A Retrospective Analysis of Implantable Loop Recorder in Victoria - Time to Diagnosis of Aetiology, and Correlation with R-Wave Amplitude

As the field of Cardiovascular Genetics has evolved, so has the demand for genetic assessment, counselling, and testing of individuals at risk of an inherited cardiac condition. In 2016, our publicly funded, metropolitan Clinical Genetics Service developed a dedicated Cardiac Genetics Clinic to address this need. We present a summary of this clinic’s evolution since its inception, including the development of a Genetic Counsellor-led Clinic. Between 2016 and 2021, annual referrals to the Cardiac Genetics Clinic doubled (from 48 to 95 per year). Cardiologists provided the majority of all referrals to the service, despite a reduction of internal cardiology referrals during pandemic-affected years (from 19 in 2019 to 9 in 2021). We saw a 5.7-fold increase in referrals from general practitioners (from 7 to 47) and a 4.3-fold increase from other specialists (from 4 to 21). The number of requests for predictive testing increased over time (from 10 to 24), however, consistently accounted for 20% of all referrals. Initially, 73% of annual referrals were for cardiomyopathy, however, this decreased to 34% as referrals for other indications increased. During 2016-2019, annual referrals increased from 1 to 10 for aortopathy, from 6 to 10 for arrhythmia, and from 1 to 4 for familial hypercholesterolaemia. To adapt to increased demand, the Cardiac Genetics Clinic evolved from a Consultant Geneticist-led clinic to a Genetic Counsellor-led clinic. This model required initial investment in upskilling Genetic Counsellors; however, we have found it to be an effective and safe model for cardiac genetics service delivery.
