Serotonin 5-HT2 Receptors Induce a Long-Lasting Facilitation of Spinal Reflexes Independent of Ionotropic Receptor Activity

The management of severe, medically intractable pain is a significant challenge for neurosurgeons and pain management physicians. An existing technique that can effectively alleviate contralateral chronic pain is cordotomy, interruption of the lateral spinothalamic tract of the spinal cord. Since 1912, cordotomy has evolved from a relatively morbid open surgical procedure to a percutaneous radiofrequency procedure with low morbidity. However, since cordotomy is utilized primarily in cancer pain patients, long-term patient follow-up is rare, and the potential duration of analgesia following cordotomy is not known. Here we describe a case with a 41-year follow-up of percutaneous cordotomy for noncancer pain that resulted in over 35 years of complete analgesia, the longest recorded in the literature to date. This case demonstrates that percutaneous cordotomy can provide long-lasting, complete analgesia in some patients and merits continuation as a part of the neurosurgical arsenal of pain therapies.

The World Federation of Neurological Surgeons (WFNS) consists of three society groupings. Group A has five regional associations: North American, Latin American, European, Pan African, and Asian and Australian associations. Group B includes national, regional, multinational neurosurgical societies that include 89 societies participated by 72 countries. Group C has six affiliated organizations, which is the smallest group with 5–20 members (Fig. 1). The Pediatric Neurosurgery Committee of WFNS has played the important role of sponsoring region-based education courses, as well as organizing pediatric neurosurgeryrelated symposiums. Dr. Di Rocco, who serves as the current chair of this committee, has organized many courses and symposiums such as the Asian Australian Advanced Course and Latin American Courses, among others.

A 46-year-old woman was readmitted, two years after removal of a large convexity meningioma, because of cerebrospinal fluid rhinorrhoea from a nasal encephalocele eventually visualized by CT-scan. The encephalocele was extirpated via an intracranial approach, and its histological appearance is described. The aetiology of this extracranial brain tissue protrusion is discussed. It is suggested that the mechanisms behind the formation of this encephalocele are a combination of a congenital malformation and the long term increase of the intracranial pressure due to the meningioma.
