Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) is the “gold standard” procedure for patients with ulcerative colitis (UC) requiring surgical intervention. A de-functioning ileostomy is usually performed, as a step for the IPAA procedure. The aim of this study is to present the methodology and results of the routine double assessment of IPAA integrity in asymptomatic patients prior to the ileostomy reversal and evaluate its necessity. This is a retrospective study of 61 UC patients, who underwent IPAA construction, in 2010–2016. A diverting ileostomy was created after IPAA construction, which was reversed at least 3 months later. A double assessment, with pouchogram and pouchoscopy, of IPAA integrity was performed, before stoma closure. Post-operative symptoms and signs of complications, imaging studies, and endoscopic findings were recorded during follow-up. Prior to the ileostomy reversal, both pouchoscopy and pouchogram identified no patient with evidence of anastomotic leakage. During a mean follow-up of 3.67 years after ileostomy reversal, 11 patients developed complications but only one had signs of leakage, which presented as a pouch-vaginal fistula. The specificity of both the pouchogram and pouchoscopy reached 100% and the negative predictive value ranged between 98.4 and 100%. The specificity of pouchoscopy and pouchogram prior to ileostomy closure, in asymptomatic patients with IPAA for UC, is very high in recognizing an intact anastomosis, but their combination did not alter the diagnostic accuracy or had any effect in further management. At least, pouchogram could be selectively performed only in patients with high-risk clinical indicators. • The double assessment of ileal pouch-anal anastomosis with pouchogram and pouchoscopy, prior to ileostomy closure, specifically in patients with ulcerative colitis has not been evaluated before. • The specificity of pouchoscopy and pouchogram prior to ileostomy closure, in asymptomatic patients with IPAA for UC, is very high in recognizing an intact anastomosis. • However, their combination did not alter the diagnostic accuracy or had any effect in further management, in asymptomatic patients.

Are radiologic pouchogram and pouchoscopy useful before ileostomy closure in asymptomatic patients operated for ulcerative colitis

Cuffitis represents a spectrum of disorders located at the rectal cuff in patients with ileal pouch-anal anastomosis (IPAA). Cuffitis occasionally occurs in patients with prior history of mucosectomy during pouch construction. Classic cuffitis is considered as a form of reminiscent UC. Other causes of cuffitis including Crohn’s disease of the pouch, anastomotic separation due to injury, ischemia, and prolapse. Cuffitis is one of common complications of IPAA. Cuffitis can be classified into: (1) acute (≤ 4 weeks) vs. chronic (> 4 weeks), based on the disease course; and (2) steroid/mesalamine-responsive vs. steroid/mesalamine-dependent vs. steroid/mesalamine-refractory phenotypes, based on the response to the medical therapy. While the majority of patients with cuffitis respond favorably to topical corticosteroids and/or mesalamine therapy, some may show no response. In the latter condition, further evaluation with abdominal imaging and manometry is needed to exclude disease process outside of the cuff and functional disorders of pelvic muscles. Endoscopic intralesional injection of long-acting steroids or anti-tumor necrosis factor agents may be attempted in patients with refractory cuffitis. While almost all pouch cancers originate from the cuff or anal transitional zone, whether chronic cuffitis is a risk factor for colitis-associated neoplasia in the pouch is not known.

Diagnosis and Management of Cuffitis

BACKGROUND: The standard of care for surgical treatment of ulcerative colitis is restorative proctocolectomy with ileal J-pouch. Leaks from the tip of the J-pouch are a known complication, but there is a paucity of literature regarding this type of leak. OBJECTIVE: We aimed to describe the diagnosis, management, and long-term clinical outcomes of leaks from the tip of the J-pouch at our institution. RESULTS: We identified 74 patients with leaks from the tip of the J-pouch. Pain (68.9%) and pelvic abscess (40.9%) were the most common presentations, whereas 10.8% of patients presented with an acute abdomen. The leak was diagnosed by imaging and/or endoscopy in 74.3% of patients but only discovered during surgical exploration in 25.6% of patients. Some 63.5% of patients were diagnosed only after loop ileostomy closure, whereas 32.4% of patients were diagnosed before ileostomy closure. The most common methods used for diagnosis were pouchoscopy (31.1%) and gastrograffin enema (28.4%). A definitive nonoperative approach was attempted in 48.6% of patients but was successful in only 10.8% of patients overall. Surgical repair was attempted in 89.2% of patients, whereas 4.5% of patients had pouch excision. Salvage operations (n = 63) included sutured or stapled repair of the tip of the J (65%), pouch excision with neo-pouch (25.4%), and pouch disconnection, repair, and reanastomosis (9.5%). Ultimately‚ 10 patients (13.5%) required pouch excision, yielding an overall 5-year pouch survival rate of 86.3%. LIMITATIONS: This was a retrospective review; referral bias may limit the generalizability. CONCLUSIONS: Leaks from the tip of the J-pouch have variable clinical presentations and require a high index of suspicion. Die operative Therapie der Colitis ulcerosa ist in allen Fallen mit einer temporaren oder permanenten Ileostomaanlage verbunden. Die haufigsten Konstellationen sind: endstandiges Ileostoma nach Kolektomie, protektives Loop-Ileostoma im Rahmen der Ileumpouch-Anlage und endstandiges Ileostoma bei Unmoglichkeit einen Pouch anzulegen oder zu erhalten. Generell weisen die Colitis-ulcerosa-Patienten mit endstandigem Ileostoma eine sehr gute Lebensqualitat auf, die mit dem Ileumpouch vergleichbar ist. Die Anlage und die Ruckverlagerung des protektiven Ileostomas im Rahmen der Pouchanlage sind dagegen mit uberdurchschnittlichen Morbiditat vergesellschaftet.

Stomachirurgie bei Patienten mit Colitis ulcerosa

BACKGROUND Creation of a J pouch is the gold standard surgical intervention in the treatment of chronic ulcerative colitis (UC). Pouchoscopy prior to ileostomy takedown is commonly performed. We describe the frequency, indication, and findings on pouchoscopy, and determine if pouchoscopy affects rates of complications after takedown. METHODS All UC or indeterminate inflammatory bowel disease patients with a J pouch were retrospectively evaluated from January 1994 to December 2014. Cases were defined as having routine (asymptomatic) pouchoscopy after pouch creation but before ileostomy takedown. Controls were defined as having no pouchoscopy or pouchoscopy on the same day as that of takedown. RESULTS The study included 178 patients (81.5% cases, 18.5% controls). Fifty two percent of pouchoscopies were reported as normal. Common abnormal endoscopy findings included stricture (35%), pouchitis (7%), and cuffitis (0.7%). Length of stay during takedown hospitalization was shorter for cases than controls (3 vs. 5 days; p = 0.001), but neither short- nor long-term complications were statistically different between cases and controls. Abnormalities on pouchoscopy were not predictive for short-term complications (p = 0.73) or long-term complications (p = 0.55). Routine pouchoscopy did not delay takedown surgery in any of the included patients. CONCLUSIONS Routine pouchoscopy may not be necessary prior to ileostomy takedown; its greatest utility is in patients with suspected pouch complications.
