A method for processing forms in a computer system includes receiving from a human operator a design of a unified from template including data fields and respective attributes. Paper forms and electronic forms (e-forms) are automatically produced based on the unified form template. Filled-in paper forms and filled-in e-forms are accepted from the clients of the computer system, the filled-in forms including data items filled into the respective data fields by the clients. The data items are extracted from the filled-in paper forms and the filled-in e-forms responsively to the unified form template.

Method for forming information providing sheet

PROBLEM TO BE SOLVED: To provide a digital pen capable of changing the method of sending handwritten information depending on the situation. SOLUTION: The digital pen includes a reading part for reading a pattern printed on paper and creating pattern information; a storage part for storing this pattern information; and a sending part for sending the pattern information to a connected server, either directly or via a terminal. The digital pen has a switching part for switching between a streaming mode in which the pattern information read is immediately sent to the server and a storage mode in which the pattern information stored is accumulated until an instruction to send it is inputted. PROBLEM TO BE SOLVED: To provide an entry information processing system for easily and accurately sub-dividing and processing entry information entered by a user in a node which is not divided into a plurality of items based on the intention of the user. SOLUTION: In an entry information processing system 100, a user make an entry in a note 3 by using an electronic pen 10, and the entry information is acquired through a user terminal 25 by a server 5. The server 5 extracts the coordinate data of each stroke from the acquired entry information, and decides whether or not the user has entered a line passing a line confirmation mark set on the node 3 by referring to a coordinate table. When the position coordinates of the line confirmation mark are included in the coordinate data, the server 5 decides that the user has entered the line passing the line confirmation mark, and specifies a paragraph area based on the position coordinates of the line confirmation mark, and stores the entry information entered in the paragraph area as a portion of the node 3 in association with the paragraph area. PROBLEM TO BE SOLVED: To provide a tabulation system capable of forming a tabulation program for tabulating information entered in a questionnaire sheet and automatically performing tabulation using the tabulation program and an electronic pen. SOLUTION: The server 5 forms sheet information composed of layout information, function information and dot pattern information based on questionnaire information acquired from an input terminal 50, and forms a format for an optimized questionnaire sheet 3 based on the sheet information. The input terminal 50 forms the questionnaire sheet 3 by printing the format formed by the server 5 on an exclusive paper. A user performs entry to the questionnaire sheet 3 using the electronic pen 10.
