessment of the effectiveness of each individual parameter in predicting pregnancy by IUI A clinically acceptable threshold was calculated when sensitivity plus specificity were maximum The average cycle of IUI performed was 43 +/- 24 Pregnancy rate per cycle and per patient were 72% (49/682) and 281% (45/160), respectively Using ROC curve, it was shown that normal sperm morphology assessed by the strict criteria before sperm separation and five parameters after sperm separation including rapid, progressive motility, average path velocity (VAP), curvilinear velocity (VCL), and straight line velocity (VSL) were able to predict pregnancy by IUI Correlation between sperm parameters and pregnancy outcome was examined by the logistic regression model In a multivariate analysis normal morphology before sperm separation >or=155% [odds ratio (OR) = 22, p = 002], rapid after sperm separation >or=255% [OR = 39, p = 0029], and VCL after sperm separation >or=10265 microm/sec [OR = 32, p = 0002] were the parameters of predictive value for pregnancy outcome Adjustment of the model for female age, female infertility factors, and the methods of ovulation induction did not change this finding, and the final model still had the same covariates Pregnancy rates per cycle according to the number (0, 1, 2 and 3) of variables satisfied with the three parameters were 0% (0/110), 16% (3/183), 97% (21/217) and 151% (23/151), respectively Three semen parameters including normal morphology before sperm separation, rapid and VCL after sperm separation were identified as predictors of pregnancy by IUI These variables would be helpful when counselling patients before they make the decision to proceed with in vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI)-ET

To increase the fertilizability of sperm for use in artificial insemination with husband's semen, it is effective to concentrate progressively motile sperm from whole ejaculated semen. The simple procedure, a continuous-step density gradient technique, was developed to selectively concentrate progressively motile sperm. The ejaculated semen was placed on 6.0 ml of 80% Percoll solution, and the density gradient was formed by mixing the semen and Percoll with an L-shaped rod for two or three strokes. After centrifugation at 400 x g for 30 min, progressively motile sperm were concentrated in the sediment, whereas the immotile sperm and other types of cells remained in the upper part of the density gradient. In most specimens sperm motility was improved by more than 80% regardless of the original motility. Thus, the fertility index (sperm concentration/ml x percentage of motility x 10(-8)). was increased to 7.7 +/- 3.7 times (n = 15).

Continuous-step density gradient centrifugation for the selective concentration of progressively motile sperm for insemination with husband's semen.

Penetration of human sperm into a underlaying Percoll layer is very effective in isolating forward motile sperm. A plastic funnel, the bottom of which having been plugged with a 1.0-ml disposable syringe, was used as a separation vessel. Ejaculated semen (2.0 ml) was loaded on 2.0 ml of 80% Percoll; after allowing to stand 60 min, forward motile sperm penetrating into the lower part of Percoll layer (1.0 ml) were collected into the syringe. Sperm motility was enhanced to 91.5% +/- 3.3% regardless of the original motility (48.4% +/- 14.6%) with a mean recovery of 34.1% +/- 6.8% (n = 22). Measurement of residual lactate dehydrogenase activity suggested that seminal plasma was excluded almost completely from the purified sperm fraction.
